Özet
Çölyak Hastalığı Olan Çocuklarda COVID-19 Pandemisinin Glütensiz Diyete Uyuma ve Antropometrik Ölçümlere Etkisinin Değerlendirilmesi Amaç: Çölyak hastalığının (ÇH) bilinen tek tedavisi yaşam boyu glütensiz diyet tedavisidir. Glütensiz diyet; yüksek maliyeti, dışarıda yemek yeme, tatil vb. sosyal durumlarda kısıtlayıcı olması gibi faktörler nedeniyle diyete uyumu olumsuz etkileyebilecek çeşitli zorluklar içermektedir. Bu zorluklara ek olarak, Mart 2020–Haziran 2021 döneminde tüm dünyada etkili olan COVID-19 pandemisinin neden olduğu çeşitli sosyal kısıtlamalar ve yaşam tarzı değişikliklerinin, çölyak hastalarının glütensiz diyete uyumunu etkilemiş olabileceği düşünülmektedir. Literatürde COVID-19 pandemisinin ÇH'li çocuklar üzerindeki etkisi çok az sayıda çalışmada incelenmiştir. Bu kesitsel ileriye dönük çalışma, COVID-19 pandemisinin ÇH'li çocuklarda glütensiz diyete uyum ve antropometrik ölçümler üzerindeki etkilerini; diyete uyumu etkileyen faktörleri ve pandemi sürecinde diyet konusunda yaşanan zorlukların nedenlerini araştırmayı amaçlamaktadır. Yöntem: Ankara Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Çocuk Gastroenteroloji Bilim Dalında Avrupa Çocuk Gastroenteroloji Hepatoloji ve Beslenme Derneği kriterlerine göre ÇH tanısı almış ve COVID-19 pandemisi öncesi en az 1 yıl süreyle glütensiz diyet tedavisi alan ve anket için hastane kayıt sistemindeki telefon numaralarından ulaşılan 18 yaş altındaki çocuklar çalışmamıza dahil edilmiştir. Demografik, klinik, laboratuvar, histopatolojik ve antropometrik veriler hasta dosyalarından kaydedilmiştir. Ailelere/çocuklara telefonla ulaşılarak yapılandırılmış sorular içeren anket formları araştırmacı tarafından doldurulmuştur. Hastaların anket anında ve COVID-19 pandemisinde kısıtlamalarının olduğu dönemde glütensiz diyete uyumu, glütensiz ürün temini, diyete uyumda yaşanan zorluklar, rutin kontrollere gelme durumu, Çölyakla Yaşam Derneği aktivitelerine katılım/takip, SARS-CoV-2 enfeksiyonu geçirme öyküsü (PCR pozitifliği), aşılama durumu ve pandemi sürecinde ÇH'ye ilişkin belirtilerde artış olup olmadığı ile ilgili veriler anket uygulanarak toplanmıştır. Glütensiz diyete“sözel uyum”glüten içeren besinlerin tüketim sıklığına göre belirlenmiş olup, glüten içeren besinleri hiç tüketmeyen ve/veya yılda 6 seferden az tüketen hastalar diyete sözel olarak uyumlu kabul edilmiştir. Çölyak antikorlarının negatif olması glütensiz diyete“serolojik uyum”olarak tanımlanmıştır. Pandemi öncesi (Mart 2020 tarihi öncesi son poliklinik kontrolü), pandemide kısıtlama dönemi (Haziran 2021 sonrası ilk poliklinik kontrolü) ve pandemi bitimi (Haziran 2022 sonrası ilk poliklinik kontrolü) olmak üzere pandemi sürecinde üç farklı döneme ait antropometrik ölçümler ve çölyak antikor düzeyleri ile anketin yapıldığı dönemde son 1 yıl içinde bakılan çölyak antikorlarına ait veriler geriye dönük olarak hasta dosyalarından kaydedilmiştir. Antropometrik değerlendirme için Olcay Neyzi referans büyüme eğrileri kullanılarak hastaların vücut kütle indeksi (VKİ), yaşa göre ağırlık (YGA), boya göre ağırlık (BGA) ve yaşa göre boy (YGB) z skorları hesaplanmıştır. Boy kısalığı YGB z skorunun -2'den düşük olması, düşük kilolu YGA z skorunun -2'den düşük olması, zayıflık BGA (<2 yaş) /VKİ (>2 yaş) skorunun -2'den düşük olması, şişmanlık VKİ z skorunun +2'den büyük olması olarak tanımlanmıştır. Malnütrisyon YGB ve/veya BGA/VKİ z skorunun -2'den düşük olması olarak kabul edilmiştir. Bulgular: Çalışmaya ortalama yaşı 14,9 ± 3,4 yıl olan, %67,9'u kız, 24'ü klasik ÇH, 43'ü klasik olmayan ÇH, 14'ü subklinik ÇH olmak üzere toplam 81 hasta alınmıştır. Tanı anında malnütrisyon sıklığı %28,4 olarak saptanmıştır. Hastaların %37'sinin ÇH dışında en az bir ek hastalığı olduğu, %22,3'ünün ailesinde ÇH'li birey olduğu, %27,2'sinin hane halkında glütensiz diyet uygulayan bireylerin olduğu, %86,3'ünün Ankara'da yaşadığı, %66,7'sinin çekirdek haneye sahip olduğu, %55,6'sının annelerinin %64,2'sinin ise babaların lise ve üzerinde eğitim aldığı, %69,1'inin hane gelirinin 2 asgari ücretten fazla olduğu, %74,1'inin Çölyakla Yaşam Derneği hakkında bilgilerinin olduğu ancak %25,9'unun derneğin etkinliklerinden haberdar olduğu ve %83,7'sinin rutin kontrollere düzenli geldiği belirlenmiştir. Anket yapıldığı dönemde hastaların ortalama glütensiz diyet uygulama süresi 3,9 ± 2,4 yıl olup, glütensiz diyete sözel uyum oranı %67,9, serolojik uyum oranı %77,1 idi. Yaş, cinsiyet, hastalık tipi, hane gelir düzeyi, ailede ÇH tanılı birey bulunması ve hane halkında glütensiz diyet uygulayan başka birey varlığının anket anında glütensiz diyete uyum üzerinde etkili olmadığı; çekirdek haneye sahip olmanın ise hem sözel (p=0,002) hem de serolojik (p=0,037) olarak glütensiz diyete uyumu olumlu etkilediği saptanmıştır. Hastaların %87,7'sinin glütensiz market ürünlerine ulaşımda herhangi bir nedenle güçlük çektiği, en sık karşılaşılan zorlukların maddi güçlükler/ürünlerin pahalı olması (%59,2) ve ürün çeşidinin az olması (%50,7) olduğu görülmüştür. Pandemi döneminde hastaların %28,4'ünün PCR pozitif SARS-CoV-2 enfeksiyonu geçirdiği, çoğunluğun (%95,7) hastalığı hafif atlattığı, sadece 1 (%4,3) hastaya hastane yatışı gerektiği ve %13,6'sının COVID-19 enfeksiyonuna karşı aşı yaptırdığı saptanmıştır. Pandemide hastaların %55,6'sının rutin gastroenteroloji poliklinik ziyaretlerine gelmediği, %92,6'sının Çölyakla Yaşam Derneği aktivitelerini takip etmediği, %87,7'sinin ise ÇH'ye sahip oldukları için COVID-19 enfeksiyon risklerinin artmış olacağı yönünde kaygılandıkları belirlenmiştir. Pandemi döneminde hastaların %49,4'ü diyet uyumlarının değişmediğini, %33,3'ü arttığını, %17,3'ü azaldığını bildirmiştir. Hastaların glütensiz diyete serolojik uyum oranı pandemi başlangıcında %51,9, kısıtlama döneminde %82,7, pandemi bitiminde %76,5 olarak saptanmıştır. Pandemide glütensiz diyete“serolojik uyum”ile tanı yaşı, yaş, cinsiyet, hastalık tipi, glütensiz diyet süresi, eşlik eden hastalık, aile yapısı, baba eğitim düzeyi, Çölyakla Yaşam Derneği faaliyetlerinin takibi, pandemide kaygı durumu, gastroenteroloji poliklinik kontrollerine gelme durumu arasında istatistiksel açıdan anlamlı bir ilişki olmadığı ancak evde yemek pişirme (p<0,001), ailede ÇH'li birey (p=0,032) ve glütensiz diyet uygulayan başka birey bulunması (p=0,002) (yalnızca pandemide kısıtlama döneminde) ve anne eğitim düzeyi (pandemide kısıtlama döneminde p=0,043, pandemi bitiminde p=0,002) arasında istatistiksel açıdan anlamlı bir ilişki olduğu saptanmıştır. Pandemide glütensiz market ürünlerine erişimde hastaların %87,6'sı problem yaşadığını bildirmiştir. Başlıca zorlukların maddi zorluklar/ürünlerin pahalı olması (%40,8), ürün çeşidinin az olması (%28,1), marketlerde glütensiz ürünlerin bulunamaması (%16,9) ve pandemi nedeniyle kalabalık yerlerde bulunmaktan kaçınma (%15,5) olduğu belirtilmiştir. Pandemide antropometrik parametrelerde 3 farklı dönem arasında istatistiksel açıdan anlamlı bir fark bulunmamıştır. Pandemi öncesinde hastaların %86,4'ünün, pandemide kısıtlama dönemi ve bitiminde %84'ünün beslenme durumunun normal olduğu, pandemi öncesinde malnütrisyonu olan hasta oranının %17,3 olduğu, bu oranın kısıtlama döneminde sabit kaldığı pandemi bitiminde ise %18,8'e yükseldiği gözlenmiştir. Pandemi dönemine kıyasla anket yapıldığı dönemde hem maddi açıdan zorluk/ürünlerin pahalılığı nedeniyle glütensiz ürün temininde zorlanan hasta sayısında (p=0,001) hem de ürün çeşidinde azlık olduğunu bildiren hasta sayısında (p=0,002) artış olduğu saptanmıştır. Sonuç: Çalışmamızda COVID-19 pandemisinin ÇH'li çocuklarda glütensiz diyete uyumu olumlu yönde etkilediği ve bu etkinin pandemi bitiminde de devam ettiği saptanmıştır. Çalışmamız pandemi döneminde evde yemek pişirme, ailede ÇH'li/glütensiz diyet uygulayan birey olması, anne eğitim düzeyi ile glütensiz diyete serolojik uyum arasında istatistiksel açıdan anlamlı ilişki olduğunu ortaya koymuştur. Ayrıca, COVID-19 pandemisinin ÇH'li hastaların antropometrik ölçümleri üzerinde herhangi bir etkisinin olmadığı gösterilmiştir. İlginç olarak, pandemi dönemine kıyasla anket yapıldığı dönemde ürün çeşidinde azlık olduğunu bildiren ve maddi açıdan zorluk/ürünlerin pahalılığı nedeniyle glütensiz ürün temininde zorlanan hasta sayısında artış olduğu gözlenmiştir.

Evaluation of the Effect of the COVID-19 Pandemic on Gluten-Free Diet Compliance and Anthropometric Measurements in Children with Celiac Disease Objective: The only known treatment for Celiac Disease (CD) is a lifelong strict gluten-free diet (GFD). This diet includes various challenges that may negatively affect adherence to diet including high costs and restrictions in social situations such as dining out and traveling etc. In addition to these challenges, it is believed that the social restrictions and lifestyle changes caused by the COVID-19 pandemic, which was effective worldwide from March 2020 to June 2021, may have also affected the adherence of celiac patients to the GFD. There are few studies in the literature investigating the effects of the COVID-19 pandemic on children with CD. This cross-sectional prospective study aims to evaluate the effects of the COVID-19 pandemic on adharence to GFD and anthropometric measurements in children with CD, as well as to identify the factors affecting dieatry adherence and challanges experinced regarding the diet during the pandemic. Methods: Children under the age of 18 who were diagnosed with CD according to the criteria of the European Society of Pediatric Gastroenterology, Hepatology and Nutrition, at the Ankara University Department of Pediatrics, Division of Pediatric Gastroenterology Hepatology and Nutrition and who were on a GFD for at least 1 year before the COVID-19 pandemic and who could be reached through the telephone numbers in the hospital registrastion system for the survey were included in the study. Demographic, clinical, laboratory, histopathological and anthropometric data were obtained from the medical records. Families/children were contacted via phone, and structured questionnaires were filled out by the researcher in April 2024. Data regarding patient's GFD compliance at the time of the survey and during the COVID-19 pandemic restrictions, access to gluten-free products, difficulties in diet compliance, attendance to routine outpatient clinic check-ups, participation in Celiac Disease Association activities, history of SARS-CoV-2 infection (PCR positivity), vaccination status, and whether there was an increase in symptoms related to CD during the pandemic were collected through the questionnaire. Verbal compliance to the GFD was determined based on the frequency of gluten-containing food consumption, with patients who never consumed or consumed gluten-containing foods less than six times a year were considered“verbally compliant”. Negative celiac antibodies were defined as“serological compliance”to the GFD. Anthropometric measurements and celiac antibody levels in three different periods during the pandemic, namely before the pandemic (last outpatient visit before March 2020), the pandemic restriction period (first outpatient visit after June 2021) and the end of the pandemic (first outpatient visit after June 2022), and data on celiac antibodies tested within the last 1 year during the survey period were recorded retrospectively from patient files. For anthropometric evaluation, the Olcay Neyzi reference growth curves were used to calculate the body mass index (BMI), weight-for-age (WFA), weight-for-lenght (WFL), and height-for-age (HFA) z-scores of the patients. Short stature was defined as an HFA z-score below -2, underweight as a WFA z-score below -2, thinness as a WFL (for children under 2 years) / BMI (for children over 2 years) scores below -2, and obesity as a BMI z-score greater than +2. Malnutrition was considered as a HFA and/or WLA/BMI z-scores less than -2. Results: A total of 81 patients with a mean age of 14.9 ± 3.4 years were included, of whom 67.9% were female, 24 had classical CD, 43 had non-classical CD, and 14 had subclinical CD. At the time of diagnosis, 28.4% of the patients were found to be malnourished. It was determined that 37% of the patients had at least one comorbidity other than CD, 22.3% had a family member with CD, 27.2% had individuals in their household following a GFD, 86.3% lived in Ankara, 66.7% had a nuclear family, 55.6% of mothers and 64.2% of fathers had education at the high school level or above, 69.1% had a household income exceeding two minimum wages, and 74.1% had knowledge about the Celiac Disease Association, although only 25.9% were aware of the association's activities. It was found that 83.7% of the patients regularly attended routine outpatient clinic check-ups. The mean duration of GFD at the time of the survey was 3.9 ± 2.4 years, with a verbal compliance rate of 67.9% and a serological compliance rate of 77.1%. Age, gender, type of disease, household income level, presence of a family member with CD, and presence of individuals following a GFD in the household did not significantly affect GFD compliance; however, having a nuclear family positively influenced both verbal (p=0.002) and serological (p=0.037) compliance. It was observed that 87.7% of patients faced difficulties in accessing gluten-free market products, with the most common challenges being financial difficulties/high product costs (59.2%) and limited product variety (50.7%). During the pandemic, 28.4% of patients experienced PCR-positive SARS-CoV-2 infection, with the majority (95.7%) having mild illness, and only one patient (4.3%) required hospitalization. It was found that 13.6% of our population had been vaccinated against COVID-19. During the pandemic, 55.6% of patients did not attend routine gastroenterology outpatient clinic visits, 92.6% did not follow Celiac Disease Association activities and 87.7% were concerned that their risk of COVID-19 infection would increase due to having CD. During the pandemic, 49.4% of patients reported that their diet compliance had not changed, 33.3% reported an increase, and 17.3% reported a decrease. The serological compliance rate to the GFD was found to be 51.9% at the beginning of the pandemic, 82.7% during the restriction period, 76.5% at the end of the pandemic. There was no statistically significant relationship between serological compliance during the pandemic and age at diagnosis, age, gender, type of disease, duration of gluten-free diet, comorbidities, family structure, father's education level, following Celiac Disease Association activities, anxiety during the pandemic, and attendance to gastroenterology outpatient clinic controls; however, there were statistically significant relationships with home cooking (p<0.001), having a family member with CD (only at the end of the pandemic, p=0.032), presence of individuals following a GFD in the household (only at the end of the pandemic, p=0.002), and mother's education level (during the restriction period, p=0.043; at the end of the pandemic, p=0.002). During the pandemic, 87.6% of patients reported problems accessing gluten-free market products. The main challenges reported were financial difficulties/high product costs (40.8%), limited product variety (28.1%), unavailability of gluten-free products in stores (16.9%), and avoidance of crowded places due to the pandemic (15.5%). No statistically significant difference was found in anthropometric parameters among the three different periods during the pandemic. Before the pandemic, 86.4% of patients had normal nutritional status, while 84% maintained normal status during the restriction period and at the end of the pandemic. The proportion of patients with malnutrition before the pandemic was 17.3%, which remained stable during the restriction period and increased to 18.8% at the end of the pandemic. Compared to the pandemic period, there was an increase in the number of patients reporting difficulties in accessing gluten-free products due to financial constraints/high costs (p=0.001) and in the number of patients reporting limited product variety (p=0.002) at the time of the survey. Conclusion: Our study found that the COVID-19 pandemic positively affected adherence to GFD in children with CD, and this effect continued even after the pandemic. The study revealed a statistically significant relationship between home cooking, having a family member with CD/GFD, and mother's education level with serological compliance to the GFD during the pandemic. Additionally, no effect of the COVID-19 pandemic on the anthropometric measurements of patients with CD was observed. Interestingly, compared to the pandemic period, there was an increase in the number of patients reporting limited product variety and difficulties in accessing gluten-free products due to financial constraints/high costs at the time of the survey.
