Özet

Giriş-Amaç: Prostat kanseri, erkeklerde en sık rastlanan ikinci malignite olup, cerrahi tedavide robotik yardımlı laparoskopik radikal prostatektomi (RYLRP) günümüzde en sık tercih edilen yöntem haline gelmiştir. RYLRP, derin trendelenburg pozisyonu ve CO₂ pnömoperitoneumu gerektirmesi nedeniyle, serebral hemodinamik üzerinde belirgin fizyolojik değişikliklere yol açabilmektedir. Bu değişiklikler, özellikle ileri yaş hastalarda serebral otoregülasyon mekanizmasını zorlayarak hipoperfüzyon ve buna bağlı nörolojik komplikasyon riskini artırabilir. Serebral oksijenasyonun intraoperatif olarak değerlendirilmesi, bu tür komplikasyonların önlenmesinde kritik bir öneme sahiptir. Yakın kızılötesi spektroskopi (Near-Infrared Spectroscopy – NIRS), frontal bölgede yer alan sensörler aracılığıyla bölgesel serebral oksijen saturasyonunu (rSO₂) noninvaziv ve sürekli olarak monitörize etme imkânı sunan, güncel ve güvenilir bir teknolojidir. NIRS, özellikle periferik oksijen satürasyonunun yeterli göründüğü ancak serebral hipoksinin gelişebileceği durumlarda erken tanı sağlayarak klinik yönetimi yönlendirmede önemli katkılar sunmaktadır. Çalışmamızın birincil amacı, robotik yardımlı laparoskopik prostatektomi uygulamalarının ve operasyon sürecindeki pozisyon değişikliklerinin serebral perfüzyona etkisini NIRS ile izlenmesidir. Çalışmamızın ikincil amacı ise robotik cerrahinin serebral perfüzyon açısından güvenirliğinin araştırılmasıdır. Gereç-Yöntem: Çalışmamıza etik kurul onayı alındıktan sonra 65 yaş üstü, ASA II-III risk grubu 68 hasta dâhil edildi. Hastaların yaş, boy, kilo, vücut kitle indeksi (VKİ), eşlik eden komorbiditeler, düzenli kullanılan ilaçlar, ASA skoru ve eğitim düzeyi ameliyathane girişinde kaydedilmiştir. Standart ASA monitörizasyonuna ek olarak, serebral oksijenasyonu değerlendirmek amacıyla bilateral frontotemporal yerleşimli NIRS sensörleri ve anestezi derinliği için BİS monitörü kullanılmıştır. Bazal veriler dahil tüm hemodinamik ve metabolik parametreler sistematik olarak izlenmiştir. Anestezi indüksiyonu preoksijenizasyon sonrası midazolam, fentanil, lidokain, propofol ve rokuronyum ile gerçekleştirilmiş; entübasyon sonrası anestezi idamesi sevofluran ve remifentanil infüzyonuyla, BİS hedefi 40–60 ve MAC 0,8–1,0 aralığında olacak şekilde sağlanmıştır. İntraabdominal basınç 12 mmHg ile sınırlı tutulmuştur. SAB, DAB, OAB, kalp hızı, SpO₂, BIS, NIRS (sağ ve sol), EtCO₂, PaO₂, PaCO₂ ve hemoglobin gibi parametreler; bazal, anestezi indüksiyonu sonrası, cerrahi başlangıcı, insüflasyon sonrası, trendelenburg pozisyonu (30°) sonrası ve her 30 dakikada bir olmak üzere; ayrıca supine dönüş, cerrahi sonu ve anestezi sonrası dönemlerde kaydedilmiştir. Ek olarak, trendelenburg süresi, total cerrahi süresi, anestezi süresi, intraoperatif kanama miktarı kayıt altına alınmıştır. Bulgular: Hastaların yaş ortalaması 69.00±3.59 yıl olup, yaş aralığı 65 ile 80 yıl arasında değişmektedir. Steep trendelenburg pozisyonu sonrası 30. dakikada ölçülen sağ NIRS değerlerinin, giriş (bazal) değerlerine göre daha düşük olduğu tespit edilmiştir (p<0.05). Diğer zaman dilimlerinde sağ NIRS değerlerinde anlamlı bir fark bulunmamıştır. Steep trendelenburg pozisyonu sonrası 30. ve 60. dakikalarda ölçülen sol NIRS değerlerinin, giriş değerlerine göre daha düşük olduğu saptanmıştır (sırasıyla p<0.05, p<0.01). Diğer zaman dilimlerinde sol NIRS değerlerinde anlamlı bir değişiklik bulunmamıştır. Hastaların yaşları ile sağ NIRS değişim değerleri arasında pozitif yönlü bir korelasyon saptanmıştır (r=0.295, p<0.05). Bu bulguya göre, hastaların yaşları arttıkça, sağ NIRS pnömoperitoneum sonrası - giriş değerleri, sağ NIRS steep trendelenburg 0. dk - giriş değerleri arasındaki farkın arttığı görülmüştür. Ancak, hastaların yaşları ile sol NIRS değişim değerleri arasında herhangi bir korelasyon bulunmamıştır (p>0.05). Sonuç: Çalışmamızda, serebral oksijenasyon değerlerinin stabil tutulmasının serebrovasküler komplikasyonların önlenmesine katkı sağladığını göstermektedir. Özellikle yaşlı hastalarda serebrovasküler otoregülasyon bozulma riski arttığı için, bu hastalar için sürekli ve dikkatli bir serebral oksijenasyon izlemi gerekliliği ortaya çıkmaktadır. İntraoperatif serebral oksijenasyon takibi, hemodinamik parametrelerin doğru yönetimi ve uygun cerrahi pozisyonlama ile birlikte, serebral güvenliği etkin bir şekilde sağlamaktadır. Çalışmamız, NIRS teknolojisinin intraoperatif serebral oksijenasyon takibinde güvenli ve etkili bir yöntem olduğunu desteklemektedir.

Introductıon and Aim: Prostate cancer is the second most common malignancy in men, and robotic-assisted laparoscopic radical prostatectomy (RALRP) has become one of the most frequently preferred methods in surgical treatment today. Due to the requirement of deep trendelenburg positioning and CO₂ pneumoperitoneum, RALRP can lead to significant physiological changes in cerebral hemodynamics. These changes, particularly in older patients, may challenge the cerebral autoregulation mechanism, increasing the risk of hypoperfusion and associated neurological complications. Intraoperative assessment of cerebral oxygenation is crucial in preventing such complications. Near-infrared spectroscopy (NIRS) is a contemporary and reliable technology that allows non-invasive and continuous monitoring of regional cerebral oxygen saturation (rSO₂) through sensors placed on the frontal region. NIRS provides early detection in situations where peripheral oxygen saturation appears sufficient, but cerebral hypoxia may develop, offering valuable contributions to clinical management. The primary aim of this study is to monitor the effects of robotic-assisted laparoscopic prostatectomy procedures and position changes during surgery on cerebral perfusion using NIRS. The secondary aim is to evaluate the reliability of robotic surgery in terms of cerebral perfusion. Materials and Methods: After obtaining ethical committee approval, 68 patients aged 65 and above, classified as ASA II-III risk, were included in our study. The patients' age, height, weight, body mass index (BMI), concomitant comorbidities, regularly used medications, ASA score, and educational level were recorded upon entry to the operating room. In addition to standard ASA monitoring, bilateral frontotemporal NIRS sensors were used to assess cerebral oxygenation, and a BIS monitor was employed to monitor anesthesia depth. All hemodynamic and metabolic parameters, including baseline data, were systematically monitored. Anesthesia induction was performed with pre-oxygenation, followed by midazolam, fentanyl, lidocaine, propofol, and rocuronium; after intubation, anesthesia maintenance was provided with sevoflurane and remifentanil infusion, ensuring a BIS target of 40–60 and MAC between 0.8 and 1.0. Intraabdominal pressure was kept limited to 12 mmHg. Parameters such as SAP, DAP, MAP, heart rate, SpO₂, BIS, NIRS (right and left), EtCO₂, PaO₂, PaCO₂, and hemoglobin were recorded at baseline, after anesthesia induction, at surgical initiation, after insufflation, after trendelenburg positioning (30°), and every 30 minutes, as well as during supine repositioning, surgical completion, and post-anesthesia periods. Additionally, the duration of Trendelenburg positioning, total surgical time, anesthesia duration, and intraoperative blood loss were recorded. Results: The mean age of the patients was 69.00±3.59 years, with an age range between 65 and 80 years. After the steep Trendelenburg position, it was observed that the right NIRS values measured at 30 minutes were significantly lower compared to the baseline values (p<0.05). No significant difference was found in the right NIRS values at other time points. Similarly, the left NIRS values measured at 30 and 60 minutes after the steep Trendelenburg position were significantly lower compared to the baseline values (p<0.05, p<0.01, respectively). No significant change was observed in the left NIRS values at other time points. A positive correlation was found between the patients' age and the change in right NIRS values (r=0.295, p<0.05). According to this finding, as the patients' age increased, the difference between the right NIRS values after pneumoperitoneum and baseline values, as well as the difference between the right NIRS values at the 0-minute mark after steep Trendelenburg and baseline values, also increased. However, no correlation was found between the patients' age and the change in left NIRS values (p>0.05). Conclusion: In our study, it has been demonstrated that maintaining stable cerebral oxygenation values contributes to the prevention of cerebrovascular complications. Particularly in older patients, the risk of impaired cerebrovascular autoregulation increases, highlighting the need for continuous and careful monitoring of cerebral oxygenation in these patients. Intraoperative cerebral oxygenation monitoring, combined with the correct management of hemodynamic parameters and appropriate surgical positioning, effectively ensures cerebral safety. Our study supports the idea that NIRS technology is a safe and effective method for intraoperative cerebral oxygenation monitoring.
