Özet

Amaç: Total tiroidektomi ameliyatı sonrasında hipokalsemi (geçici ve kalıcı) gelişebilmekte ve paratiroid bezlerinin korunmaya çalışılmasına rağmen insidental paratiroidektomi (İP) görülebilmektedir. İP'nin hipokalsemi üzerine etkisi halen tartışmalıdır. Bu çalışmanın amacı, total tiroidektomi yapılan hastalarda İP'nin geçici ve kalıcı hipokalsemiye olan etkisini ve altta yatan potansiyel risk faktörlerini belirlemek, ayrıca İP potansiyel risk faktörlerinin hipokalsemi (geçici ve kalıcı) gelişimindeki etkisini araştırmaktır. Materyal ve Metod: Etik kurul onayı alındıktan sonra, Haziran 2007 ile Ocak 2024 tarihleri arasında Mersin Üniversitesi Tıp Fakültesi Hastanesi Genel Cerrahi Kliniği'nde total tiroidektomi ameliyatı yapılan hastaların verileri retrospektif olarak incelendi. Hastalar İP yapılan (önlenebilir [EİP] ve önlenemez [İİP]) ve yapılmayan (NİP) olmak üzere iki gruba ayrıldı. Gruplar İP'nin hipokalsemi üzerine etkisini araştırmak için hipokalsemi (geçici, kalıcı) gelişme durumu ve İP'ye etki eden risk faktörlerinin belirlenmesi için potansiyel İP risk faktörleri (demografik faktörler [yaş, cinsiyet], (ASA) skoru, ameliyatı yapan cerrahın unvanı (öğretim üyesi, uzman dr, asistan), tiroid hastalığının retosternal uzanımı ve tiroidektomi materyalinin histopatolojik tanı sonucu (iki farklı sınıflama: malign/benign ve malign/multinodüler guatr[mng]/tiroidit) bakımından karşılaştırıldı. Ayrıca hipokalsemi gelişimine etki eden İP risk faktörlerinin belirlenmesi için hastalar geçici (GHK) ve kalıcı (KHK) hipokalsemi (HK) olmak üzere iki grubu ayrılarak İP risk faktörleri bakımından karşılaştırıldı. Bulgular: Çalışmaya 1702 hasta dahil edildi. İP 257 (%15,1) hastada, geçici hipokalsemi 1118 (%65,6) hastada, kalıcı hipokalsemi 52 (%3) saptandı. İP'nin kalıcı ve total hipokalsemi, önlenebilir İP'nin kalıcı ve total hipokalsemi, önlenemez İP'nin geçici ve total hipokalsemi gelişiminini istatistiksel olarak anlamlı oranda arttırdığı saptandı (p≤0,05). Benign ve tiroidit histopatolojik tanıları İP ve EİP (önlenebilir İP) yapılmasını arttırdı (p≤0,05). Kadın cinsiyeti, geçici ve kalıcı hipokalsemiyi arttırırken, malign hastalık tanısı ve ≥50 yaş geçici hipokalsemiyi, <50 yaş kalıcı hipokalsemiyi arttırdı (p≤0,05). Sonuç: Total tiroidektomi ameliyatı yapılan hastalarda İP yapılması hipokalsemi gelişiminde etkili olmaktadır. İP ve EİP yapılması kalıcı hipokalsemi, İİP yapılması geçici hipokalsemi gelişiminde etkili olurken, İP, EİP ve İİP total hipokalsemi gelişiminde etkili olmaktadır. Histopatolojik tanılardan benign hastalıklar ve tiroidit özellikle EİP için risk faktörüdür. Cinsiyet, yaş ve histopatolojik tanı hipokalsemi gelişiminde etkili olan risk faktörlerindendir. Kadın cinsiyeti, ≥50 yaş ve malign hastalık tanısı geçici hipokalsemi gelişmesi için, kadın cinsiyeti ve <50 yaş kalıcı hipokalsemi gelişmesi için risk faktörüdür. Aim: Following total thyroidectomy, hypocalcemia (both temporary and permanent) may develop, and despite efforts to preserve the parathyroid glands, incidental parathyroidectomy (IP) can occur. The impact of IP on hypocalcemia remains a subject of ongoing debate. The objective of this study is to assess the effects of IP on the development of temporary and permanent hypocalcemia in patients undergoing total thyroidectomy, identify underlying potential risk factors, and investigate the influence of these risk factors on the development of hypocalcemia (both temporary and permanent). Materials and Methods: After obtaining ethical committee approval, data from patients who underwent total thyroidectomy at the General Surgery Department of Mersin University Hospital between June 2007 and January 2024 were retrospectively reviewed. The patients were divided into two groups: those who underwent incidental parathyroidectomy (IP) (preventable [EIP] and non-preventable [IIP]) and those who did not undergo IP (NIP). The groups were compared to investigate the effect of IP on hypocalcemia and to identify risk factors influencing IP. Potential risk factors for IP were evaluated, including demographic factors (age, sex), the American Society of Anesthesiologists (ASA) score, the title of the operating surgeon (faculty member, specialist doctor, resident), the retrosternal extension of thyroid disease, and the histopathological diagnosis of the thyroidectomy specimen (two classifications: malignant/benign and malignant/multinodular goiter [MNG]/thyroiditis). Additionally, to identify risk factors for IP influencing the development of hypocalcemia, the patients were divided into two groups: those with temporary (THC) and permanent (PHC) hypocalcemia (HC), and comparisons were made based on IP risk factors. Results: A total of 1702 patients were included in the study. Incidental parathyroidectomy (IP) was performed in 257 patients (15.1%), temporary hypocalcemia was observed in 1118 patients (65.6%), and permanent hypocalcemia was found in 52 patients (3%). It was determined that IP significantly increased the incidence of permanent and total hypocalcemia, with preventable IP increasing the incidence of permanent and total hypocalcemia, and non-preventable IP increasing temporary and total hypocalcemia (p≤0.05). Benign and thyroiditis histopathological diagnoses were associated with an increased likelihood of IP and preventable IP (EIP) being performed (p≤0.05). Female sex was found to increase the risk of both temporary and permanent hypocalcemia, while a diagnosis of malignancy and age ≥50 years increased the risk of temporary hypocalcemia, and age <50 years increased the risk of permanent hypocalcemia (p≤0.05). Conclusion: Incidental parathyroidectomy (IP) during total thyroidectomy significantly influences the development of hypocalcemia. The performance of IP and preventable IP (EIP) contributes to the development of permanent hypocalcemia, while non-preventable IP (IIP) increases the incidence of temporary hypocalcemia. Furthermore, IP, EIP, and IIP are all associated with an increased risk of total hypocalcemia. Among histopathological diagnoses, benign diseases and thyroiditis are particularly risk factors for EIP. Gender, age, and histopathological diagnosis are identified as risk factors for the development of hypocalcemia. Female sex, age ≥50 years, and a malignancy diagnosis are risk factors for the development of temporary hypocalcemia, while female sex and age <50 years are risk factors for the development of permanent hypocalcemia.
