Özet

Giriş ve Amaç: Primer kutanöz T hücreli lenfomaların en sık alt tipi olan Mikozis Fungoides (MF), erken evrelerde inflamatuar dermatozları taklit edebilmesi ve özgün bir tanı testinin bulunmaması nedeniyle gecikmiş tanı oranı yüksek olan bir hastalıktır. MF'de T hücre klonalitesinin gösterilmesi tanıda kritik olup, PCR ve yeni nesil dizileme teknikleri yüksek duyarlılığa rağmen maliyet ve erişilebilirlik açısından sınırlılıklar taşır. TRBC1 antikorunun, T hücre klonalitesini göstermedeki potansiyeli flow sitometride gösterilmiş olsa da, yalnızca taze dokuda uygulanabilir olması kullanımını kısıtlamaktadır. Bu çalışmada TRBC1 immünohistokimyasının, MF ile inflamatuar dermatozların ayırıcı tanısındaki tanısal değerinin ve histopatolojik parametrelerle ilişkisinin, QuPath tabanlı dijital analiz ile araştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmamızda, 2015–2023 yılları arasında Tıbbi Patoloji arşivinden seçilen 59 MF ve 47 inflamatuar dermatit olgusuna ait punch biyopsiler histomorfolojik ve immünohistokimyasal olarak değerlendirildi. Tüm vakaların H&E kesitleri histopatolojik parametreler açısından sınıflandırıldı; CD3 ve TRBC1 immünohistokimyasal boyamaları ise QuPath 0.6.0 yazılımında epidermis, dermis ve total alanlarda dijital olarak analiz edildi. Her olgu için TRBC1/CD3 oranı hesaplanarak literatürde bildirilen cut-off değerlerine göre klonalite (≤25 veya ≥70 monoklonal; %26–69 arası poliklonal) sınıflaması yapıldı ve bu oranların hem MF– inflamatuar dermatit grupları hem de histopatolojik değişkenlerle ilişkisi istatistiksel testlerle incelendi. Elde edilen tüm veriler Number Cruncher Statistical System (NCSS) 2020 istatistik yazılımı kullanılarak analiz edilmiş ve anlamlılık düzeyi p<0.05 olarak kabul edilmiştir. Bulgular: Çalışmamızda lokalizasyon açısından MF olgularında gövde tutulumu anlamlı olarak daha sık, ekstremite tutulumu ise inflamatuar dermatitlerde daha yaygın bulunmuştur (p=0.010). Histopatolojik incelemede MF grubunda papiller dermal infiltrasyon, belirgin lenfosit atipisi, halo varlığı, pagetoid yayılım ve papiller fibroplazi anlamlı olarak daha yüksek; buna karşılık akantoz inflamatuar dermatitlerde belirgin şekilde daha sık saptanmıştır (p=0.005). TRBC1/CD3 oranına dayalı klonalite analizi total, epidermal ve dermal düzeylerde MF lehine anlamlı farklılık göstermiştir (her üçü için p=0.001). MF grubunda TRBC1 klonalitesi histomorfolojik parametrelerle büyük ölçüde ilişkili bulunmamış olup, yalnızca klonalite varlığı daha düşük akantoz ile anlamlı ilişkili bulunmuştur (p=0.034). Sonuç: TRBC1 immünohistokimyasal klonalite analizi, MF lehine anlamlı klonalite artışı göstererek, PCR ve yeni nesil dizileme yöntemlerine düşük maliyetli, erişilebilir ve pratik bir tamamlayıcı tanısal seçenek sunmaktadır. Klinik ve histomorfolojik bulgularla birlikte değerlendirildiğinde TRBC1 immunhistokimyası, özellikle erken evre MF'nin inflamatuar dermatozlardan ayrımında tanısal güvenilirliği belirgin şekilde güçlendirmektedir.

Introduction and Aim: Mycosis fungoides (MF), the most common subtype of primary cutaneous T-cell lymphoma, is a disease with a high rate of delayed diagnosis due to its ability to mimic inflammatory dermatoses in the early stages and the lack of a specific diagnostic test. Demonstrating T-cell clonality is critical for the diagnosis of MF; however, PCR and next-generation sequencing techniques, despite their high sensitivity, have limitations in terms of cost and accessibility. Although the potential of the TRBC1 antibody to demonstrate T-cell clonality has been shown in flow cytometry, its applicability is limited to fresh tissue only. This study aimed to investigate the diagnostic value of TRBC1 immunohistochemistry in the differential diagnosis of MF and inflammatory dermatoses and its relationship with histopathological parameters using QuPath-based digital analysis. Materials and Methods: In our study, punch biopsies from 59 MF and 47 inflammatory dermatitis cases selected from the Medical Pathology archive between 2015 and 2023 were evaluated histomorphologically and immunohistochemically. H&E sections of all cases were classified according to histopathological parameters; CD3 and TRBC1 immunohistochemical stains were digitally analyzed in the epidermis, dermis, and total areas using QuPath 0.6.0 software. The TRBC1/CD3 ratio was calculated for each case and classified according to the cut-off values reported in the literature (≤25 or ≥70 monoclonal; 26–69% polyclonal). The relationship between these ratios and both the MF–inflammatory dermatitis groups and histopathological variables was examined using statistical tests. All data obtained were analyzed using the Number Cruncher Statistical System (NCSS) 2020 statistical software, and a significance level of p<0.05 was accepted. Results: In our study, in terms of localization, MF cases were significantly more common in the trunk, while extremity involvement was more common in inflammatory dermatitis (p=0.010). Histopathological examination revealed that papillary dermal infiltration, marked lymphocyte atypia, halo presence, pagetoid spread, and papillary fibroplasia were significantly higher in the MF group; conversely, acanthosis was significantly more common in inflammatory dermatoses (p=0.005). Clonality analysis based on the TRBC1/CD3 ratio showed significant differences in favor of MF at the total, epidermal, and dermal levels (p=0.001 for all three). In the MF group, TRBC1 clonality was not significantly associated with histomorphological parameters; only the presence of clonality was significantly associated with lower acanthosis (p=0.034). Conclusion: TRBC1 immunohistochemical clonality analysis demonstrates a significant increase in clonality in favor of MF, offering a low-cost, accessible, and practical complementary diagnostic option to PCR and next-generation sequencing methods. When evaluated in conjunction with clinical and histomorphological findings, TRBC1 immunohistochemistry significantly enhances diagnostic reliability, particularly in distinguishing early-stage MF from inflammatory dermatoses.
