Özet

Lyme hastalığı, küresel bir halk sağlığı sorunu haline gelen ve kene kaynaklı Borrelia burgdorferi sensu lato kompleksine ait spiroketlerin neden olduğu zoonotik bir enfeksiyondur. Hastalık farklı klinik evreler şeklinde seyrederken, tanıda genellikle anti-Borrelia IgM ve IgG antikorlarına yönelik serolojik testler kullanılmaktadır. Ancak kullanılan enzim işaretli immun test (EIA) ve blot esaslı testlerin duyarlılık ve özgüllük sorunları olup, tanısal güvenilirlik açısından önemli farklılıklar gösterebilmektedir. Bu nedenle tarama ve doğrulama testinden oluşan iki basamaklı tanı algoritmaları kullanılmaktadır. Bu çalışmada, farklı hasta gruplarında Lyme antikor pozitifliğini ve çapraz reaksiyonları değerlendirmek, klasik (EIA ve blot testi) ve modifiye (çift EIA testi) serolojik tanı algoritmalarını karşılaştırmak amaçlanmıştır. Ülkemizde tanı algoritmalarının karşılaştırılması ve özgüllük analizleri konularında yeterli çalışma bulunmaması nedeniyle elde edilen verilerin Lyme hastalığının tanısal süreçlerine katkı sağlaması beklenmektedir. Lyme hastalığının serolojik tanısında kullanılan klasik ve modifiye test algoritmalarının karşılaştırılması amacı ile Dokuz Eylül Üniversitesi Hastanesi'ne artrit/artralji şikayetiyle başvuran 485 hasta ve kontrol grubu olarak kan bankasından temin edilen 150 serum örneği incelenmiştir. Ayrıca, EIA testlerindeki olası çapraz reaksiyonları değerlendirmek amacıyla, parvovirüs, sifiliz, HIV, CMV, EBV, ANA ve RF testleri pozitif serumlar çalışmaya dahil edilmiştir. Serolojik testler, VIDAS® Lyme IgM/IgG (BioMerieux, St. Louis, Mo.), Anti-Borrelia Select IgM/IgG ELISA (Euroimmun, Almanya) ve Anti-Borrelia IgM ELISA ile Anti-Borrelia plus VlsE IgG ELISA (Euroimmun, Almanya) kitleri kullanılarak gerçekleştirilmiştir. EIA testleri ile şüpheli veya pozitif olarak değerlendirilen örnekler, Anti-Borrelia EUROLİNE-RN-AT IgM/IgG (Euroimmun, Almanya) kitleri ile immunblot yöntemi kullanılarak doğrulanmıştır. İmmunblot değerlendirmesi, üretici firma kriterlerinin yanı sıra A.B.D. Hastalık Kontrol ve Korunma Merkezleri'nin (CDC) önerileri doğrultusunda gerçekleştirilmiştir. Özgüllük çalışmasına dahil edilen yedi farklı hastalık grubundan 130 hasta serumu, çapraz pozitifliği değerlendirmek amacıyla, çalışmada kullanılan her üç EIA (VIDAS, Euroimmun Select ve Euroimmun Plus) ile çalışılmıştır. Sonuçlar değerlendirildiğinde kitler arasında belirgin farklar gözlenmiştir. Tek başına kullanıldığında en az yalancı pozitiflik (n: 12) VIDAS testleri ile elde edilmiştir. Anti-Borrelia IgM antikorları için sadece OspC antijenini içeren Euroimmun Select (n: 37), IgG için ise tam hücre lizatına dayalı antijenler içeren Euroimmun Plus kiti (n:33) en yüksek çapraz pozitiflik veren test olmuştur. İncelenen gruplar arasında kullanılan kite göre değişkenlik olmakla birlikte en fazla yalancı pozitiflik bir diğer spiroket enfeksiyonu olan sifiliz grubunda görülmüştür. EIA ile reaktif bulunan 42 örnekte IgM, 35 örnekte IgG antikorlarını doğrulama amacıyla immunblot testi uygulanmıştır. IgM blot analizi, üretici firma ve CDC kriterleri ile değerlendirildiğinde bir fark saptanmamış, 12 örnekte (%28,5) pozitiflik elde edilmiştir. IgG için ise üretici firma kriterleri ile 8 örnek (%22,8) pozitif saptanırken CDC kriterlerine göre tüm örnekler negatif bulunmuştur. Artrit/artralji ile başvuran 485 hastada ilk basamakta tarama amaçlı VIDAS IgG çalışılmış ve 3 hastada pozitiflik saptanmıştır. İmmunblot doğrulamasında tümü negatif olarak değerlendirilmiştir. Eklem tutulumunun Lyme hastalığının geç döneminde olması nedeniyle IgM analizi yapılmamıştır. Artrit/artralji grubunda gerçek pozitif olgu saptanmaması çalışmanın önemli bir kısıtlılığı olmuştur. Çalışma, Lyme hastalığının serolojik tanısında kullanılan EIA kitlerinde yalancı pozitifliklerin özellikle IgM analizinde önemli bir sorun olabileceğini ve testlerin antijenik içeriğine göre sonuçların değiştiğini göstermiştir. Yalancı reaksiyonlardan IgM için OspC antijeninin, IgG için tam hücre lizatı antijenlerinin sorumlu olabileceği belirlenmiştir. Standart iki basamaklı algoritmada doğrulama için kullanılan immunblot testi ile de sorun devam etmiş ve IgM için yalancı reaksiyonlar görülmüştür. IgG için immunblot testi değerlendirmesinin CDC kriterleri ile yapılması, yalancı reaksiyonları dışlamayı sağlamıştır. Modifiye iki basamaklı algoritmada ise hem IgM hem IgG için VIDAS testinin ilk basamak olması, ikinci EIA olarak Euroimmun plus testinin kullanılması en uygun kombinasyon olmuştur. Yalancı pozitifliklerin en aza indirilmesinde IgM testinde modifiye algoritmanın, IgG testinde ise standart algoritmanın daha başarılı olduğu saptanmıştır. Sonuçlarımız, Lyme hastalığı için serolojik testlerde yalancı pozitiflikler olduğunu, tanı koymada hastanın öykü ve kliniğinin de değerlendirmesinin önemini ve IgM testinin sadece erken dönem (ilk 30 gün) enfeksiyon şüphesinde kullanılması gerektiği yönündeki literatür bilgilerini desteklemektedir. Modifiye tanı algoritmasının doğruluk oranını artırma potansiyeline sahip olduğu, ancak testlerin özgüllüğünün geliştirilmesi gerektiği saptanmıştır.

Lyme disease is a zoonotic infection caused by spirochetes belonging to the Borrelia burgdorferi sensu lato complex, which has become a global public health problem. While the disease progresses in different clinical stages, serological tests targeting anti-Borrelia IgM and IgG antibodies are generally used in diagnosis. However, the enzyme-linked immunoassay (EIA) and blot-based tests used have sensitivity and specificity problems and may show significant differences in terms of diagnostic reliability. For this reason, two-step diagnostic algorithms consisting of screening and confirmation tests are used. In this study, it was aimed to evaluate Lyme antibody positivity and cross-reactions in different patient groups, and to compare classical (EIA and blot test) and modified (dual EIA test) serological diagnostic algorithms. Since there are not enough studies in our country on the comparison of diagnostic algorithms and specificity analyses, the data obtained are expected to contribute to the diagnostic processes of Lyme disease. In order to compare the classical and modified test algorithms used in the serological diagnosis of Lyme disease, serum samples from 485 patients who presented to Dokuz Eylül University Hospital with complaints of arthritis/arthralgia and 150 control serum samples obtained from the blood bank were examined. In addition, to evaluate possible cross-reactions in EIA tests, serum samples that tested positive for parvovirus, syphilis, HIV, CMV, EBV, ANA, and RF were included in the study. Serological tests were performed using the following kits: VIDAS® Lyme IgM/IgG (BioMerieux, St. Louis, Mo.), Anti-Borrelia Select IgM/IgG ELISA (Euroimmun, Germany), and Anti-Borrelia IgM ELISA with Anti-Borrelia plus VlsE IgG ELISA (Euroimmun, Germany). Samples that were evaluated as suspicious or positive by EIA tests were confirmed using the immunoblot method with Anti-Borrelia EUROLINE-RN-AT IgM/IgG (Euroimmun, Germany) kits. The immunoblot evaluations were performed according to the manufacturer's criteria as well as the recommendations of the U.S. Centers for Disease Control and Prevention (CDC). A total of 130 patient sera from seven different disease groups included in the specificity study were tested with all three EIA kits used in the study (VIDAS, Euroimmun Select, and Euroimmun Plus) to evaluate cross-reactivity. Upon evaluation of the results, significant differences were observed between the kits. When used alone, the VIDAS tests yielded the lowest number of false positives (n: 12). The Euroimmun Select kit, which includes only the OspC antigen for detecting anti-Borrelia IgM antibodies, showed the highest cross-reactivity (n: 37); for IgG, the Euroimmun Plus kit, based on whole-cell lysate antigens, produced the highest rate of false positives (n: 33). Although it varied depending on the kit used among the study groups, the highest rate of false positivity was observed in the syphilis group, which is another spirochetal infection. Immunoblot testing was performed on 42 EIA-reactive samples for IgM and 35 samples for IgG antibodies to confirm the results. When the IgM blot analysis was evaluated according to both the manufacturer's and CDC's criteria, no differences were observed, and 12 samples (28.5%) were found to be positive. For IgG, 8 samples (22.8%) were found to be positive according to the manufacturer's criteria, while all samples were found to be negative according to the CDC criteria. In the 485 patients presenting with arthritis/arthralgia, VIDAS IgG was used as the initial screening test, and three patients were found to be positive. However, all were evaluated as negative upon immunoblot confirmation. Since joint involvement occurs in the late stage of Lyme disease, IgM analysis was not performed. The absence of true positive cases in the arthritis/arthralgia group has been an important limitation of the study. The study demonstrated that false positivity in EIA kits used for the serological diagnosis of Lyme disease can be a significant issue, particularly in IgM analysis, and that test results vary depending on the antigenic content. It was determined that OspC antigen may be responsible for false reactions in IgM, while whole-cell lysate antigens may contribute to false positivity in IgG. The problem persisted even with the immunoblot test used for confirmation in the standard two-step algorithm, as false IgM reactions were also observed. For IgG, evaluating immunoblot results according to CDC criteria helped eliminate false positivity. In the modified two-step algorithm, using the VIDAS test as the first step for both IgM and IgG, followed by the Euroimmun Plus test as the second EIA, was found to be the most appropriate combination. The modified algorithm was found to be more effective in minimizing false positivity for IgM, while the standard algorithm was more successful for IgG testing. Our results support the existing literature indicating that false positivity can occur in serological tests for Lyme disease, that patient history and clinical presentation are essential in making a diagnosis, and that IgM testing should be used only when early-stage infection (within the first 30 days) is suspected. It was also determined that the modified diagnostic algorithm has the potential to improve diagnostic accuracy; however, further development is needed to enhance the specificity of the tests.
