Özet
Gelişen toplum yapısı, sağlık hizmetlerine olan talebi süratle arttırmakta ve kaçınılmaz bir arz talep dengesizliği ortaya çıkmaktadır. Gelişmiş ülkeler, sağlık hizmetlerinin artan talebi karşılayabilecek düzeyde üretimi ve dağılımı konusunda etkin tedbirler almış ve bu alana önemli ölçülerde kaynak tahsis etmişlerdir. Ülkemizde ise durum çok farklıdır. Sağlık sektörüne G.S.M.H.'dan ayrılan pay %4, bütçeden Sağlık Bakanlığı'na ayrılan pay ise %3'tür. Bu rakamlar O.E.C.D. ortalamasının yarısından azdır. Bu tahsis edilen kaynakların da ancak %25'i yatırıma ayrılabilmektedir. Böyle bir durumda kamu hastane işletmelerinin devamlı gelişen tıbbi teknolojiye ayak uydurabilmeleri güçleşmektedir. Ellerindeki kıt kaynaklarla ancak belirli yatırımları yapabilmektedirler. Bu durum, kamu hastane işletmelerinin yatırım projelerini bir öncelik sırasına koymasını gerektirmektedir. Sosyo-ekonomik kurumlar olan kamu hastanelerinde, yatırım projelerini öncelik sırasına koymada sadece fınansal verilerin etüdü yeterli değildir. Finansal değerlendirme verileri, para ile ölçülemeyen hasta bakımları olarak sağlanan faydalarla bütünleşmelidir. Bu tezde, kamu hastanelerinin yatırım projelerini sıralamasında, sermaye ve işletme maliyeti faktörlerini, bu faktörlerin sermaye maliyetine oranlarını ve bütün fınansal faktörlerin toplam faydalarla ilişkilerini belirleyen Maliyet Fayda Endeksi önerilmektedir. Bu tezde söz konusu yöntem kullanılarak Osmangazi Üniversitesi Eğitim Uygulama ve Araştırma Hastanesi'nde üç yatırım projesi söz konusu yöntem kullanılarak değerlendirilmiştir.

The developing structure of society is rapidly augmenting the demand for health services and balance between supply and demand is inevitably upset. Developed Countries have taken effective precautions in the field of production and distribution of health services to the extent of meeting increasing demad and have assigned substantial sources to this field. However, in our country the situation is quite different The percentage allowed from G.N.D.P. for the health sector is 4 %, the percentage allowed from the budget for the Ministry of Health is 3 %. These figures are less than half of the average of O.E.C.D. However, only 25% of these assigned sources can be set asside for investment. In such a situation, it is diffucult for public hospital undertakings to adopt themselves to the constantly developing medical technology, with the insufficent sources they possess they can only make certain investment. This fact requires the public hospital undertakings to put the investment projects in order of precedence. In public hospitals which are socio-economic institutions, the study of financial data alone is not sufficent in putting the investment projects in order of precedence. The data of financial assesment must be integrated with the benefits obtained as patient care which can not be considered in terms of money. In this thesis, while enumerating the investment project of public hospitals, we propose a Cost Benefit Index which determines the factors of capital and management cost, the rates of capital cost to these factors and the reletions of total benefits with all financial factors. In this thesis, three investment projects have been assessed using the above mentioned method in the Education Practice and Research Hospital of Osmangazi University. m
