Özet

ÖZET Ratlarda Hypericum Perforatum (St. John's Wort)' un Enrofloksasin Farmakokinetiği Üzerine Etkisi Bu çalışmada, dünyada ve ülkemizde gerek insanlarda gerekse hayvanlarda antibakteriyel, antiinflamatuar ve antidepresan etkisi nedeniyle yaygın olarak kullanılan H. perforatum bitki ekstresi ön uygulaması sonrasında, veteriner hekimlikte yaygın kullanım alanı bulan geniş spektrumlu bir kinolon grubu antibiyotik olan enrofloksasin (ENR)' in kullanımı durumunda kendisi ve metabolitinin (Siprofloksasin-CPFX) farmakokinetik profilinde değişiklik olup olmadığı araştırıldı. Çalışmada validasyon için 23 adet rat, deney grupları için 56 (11 rat/her grup ve her grup için 3 rat yedek) adet rat kullanıldı. Gruplar, Grup 1 (gavajla tek doz ENR uygulama grubu), Grup 2 (bir hafta süre ile gavajla H. perforatum uygulama sonrası ertesi gün gavajla tek doz ENR uygulama grubu), Grup 3 (periton içi-ip tek doz ENR uygulama grubu) ve Grup 4 (bir hafta süre ile gavajla H. perforatum uygulama sonrası ertesi gün periton içi-ip tek doz ENR uygulama grubu) şeklinde oluşturuldu. Grup 2 ve Grup 4' te bulunan ratlara günde 144 mg/kg canlı ağırlık (c.a) dozunda bitki ekstresi, ön uygulama olarak bir hafta boyunca gavaj ile uygulandı. H. perforatum bitki ekstresi uygulanan grupların ön uygulama bitiminin ertesi günü tüm deney gruplarına ENR (Grup 1 ve Grup 2'ye 10 mg/kg c.a. dozunda gavajla tek doz; Grup 3 ve Grup 4'e 10 mg/kg c.a. dozunda ip tek doz) uygulandı. ENR uygulaması sonrası 0,25; 0,75; 1, 2, 4, 6 ve 8' inci saatlerde ratlardan kan örnekleri alınarak serumları ayrıldı. Elde edilen serumların ENR ve CPFX analizleri HPLC cihazı ile gerçekleştirildi. Serum analizleri ile ENR ve CPFX' in zamana bağlı serum-ilaç konsantrasyon eğrileri hazırlandı. Farmakokinetik hesaplamalar kompartımansız metoda göre, PK Solver programı kullanılarak yapıldı. Verilerin istatistik analizleri IBM SPSS Statistics 30 programı ile yapıldı. Metot validasyonu çalışma sonuçları yöntemin, doğruluk ve kesinlik, tekrarlanabilirlik, tekrar üretilebilirlik, seçicilik ve doğrusallık açısından spesifik olduğunu gösterdi. ENR için altı konsantrasyonda (0,57; 0,83; 1,08; 1,19; 1,48 ve 1,69 µg/ml) ve CPFX için beş konsantrasyonda (0,53; 0,76; 1,09; 1,35 ve 1,56 µg/ml) yapılan validasyon çalışmalarında, ENR geri kazanımı (sırasıyla %70, 103, 92, 103, 103 ve 101) ve CPFX geri kazanımının (sırasıyla %75, 109, 108, 107 ve 99) yüksek olduğu saptandı. ENR analizi için tespit (LOD) ve ölçüm limiti (LOQ) sırasıyla 0,039 ve 0,39 µg/ml, CPFX analizi için tespit (LOD) ve ölçüm limiti (LOQ) sırasıyla 0,014 ve 0,14 µg/ml olarak bulundu. Grupların ENR ve CPFX farmakokinetik değerlendirmesi; eliminasyon hız sabiti-(λz (1/h), eliminasyon yarı ömrü-(t1/2 (h), maksimum konsantrasyona ulaşma süresi-(Tmax (h), en yüksek plazma konsantrasyonu-(Cmax (μg/ml), konsantrasyon-zaman eğrisi altındaki alan-(AUC0-inf,obs (μg/ml*h), ortalama kalış süresi-(MRT0-inf,obs (h), F ile düzeltilmiş gözlenen dağılım hacmi-(Vz/Fobs (mg/kg)/(μg/ml), F ile düzeltilmiş gözlenen klirens-(Cl/Fobs (mg/kg)/(μg/ml)/h), nispi biyoyararlanım yüzdesi (%Fnispi) parametreleri üzerinden yapıldı. Grup 1 ENR değerleri sırasıyla 0,36 ± 0,06; 2,18 ± 0,38; 0,6 ± 0,15; 0,62 ± 0,08; 1,9 ± 0,17; 3,25 ± 0,49; 16,19 ± 1,65; 5,43 ± 0,5 ve 63, Grup 2 ENR değerleri sırasıyla 0,38 ± 0,03; 1,89 ± 0,14; 0,55 ± 0,12; 0,31 ± 0,04; 0,72 ± 0,05; 2,6 ± 0,18; 38,76 ± 4,12; 14,11 ± 0,91 ve 24, Grup 3 ENR değerleri sırasıyla 0,49 ± 0,04; 1,44 ± 0,13; 0,25 ± 0; 1,92 ± 0,19; 3 ± 0,28; 1,45 ± 0,05; 7,29 ± 1,08 ve 3,45 ± 0,33, Grup 4 ENR değerleri sırasıyla 0,33 ± 0,03; 2,21 ± 0,2; 0,25 ± 0; 0,79 ± 0,07; 1,22 ± 0,14; 2;47 ± 0,17; 28,9 ± 5,03 ve 8,72 ± 0,89 olarak bulundu. CPFX farmakokinetik değerleri ise Grup 1'de sırasıyla 0,34 ± 0,08; 2,94 ± 0,89; 1,38 ± 0,59; 0,08 ± 0,01; 0,37 ± 0,07; 4,83 ± 1,19; 117,61 ± 20,69; 32,30 ±5,74 ve 82, Grup 2'de sırasıyla 0,21 ± 0,07; 5,19 ± 1,37; 0,63 ± 0,13; 0,11 ± 0,02; 0,43 ± 0,08; 7,54 ± 1,7; 168,64 ± 27,44; 28,48 ± 5,89 ve 95, Grup 3'de sırasıyla 0,19 ± 0,02; 3,91 ± 0,45; 0,79 ± 0,12; 0,08 ± 0,01; 0,45 ± 0,04; 6,14 ± 0,7; 131,1 ± 18,39 ve 23,03 ± 2,03, Grup 4'de sırasıyla; 0,1 ± 0,01; 7,77 ± 1,33; 0,75 ± 0,18; 0,07 ± 0; 0,61 ± 0,09; 11,99 ± 2,26; 184,05 ± 18,18 ve 17,43 ± 2,57 olarak bulundu. Gavaj uygulama grupları H. perforatum' lu ve H. perforatum' suz (Grup 1 ve Grup 2) olarak ENR farmakokinetiği bakımından karşılaştırıldığında Grup 2 Cmax (0,31 ± 0,04, p<0,05), AUC0-inf,obs (0,72 ± 0,05, p<0,05), Vz/Fobs (38,76 ± 4,12, p<0,05), Cl/Fobs (14,11 ± 0,91, p<0,05) değerleri Grup 1' e göre istatistiksel olarak anlamlı bulundu. Araştırma sonucu, H. perforatum bitki ekstresinin bir hafta ön uygulama sonrası ertesi gün ENR' nin gavajla ve periton içi kullanımında kendisi ve metaboliti CPFX' in biyoyaralanımını azaltarak, diğer farmakokinetik parametrelerde de özellikle gavaj gruplarında daha belirgin olmak üzere genel anlamda farmakokinetik profilde değişikliğe neden olduğunu gösterdi. Bu durumun metabolik enzimler ve/veya taşıyıcı proteinlerin inhibisyonu ve/veya indüksiyonundan kaynaklanabileceği değerlendirilerek, yetersiz terapötik etki ve toksisite açısından, bu gibi birlikte ilaç/bitki kullanımlarında etkileşimlerin göz önünde tutulması gerektiğinin önemini ortaya koydu.

SUMMARY Effect of Hypericum Perforatum (St. John's Wort) on the Pharmacokinetics of Enrofloxacin in Rats. In this study, it was investigated whether there was a change in the pharmacokinetic profile of enrofloxacin (ENR), a broad-spectrum quinolone antibiotic widely used in veterinary medicine, and its metabolite (Ciprofloxacin-CPFX) after pretreatment with H. perforatum plant extract, which is widely used in humans and animals in the world and in our country due to its antibacterial, anti-inflammatory and antidepressant effects. In the study, 23 rats were used for validation and 56 rats (11 rats/each group and 3 spare rats for each group) were used for experimental groups. The groups were formed as Group 1 (single dose ENR administration by gavage group), Group 2 (one week of H. perforatum administration by gavage followed by a single dose ENR administration by gavage the next day), Group 3 (single dose intraperitoneal-ip ENR administration group) and Group 4 (one week of H. perforatum administration by gavage followed by a single dose ENR administration by intraperitoneal-ip the next day). The plant extract was administered to rats in Group 2 and Group 4 via gavage at a dose of 144 mg/kg body weight (bw) per day as a pre-treatment for one week. The day after the end of the pretreatment of the groups in which H. perforatum plant extract was applied, ENR (Group 1 and Group 2 at a single dose of 10 mg/kg body weight via gavage; Group 3 and Group 4 at a single dose of 10 mg/kg body weight via ip) was applied to all experimental groups. After the ENR application, blood samples were taken from the rats at 0.25, 0.75, 1, 2, 4, 6 and 8 hours and their sera were separated. ENR and CPFX analyses of the obtained sera were performed with an HPLC device. Time-dependent serum-drug concentration curves of ENR and CPFX were prepared with serum analyses. Pharmacokinetic calculations were performed using the PK Solver program according to the non-compartmental method. Statistical analyses of the data were performed using the IBM SPSS Statistics 30 program. Method validation study results showed that the method was specific in terms of accuracy and precision, repeatability, reproducibility, selectivity and linearity. In validation studies performed at six concentrations for ENR (0.57, 0.83, 1.08, 1.19, 1.48 and 1.69 µg/ml) and five concentrations for CPFX (0.53, 0.76, 1.09, 1.35 and 1.56 µg/ml), it was found that ENR recovery (70, 103, 92, 103, 103 and 101%, respectively) and CPFX recovery (75, 109, 108, 107 and 99%, respectively) were high. The limit of detection (LOD) and quantification (LOQ) for ENR analysis were found to be 0.039 and 0.39 µg/ml, respectively, and the limit of detection (LOD) and quantification (LOQ) for CPFX analysis were found to be 0.014 and 0.14 µg/ml, respectively. ENR and CPFX pharmacokinetic evaluation of the groups was performed using the following parameters: elimination rate constant-(λz (1/h), elimination half-life-(t1/2 (h), time to reach maximum concentration-(Tmax (h), highest plasma concentration-(Cmax (μg/ml), area under the concentration-time curve-(AUC0-inf,obs (μg/ml*h), mean residence time-(MRT0-inf,obs (h), F-corrected observed volume of distribution-(Vz/Fobs (mg/kg)/(μg/ml), F-corrected observed clearance-(Cl/Fobs (mg/kg)/(μg/ml)/h), relative bioavailability percentage (%Frelative). Group 1 ENR values were 0.36 ± 0.06, 2.18 ± 0.38, 0.6 ± 0.15, 0.62 ± 0.08, 1.9 ± 0.17, 3.25 ± 0.49, 16.19 ± 1.65, 5.43 ± 0.5 and 63, respectively. Group 2 ENR values were 0.38 ± 0.03, 1.89 ± 0.14, 0.55 ± 0.12, 0.31± 0.04, 0.72 ± 0.05, 2.6 ± 0.18, 38.76 ± 4.12, 14.11 ± 0.91 and 24, respectively. Group 3 ENR values were 0.49 ± 0.04, 1.44 ± 0.13, 0.25 ± 0, 1.92 ± 0.19, 3 ± 0.28, 1.45 ± 0.05, 7.29 ± 1.08 and 3.45 ± 0.33 respectively, Group 4 ENR values were 0.33 ± 0.03, 2.21 ± 0.2, 0.25 ± 0, 0.79 ± 0.07, 1.22 ± 0.14, 2.47 ± 0.17, 28.9 ± 5.03 and 8.72 ± 0.89 respectively. The pharmacokinetic values of CPFX in Group 1 were found to be 0.34 ± 0.08, 2.94 ± 0.89, 1.38 ± 0.59, 0.08 ± 0.01, 0.37 ± 0.07, 4.83 ± 1.19, 117.61 ± 20.69, 32.30 ±5.74 and 82 in Group 2, respectively. 0.21 ± 0.07, 5.19 ± 1.37, 0.63 ± 0.13, 0.11 ± 0.02, 0.43 ± 0.08, 7.54 ± 1.7, 168.64 ± 27.44, 28.48 ± 5.89 and 95 in Group 3, respectively. 0.19 ± 0.02, 3.91 ± 0.45, 0.79 ± 0.12, 0.08 ± 0.01, 0.45 ± 0.04, 6.14 ± 0.7, 131.1 ± 18.39 and 23.03 ± 2.03 in Group 4, respectively; 0.1 ± 0.01, 7.77 ± 1.33, 0.75 ± 0.18, 0.07 ± 0, 0.61 ± 0.09, 11.99 ± 2.26, 184.05 ± 18.18 and 17.43 ± 2.57. When the gavage application groups with and without H. perforatum (Group 1 and Group 2) were compared in terms of ENR pharmacokinetics, Group 2 Cmax (0.31 ± 0.04, p<0.05), AUC0-inf,obs (0.72 ± 0.05, p<0.05), Vz/Fobs (38.76 ± 4.12, p<0.05), Cl/Fobs (14.11 ± 0.91, p<0.05) values were found to be statistically significant compared to Group 1. The results of the study showed that the plant extract of H. perforatum decreased the bioavailability of ENR and its metabolite CPFX by gavage and intraperitoneal use the next day after a week of pre-application, and caused changes in the pharmacokinetic profile in general, especially in the gavage groups, in other pharmacokinetic parameters. It was evaluated that this situation may be due to the inhibition and/or induction of metabolic enzymes and/or carrier proteins, and it revealed the importance of considering such interactions in such concomitant drug/plant use in terms of insufficient therapeutic effect and toxicity.
