Özet

Çocuk hastalarda gastroenteritler önemli morbidite ve mortalite nedenlerindendir. Gastroenterit yapan etkenler içinde paraziter etkenler önemli bir yere sahiptir. Bu çalışmada, ishali olan çocuk hastalarda mikroskobik bakı ve polimeraz zincir reaksiyonu (PZR) ile paraziter akut gastroenterit etkenlerinin sıklığının araştırılması ayrıca mikroskobik bakı sonuçlarının PZR tanı yöntemi ile karşılaştırılması amaçlanmıştır. Çalışmada, Necmettin Erbakan Üniversitesi Tıp Fakültesi Hastanesi'ne akut gastroenterit şikayetiyle başvuran pediatrik hastalara ait 94 dışkı örneği incelenmiştir. Örneklerde konvansiyonel yöntemler ve PZR ile paraziter etkenler araştırılmıştır. Konvansiyonel yöntem olarak direkt ve lugollü bakı ile değerlendirme, giemsa boyama ve modifiye aside dirençli boyama sonrası mikroskobik değerlendirme yapılmıştır. İkinci bir yöntem olarak tüm örneklerde kantitatif gerçek zamanlı multipleks PZR ile Blastocystis hominis, Dientamoeba fragilis, Giardia intestinalis, Entamoeba histolytica ,Cryptosporidium spp. ve Cyclospora cayetanensis varlığı araştırılmıştır. Çalışmaya dahil edilen 94 hastanın yaş ortalaması 5,985,65'tir. Hastaların %32'si (30) kız, %68'i (64) erkektir. Çalışmada Blastocystis hominis ve Dientamoeba fragilis en yaygın tespit edilen parazitlerdir. Multipleks PZR ile tüm örneklerin %66'sında (n=62), mikroskobik değerlendirme ile ise örneklerin %27,7'sinde (n=26) en az bir paraziter etken saptanmıştır. Paraziter etkenlerin PZR ile tek tek pozitiflik oranlarına bakılırsa 35 hastada (%37,2) B. hominis, 21 hastada D. fragilis (%22,3), 13 hastada (%13,8) E. histolytica tespit edilmiştir. PZR ile örneklerin hiçbirinde G. intestinalis, Cryptosporidium spp ve Cyclospora cayetanensis pozitifliği bulunmamıştır. Mikroskobik değerlendirmede 13 hastada (%13,8) B. hominis, 11 hastada (%11,7) E. histolytica/dispar, 2 hastada (%2,1) Cryptosporidium spp. tespit edilmiştir. G. intestinalis, D. fragilis ve Cyclospora cayetanensis örneklerin hiçbirinde mikroskobik bakıda görülmemiştir. Paraziter gastroenteritlerde tanı genellikle mikroskobik değerlendirmelere dayalıdır ve PZR tabanlı taramalar sınırlıdır. Bu da güncel prevelans ve epidemiyolojik verilerin hatalı olmasına neden olmaktadır. Bu çalışma, PZR yönteminin bağırsak protozoonlarının tanısında konvansiyonel mikroskopiye kıyasla üstün bir hassasiyet gösterdiğini ortaya koymuştur. Rutin parazitoloji laboratuvar uygulamalarında PZR yönteminin kullanımının, bağırsak parazit enfeksiyonlarının teşhisinde doğru ve hızlı tanı için önemli avantajlar sağlayacağı sonucuna varılmıştır.

Gastroenteritis is a significant cause of morbidity and mortality in pediatric patients. Among the etiological agents of gastroenteritis, parasitic pathogens hold a prominent place. This study aimed to investigate the prevalence of parasitic agents responsible for acute gastroenteritis in children with diarrhea using both microscopic examination and polymerase chain reaction (PCR), and to compare the diagnostic performance of microscopy with PCR. In this study, 94 stool samples from pediatric patients who presented with complaints of acute gastroenteritis at Necmettin Erbakan University Faculty of Medicine Hospital were analyzed. Parasitic agents were investigated using both conventional methods and PCR. As part of the conventional methods, stool samples were examined using direct and Lugol's iodine wet mount preparations, Giemsa staining, and modified acid-fast staining followed by microscopic evaluation. As a second method, quantitative real-time multiplex PCR were performed on all samples to detect the presence of Blastocystis hominis, Dientamoeba fragilis, Giardia intestinalis, Entamoeba histolytica, Cryptosporidium spp. and Cyclospora cayetanensis. The mean age of the 94 patients included in the study was 5.98 ± 5.65 years. Of the patients, 32% (n=30) were female and 68% (n=64) were male. Blastocystis hominis and Dientamoeba fragilis were the most commonly detected parasites in the study. At least one parasitic agent was detected in 66% (n=62) of all samples by multiplex PCR and in 27.7% (n=26) of samples by microscopic evaluation. According to the individual positivity rates with qPCR: B. hominis was found in 35 patients (37.2%), D. fragilis in 21 patients (22.3%), and E. histolytica in 13 patients (13.8%). G. intestinalis, Cryptosporidium spp., and Cyclospora cayetanensis were not detected by PCR in any of the samples. In the microscopic evaluation, B. hominis was observed in 13 patients (13.8%), E. histolytica/dispar in 11 patients (11.7%), and Cryptosporidium spp. in 2 patients (2.1%). G. intestinalis, D. fragilis, and C. cayetanensis were not detected microscopically in any of the samples. The diagnosis of parasitic gastroenteritis is typically based on microscopic methods, and PCR-based screening remains limited. This limitation contributes to inaccuracies in current prevalence and epidemiological data. This study demonstrates that PCR methods offer superior sensitivity in the diagnosis of intestinal protozoa compared to conventional microscopy. It is concluded that the integration of PCR techniques into routine parasitology laboratory practice would provide significant advantages in achieving accurate and rapid diagnosis of intestinal parasitic infections.
