Özet

Amaç: Narkolepsi; hipokretin/oreksin eksikliği, ani kas tonusu kaybı (katapleksi) ve gündüz aşırı uykululuk ile karakterize kronik bir uyku bozukluğudur. Bu çalışmanın temel amacı, narkolepsi tip 1 (NT-1) hastalarında beynin yapısal değişimlerini ve bu değişimlerin nöropsikolojik belirtilerle ilişkisini anlamak, böylece hastalığın patofizyolojisini daha iyi aydınlatarak klinik ve tedavi yaklaşımlarının geliştirilmesine katkı sağlamaktır. Gereç ve Yöntem: Çalışmada 25 NT-1, 25 sağlıklı kontrol olmak üzere toplam 50 birey yer almıştır. Katılımcıların manyetik rezonans (MR) görüntülemeleri için 3 Tesla MR cihazı kullanılmıştır. Volümetrik ölçümler, MRICloud ve VolBrain sistemleri ile yapılmıştır. Amigdala, hipokampus (CA1-3, CA4-DG, Subikulum), hipotalamus, talamus, bazal önbeyin, nükleus akkumbens, mezensefalon, pons, bulbus, limbik sistem, dorsolateral prefrontal korteks, prefrontal korteks ve serebral korteks dahil olmak üzere, beyin bölgelerindeki hacim değişiklikleri analiz edilmiştir. Tüm katılımcılara Epworth Uykululuk Ölçeği, Temel Empati Ölçeği, Barratt Dürtüsellik Ölçeği, Buss-Perry Saldırganlık Ölçeği, Sayı Dizisi Testi, Hamilton Derecelendirme Ölçeği ve Stroop Testi uygulanmış ve elde edilen sonuçlar hacim ölçümleriyle karşılaştırılmıştır. İstatistiksel analizlerde p<0,05 anlamlı kabul edilmiştir. Bulgular: NT-1 hastalarında bilateral hipokampus CA1-3 bölgelerinde ve sağ ponsda anlamlı hacim kaybı saptanmıştır. Hipotalamus, talamus, bazal ön beyin, mezensefalon, nükleus akkumbens, limbik sistem, prefrontal korteks ve serebellar korteks gibi diğer bölgelerde ise anlamlı bir değişiklik bulunmamıştır. Nöropsikolojik testler, hastalarda daha yüksek depresyon ve sinirlilik puanları ile daha düşük dikkat ve yürütücü işlev puanları göstermiştir. VKİ, kilo artışı ve serebellar korteks, prefrontal korteks, bazal ön beyin, nükleus akkumbens, amigdala ve hipokampus gibi beyin bölgelerinin hacimleri arasında pozitif bir korelasyon bulunmuştur. Epworth uykululuk ölçeği ve REM uyku latansı ile hipokampus hacmi arasında ise negatif bir korelasyon gözlenmiştir. Depresyon puanları, beyin sapı, bazal ön beyin, talamus ve amigdala hacimleriyle negatif korelasyon göstermiştir. Sonuç: Bu çalışmada, NT-1 hastalarında bilateral hipokampus CA1-3 bölgelerinde ve sağ ponsda belirgin hacim kaybı saptanmıştır. Bulgular, hipokampus ve ponsun narkolepsi patofizyolojisinde önemli bir role sahip olduğunu göstermektedir. Hipokampal hacim kaybının, aşırı gündüz uykululuğu ve REM uyku latansıyla ilişkili olması, hipokampusun uyku-uyanıklık döngüsündeki kritik rolünü desteklemektedir. Diğer beyin bölgelerinde anlamlı hacim değişikliği gözlenmemiş olması, narkolepsideki yapısal değişimlerin mikroskobik düzeyde olabileceğine veya mevcut ölçüm yöntemleriyle tespit edilemeyecek kadar küçük olabileceğine işaret etmektedir. Aynı zamanda, depresyon ve sinirlilik gibi nöropsikolojik belirtilerin hipokampal ve pons atrofisi ile ilişkili olması, bu yapıların ruh hali ve bilişsel işlevler üzerindeki etkisini vurgulamaktadır. Öte yandan, VKİ ve bazı beyin bölgelerinin hacimleri arasında pozitif bir korelasyon bulunması, kilo artışının beyinde yapısal değişikliklere neden olabileceğini göstermektedir. Bu bulgu, narkolepsi hastalarında metabolik ve yapısal değişimlerin birlikte değerlendirildiği daha ileri çalışmalara ihtiyaç olduğunu ortaya koymaktadır. Genel olarak bu çalışma, narkolepsi patofizyolojisinde hipokampus ve ponsun rolünü ortaya koyarken, daha ayrıntılı fonksiyonel ve yapısal araştırmaların gerekliliğine işaret etmektedir.

Objective: Narcolepsy is a chronic sleep disorder characterized by hypocretin/orexin deficiency, sudden loss of muscle tone (cataplexy), and excessive daytime sleepiness. The primary aim of this study is to investigate brain volumetric changes in patients with narcolepsy type 1 (NT-1) and to explore the relationship between these changes and neuropsychological symptoms, thereby contributing to a better understanding of the pathophysiology of the disease and improving clinical and therapeutic approaches. Materials and Methods: The study included a total of 50 participants, with 25 NT-1 patients and 25 healthy controls. A 3 Tesla MRI device was used for magnetic resonance (MR) imaging of the participants. Volumetric measurements were performed using MRICloud and VolBrain systems. Volume changes in brain regions including the amygdala, hippocampus (CA1-3, CA4-DG, subiculum), hypothalamus, thalamus, basal forebrain, nucleus accumbens, mesencephalon, pons, medulla, limbic system, dorsolateral prefrontal cortex, prefrontal cortex, and cerebral cortex were analyzed. All participants were administered the Epworth Sleepiness Scale, Basic Empathy Scale, Barratt Impulsiveness Scale, Buss-Perry Aggression Scale, Digit Span Test, Hamilton Rating Scale, and Stroop Test, and the results were compared with volumetric measurements. A p-value of <0.05 was considered statistically significant. Results: Significant volume loss was observed in the bilateral hippocampal CA1-3 regions and the right pons in NT-1 patients. No significant changes were found in other regions such as the hypothalamus, thalamus, basal forebrain, mesencephalon, nucleus accumbens, limbic system, prefrontal cortex, and cerebellar cortex. Neuropsychological tests indicated higher scores for depression and irritability, along with lower scores for attention and executive function in NT-1 patients. A positive correlation was found between BMI, weight gain, and volumes of brain regions such as the cerebellar cortex, prefrontal cortex, basal forebrain, nucleus accumbens, amygdala, and hippocampus. In contrast, there was a negative correlation between the Epworth Sleepiness Scale, REM sleep latency, and hippocampal volume. Depression scores showed a negative correlation with brainstem, basal forebrain, thalamus, and amygdala volumes. Conclusion: In this study, significant volume loss was identified in the bilateral hippocampal CA1-3 regions and the right pons in NT-1 patients. The findings suggest that the hippocampus and pons play a significant role in the pathophysiology of narcolepsy. The association between hippocampal volume loss, excessive daytime sleepiness, and REM sleep latency supports the critical role of the hippocampus in the sleep-wake cycle. The absence of significant volumetric changes in other brain regions suggests that structural changes in narcolepsy may be microscopic or too small to be detected with current measurement methods. Additionally, the association of neuropsychological symptoms such as depression and irritability with hippocampal and pontine atrophy underscores the impact of these structures on mood and cognitive functions. Moreover, the positive correlation between BMI and volumes of certain brain regions suggests that weight gain may lead to structural changes in the brain. This finding indicates a need for further studies that assess metabolic and structural changes together in narcolepsy patients. Overall, this study highlights the role of the hippocampus and pons in the pathophysiology of narcolepsy, emphasizing the need for more detailed functional and structural investigations.
