Özet
Amaç: Kolorektal kanser tedavisinde cerrahi yaklaşımların sistemik inflamatuvar yanıt üzerindeki etkisi, postoperatif sonuçlar ve onkolojik prognoz açısından kritik öneme sahiptir. Sistemik İmmun İnflamasyon İndeksi (SII), immünositten türetilmiş oranlar arasında prognostik değeri en güçlü ve bütüncül biyobelirteçlerden biri olarak öne çıkmaktadır. Literatürde SII'nin malignitelerde prognoz ile ilişkisi yaygın olarak araştırılmış olsa da, rektum kanseri cerrahisinde farklı cerrahi yaklaşımların (minimal invaziv vs. açık) postoperatif SII düzeyleri üzerindeki etkisini doğrudan inceleyen bir çalışma bulunmamaktadır. Bu çalışmanın temel amacı, minimal invaziv (Robotik ve Laparoskopik) ve açık cerrahi yöntemlerin, rektum kanseri hastalarının postoperatif Sistemik İmmun İnflamasyon İndeksi (SII) değerleri üzerindeki etkilerini karşılaştırmaktır. Gereç ve Yöntem: Çalışma, bir üniversite hastanesinde dört yıllık dönemde rektum kanseri tanısıyla opere edilen 501 hastanın verilerinin retrospektif olarak incelenmesiyle gerçekleştirilmiştir. Hastalar uygulanan cerrahi yönteme göre minimal invaziv (Robotik/Laparoskopik) ve açık cerrahi gruplarına ayrılmıştır. Gruplar patolojik evre ve postoperatif inflamatuar belirteçler (SII, NLR, PLR) açısından karşılaştırılmış, ayrıca SII, NLR ve PLR için ROC analizleri ile cerrahi yöntemleri öngörme performansları değerlendirilmiştir. Bulgular: Minimal invaziv cerrahi grubuna kıyasla, açık cerrahi uygulanan grupta istatistiksel olarak anlamlı düzeyde daha ileri evre tümör (T3, T4 evre oranları ve N2 evre) saptanmıştır. İnflamatuvar belirteçler incelendiğinde, açık cerrahi uygulanan hastalarda postoperatif PLT, NEU ve en önemlisi SII değerleri anlamlı ölçüde yüksek bulunmuştur (Sırasıyla p < 0.00, p = 0.046, p = 0.004). Minimal invaziv yaklaşımlar içinde ise Robotik cerrahi grubunda postoperatif SII değerinin en düşük olduğu saptanmıştır (p < 0.001). ROC analizleri, SII değerinin (Cut-off: 4291.2), açık cerrahi uygulanan hastaları öngörmede diğer indekslere kıyasla daha güçlü bir performans sergilediğini göstermiştir (AUC: %58.3) ix Sonuç: Bu çalışma, rektum kanseri cerrahisinde minimal invaziv yaklaşımların, açık cerrahiye göre postoperatif sistemik inflamatuvar yanıtı (SII) anlamlı düzeyde azalttığını biyokimyasal olarak kanıtlayan ilk serilerden biridir. Özellikle robotik cerrahide saptanan en düşük SII değerleri, bu yöntemin daha az doku travması oluşturduğunu ve immün sistemi daha iyi koruduğunu desteklemektedir. Düşük postoperatif SII, daha iyi immün denge ve potansiyel olarak daha hızlı iyileşme ile ilişkilidir. SII, postoperatif inflamasyonun ve cerrahi travmanın etkilerini değerlendirmede pratik, objektif ve yüksek prognostik değere sahip bir biyobelirteç olarak minimal invaziv cerrahinin klinik üstünlüğünü desteklemektedir. Gelecekte, SII'nin klinik sonuçlarla doğrudan ilişkisini kanıtlamak amacıyla prospektif, çok merkezli ve uzun dönem takipli çalışmalara ihtiyaç vardır.

Aim: The effect of surgical approaches on systemic inflammatory response in the treatment of colorectal cancer is of critical importance for postoperative outcomes and oncological prognosis. The Systemic Immune-Inflammation Index (SII) stands out as one of the most robust and holistic prognostic biomarkers among immune-derived ratios. Although the relationship between SII and prognosis in malignancies has been widely investigated in the literature, there is no direct study examining the effect of different surgical approaches (minimally invasive vs. open) on postoperative SII levels in rectal cancer surgery. The main objective of this study is to compare the effects of minimally invasive (Robotic and Laparoscopic) and open surgical techniques on the postoperative Systemic Immune-Inflammation Index (SII) values in patients with rectal cancer. Materials and Methods: This retrospective study analyzed data from 501 patients who underwent surgical treatment for rectal cancer at a university hospital over a fouryear period. Patients were divided into minimally invasive (Robotic/Laparoscopic) and open surgery groups based on the surgical method applied. The groups were compared in terms of pathological stage and postoperative inflammatory markers (SII, NLR, PLR), and ROC analyses were performed to evaluate the predictive performance of SII, NLR, and PLR for the surgical method. Findings: Compared to the minimally invasive surgery group, the open surgery group had a statistically significantly higher rate of advanced-stage tumors (T3, T4 stage and N2 stage). Regarding inflammatory markers, postoperative PLT, NEU, and most significantly, SII values were found to be remarkably higher in patients who underwent open surgery (P values: p < 0.001, p = 0.046, p = 0.004, respectively). Among minimally invasive approaches, the Robotic surgery group had the lowest postoperative SII value (p < 0.001). ROC analyses indicated that the SII value (Cut-off: 4291.2) showed a better predictive performance than other indices in predicting patients who underwent open surgery (AUC: 58.3%). xi Conclusion: This study is one of the first series to biochemically prove that minimally invasive approaches in rectal cancer surgery significantly reduce the postoperative systemic inflammatory response (SII) compared to open surgery. The lowest SII values found particularly in robotic surgery cases suggest that its technological superiorities contribute to the preservation of immune function by further reducing surgical trauma. A lower postoperative SII is associated with better immune balance and potentially a faster recovery process. SII serves as a practical, objective, and highly prognostic biomarker to assess postoperative inflammation and the impact of surgical trauma, supporting the clinical advantages of minimally invasive surgery. Prospective, multicenter studies with long-term follow-up are needed to further confirm the prognostic value of SII and its association with clinical outcomes.
