Özet
Amaç: Sanal gerçeklik (SG) tabanlı BeCure™ 9 Delikli Çubuk Testi'nin (9-DÇT) inme sonrası el becerilerinin değerlendirilmesinde geçerlilik ve güvenilirliğini incelemek ve bu testin sağlıklı gönüllülerde dominant ve non-dominant el performansı arasındaki farkı ortaya koymak Gereç ve Yöntem: Bu kesitsel çalışmaya 2023-2025 yılları arasında Ankara Üniversitesi Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon Kliniği'nde inme rehabilitasyonu amacıyla takipli 18 yaşın üzerinde, Brunnstrom üst ekstremite evresi 3 ve üzerinde olan, sözlü komut alıp uygulayabilecek 46 inmeli hasta ve sağlıklı 47 gönüllü dahil edildi. Katılımcılara araştırmacı tarafından bilişsel düzeylerini tespit etmek amacıyla yapılan MMSE (Mini Mental State Examination) yapıldı. İnmeli grupta Brunnstorm ve Modifiye Ashworth Skalası (MAS) kullanılarak üst ekstemite ve elde motor aktivite, spastisite değerlendirildi. İki gruba fiziksel 9-DÇT (9-DÇT) ve SG tabanlı 9-DÇT (S-9DÇT) uygulandı. S-9DÇT sistemi Becure™ tarafından geliştirilen PinchPeg oyunu ve hareket sensöründen (Leap Motion Controller) oluşmaktaydı. Bir hafta içinde S-9DÇT tekrar edildi. Verilerin analizi SPSS program ile yapıldı. S-9DÇT'nin geçerliliği (içsel tutarlılığı) Spearman korelasyon katsayısı ile test-tekrar test güvenirliliği sınıf içi korelasyon katsayısı (ICC) ile raporlandı. İstatistiksel anlamlılık düzeyi p<0.05 olarak kabul edildi. Bulgular: Çalışmayı 46 inmeli, 47 sağlıklı gönüllü tamamladı. Her iki grupta S-9DÇT'nin tamamlanma süresi 9-DÇT'ye kıyasla anlamlı derecede daha uzun bulundu (p<0.001). İnmeli grupta (sırasıyla hemiplejik ve sağlam tarafta rho 0,242 ve 0,086) ve sağlıklı grupta (sırasıyla dominant ve non-dominant tarafta rho -0,053 ve 0,015) S-9DÇT, 9-DÇT ile kıyaslandığında geçerli bulunmadı. İnmeli (sırasıyla hemiplejik ve sağlam tarafta ICC: 0,487 ve 0,454) ve sağlıklı grupta (sırasıyla dominant ve non-dominant tarafta sırasıyla ICC 0,484 ve 0,583) test-tekrar test güvenirliliği düşük düzeyde bulundu. İnmeli grupta eğitim düzeyi ile 9-DÇT veya S-9DÇT arasında anlamlı ilişki saptanmadı. Sağlıklı grupta 9-DÇT eğitim düzeyi yüksek olanlar tarafından daha kısa sürede tamamlanırken (p<0.001), S-9DÇT'de fark saptanmadı. Sağlıklı grupta 9-DÇT dominant elde non-dominant ele göre anlamlı derecede daha kısa sürede tamamlandı (p<0.001). S-9DÇT tamamlanma süresi açısından iki el arasında anlamlı bir fark bulunmadı (p>0,05). İnme grubunda S-9DÇT ile MMSE skoru, Brunnstrom üst ekstremite/el evreleri ve üst ekstremite kasları spastisite derecesi arasında anlamlı ilişki tespit edilmedi. Sonuç: İnmeli hastaların el becerilerinin değerlendirilmesinde fiziksel 9-DÇT ile kıyaslandığında BeCure™ SG tabanlı 9-DÇT geçerli değildir ve güvenirliliği düşüktür. Bu sistemin klinik pratikte kullanılabilmesi için öncelikle uygulanabilirlik ve tolerabilitesinin değerlendirileceği ileri araştırmalara ihtiyaç vardır. Ayrıca fiziksel testte olduğu gibi farklı yaş grupları için normatif değerlerinin belirlenmesinin uygun olacağı düşüncesindeyiz.

Aim: To examine the validity and reliability of the virtual reality-based BeCure™ Nine-Hole Peg Test (9-HPT) in the assessment of hand dexterity after stroke, and to identify the differences in performance between the dominant and non-dominant hand in healthy volunteers. Materials and Methods: This cross-sectional study included 46 stroke patients over the age of 18 who were being followed for stroke rehabilitation at Ankara University Faculty of Medicine, Department of Physical Medicine and Rehabilitation between 2023 and 2025, and 47 healthy volunteers. The stroke patients had a Brunnstrom upper extremity stage of 3 or above and were able to understand and follow verbal commands. The Mini Mental State Examination (MMSE) was administered by the researcher to assess the cognitive levels of the participants. In the stroke group, upper extremity and hand motor activity and spasticity were evaluated using the Brunnstrom stages and the Modified Ashworth Scale (MAS). Both groups were administered the physical 9-Hole Peg Test (NHPT) and the virtual reality-based 9-Hole Peg Test (VR-NHPT). The VR-NHPT system consisted of the PinchPeg game developed by Becure™ and a motion sensor (Leap Motion Controller). The VR-9HPT was repeated within one week. Data were analyzed using the SPSS software. The validity (internal consistency) of the VR-NHPT was assessed using the Spearman rho correlation coefficient, and its test–retest reliability was reported with the intraclass correlation coefficient (ICC). A p-value of <0.05 was considered statistically significant. Results: The study was completed by 46 stroke patients and 47 healthy volunteers. In both groups, the completion time of the VR-NHPT was found to be significantly longer compared to the NHPT (p<0.001). In the stroke group (rho = 0.242 and 0.086 for the hemiplegic and unaffected sides, respectively) and in the healthy group (rho = -0.053 and 0.015 for the dominant and non-dominant sides, respectively), the VR-NHPT was not found to be valid compared to the NHPT. Test–retest reliability was found to be low in both the stroke group (ICC: 0.487 and 0.454 for the hemiplegic and unaffected sides, respectively) and the healthy group (ICC: 0.484 and 0.583 for the dominant and non-dominant sides, respectively). In the stroke group, no significant relationship was found between educational level and either the NHPT or VR-NHPT. In the healthy group, the NHPT was completed in a significantly shorter time by individuals with higher educational levels (p<0.001), whereas no difference was found in the VR-NHPT. In the healthy group, the NHPT was completed in a significantly shorter time with the dominant hand compared to the non-dominant hand (p<0.001), whereas there was no significant difference in the completion time of the VR-NHPT between the two hands (p>0.05). In the stroke group, no significant relationship was found between the VR-NHPT and MMSE score, Brunnstrom upper extremity/hand stages, or the degree of spasticity in the upper extremity muscles. Conclusions: When compared to the physical NHPT, the BeCure™ VR-NHPT is not a valid test and its reliability is low in the assessment of hand dexterity in stroke patients. Further research is needed to evaluate the feasibility and tolerability of this system before it can be implemented in clinical practice. Additionally, as with the NHPT, we believe that establishing normative values for different age groups would be appropriate.
