Özet

Amaç: Füzyon prostat biyopsisinde gereksiz örnekler alınması hastanın işlem sırasında hissedeceği ağrı miktarını artırabileceği gibi, kanama ve enfeksiyon gibi bir çok komplikasyonun ortaya çıkma olasılığını da artıran faktörlerdendir. Bu nedenle hem hekim hem de hasta açısından ideal olan, mümkün olan en az örnek sayısı ile mümkün olan en yüksek doğruluk oranlarını yakalayabilmektir. Bu çalışmanın amacı, prostat kanseri (PKa) tanısında Prostate Imaging-Reporting and Data System (PI-RADS) skorlarına göre lezyonlardan doğru histopatolojik tanı elde edebilmek için gerekli olan ideal biyopsi kor sayısını belirlemektir. Gereç ve Yöntemler: Bu prospektif çalışmaya Erciyes Üniversitesi Üroloji Kliniğinde prostat spesifik antijen (PSA) yüksekliği ve/veya çekilen multiparametrik prostat manyetik rezonans görüntülemede (mpMRG) PI-RADS 3, 4 veya 5 lezyonu olup füzyon prostat biyopsisi önerilen hastalar dâhil edilmiştir. Hastalara öncelikle 12-kor standart prostat biyopsisi uygulanmış, ardından rijit yazılımlı füzyon özellikli ultrason ile işaretlenen lezyonlardan 4-kor hedefli biyopsi uygulanmıştır. Lezyon biyopside alınan örnekler ayrı şişelerde alınma sırasına göre numaralandırılmış ve histopatolojik değerlendirmeye gönderilmiştir. Bu çalışmanın birincil sonlanım noktası çalışmaya dâhil edilen hastaların PI-RADS skorlarına göre kaçıncı örnekte kanser tespit edildiğinin belirlenmesi, ikincil sonlanım noktaları ise çalışmaya dâhil edilen hastalarda kanser tespit etme oranlarının, PI-RADS skorlarına göre kanser oranlarının, biyopsi türüne göre (hedefli biyopsi-HB, standart biyopsi-SB, kombine biyopsi -KB) kanser saptama yüzdelerinin belirlenmesidir. İstatistiksel analizlerde p değerinin 0.05'ten küçük olması anlamlı farklılık olarak kabul edilmiştir. Bulgular: Çalışmaya, yaş ortalaması 63.2 ± 7.7 yıl olan toplam 1301 hasta dâhil edilmiştir. Hastaların medyan serum PSA değerleri 6.9 (5.7-7.7) ng/mL olarak tespit edilmiştir. MpMRG'de PI-RADS 3, 4 ve 5 lezyon oranları sırasıyla %36.8, %55.3 ve %7.9 olarak belirlenmiştir ve en sık rastlanan PI-RADS skorunun PI-RADS 4 olduğu tespit edilmiştir. Tüm hastalarda PKa oranı %45, PI-RADS skorlarına göre PKa tespit oranları ise skor 3,4 ve 5 lezyonlar için sırasıyla; %28.4, %52.7 ve %68.0 olarak tespit edilmiştir. Bunun yanında biyopsi türlerine göre PKa oranları HB için %31, SB için %31.6 ve KB için %45 olarak bulunmuş olup, KB'nin istatistiksel olarak anlamlı şekilde en yüksek PKa tespit oranına sahip yöntem olduğu görülmüştür (p<0.001). HB ile lezyondan kanser tespit edilen hastalarda, PI-RADS 3 lezyonlar için en az dört kor, PI-RADS 4 lezyonlar için en az üç kor ve PI-RADS 5 lezyonlar için en az iki kor alınmasının yeterli olduğunu göstermiştir. Sonuç: Bu çalışmadan elde edilen sonuçlar, tüm füzyon biyopsilerde lezyondan standart dört örnekleme yapılması yerine, özellikle büyük ve belirgin lezyonlar olarak kabul edilen PI-RADS 5 lezyonlarda iki kor, PI-RADS 4 lezyonlarda ise üç kor örneklemenin, doğru tanıya ulaşmak için yeterli olabileceğini göstermiştir. Bu bulgular, prostat biyopsisi protokollerini optimize etmek için yol gösterici olmakla birlikte daha geniş hasta popülasyonları ve uzun dönem takiplerle desteklenmesi gereken ileri çalışmaların gerekliliğini ortaya koymaktadır.

Objective: In fusion prostate biopsy, unnecessary sampling may not only increase the level of pain experienced by the patient during the procedure but also elevate the likelihood of various complications such as bleeding and infection. Thus, it is ideal for both the physician and the patient to achieve the highest possible diagnostic accuracy with the least number of samples. The aim of this study is to determine the ideal number of biopsy cores needed to obtain an accurate histopathological diagnosis from lesions based on Prostate Imaging-Reporting and Data System (PI-RADS) scores in the diagnosis of prostate cancer (PCa). Materials and Methods: This prospective study included patients at Erciyes University Urology Clinic who had elevated prostate specific antigene (PSA) levels and/or PI-RADS 3, 4, or 5 lesions detected on multiparametric prostate magnetic resonance imaging (mpMRI) and were recommended for fusion prostate biopsy. Patients initially underwent a 12-core standard prostate biopsy, followed by a 4-core targeted biopsy of marked lesions using rigid software-assisted fusion ultrasound. The biopsy samples from the lesions were numbered in sequence and sent for histopathological evaluation in separate containers. The primary endpoint of this study was to determine at which biopsy core number cancer was detected based on the PI-RADS scores of the included patients. Secondary endpoints included determining the cancer detection rates in the included patients, the cancer rates based on PI-RADS scores, and cancer detection percentages based on biopsy type (targeted biopsy - TB, standard biopsy -SB, and combined biopsy - CB). A p-value of less than 0.05 was considered statistically significant in all analyses. Results: A total of 1,301 patients, with a mean age of 63.2 ± 7.7 years, were included in the study. The median serum PSA value of the patients was 6.9 (5.7-7.7) ng/mL. The proportions of PI-RADS 3, 4, and 5 lesions on mpMRI were found to be 36.8%, 55.3%, and 7.9%, respectively, with PI-RADS 4 being the most commonly observed score. x The overall PCa detection rate was 45%, while the detection rates for PI-RADS scores 3, 4, and 5 lesions were 28.4%, 52.7%, and 68.0%, respectively. Furthermore, PCa detection rates by biopsy type were 31% for TB, 31.6% for SB, and 45% for CB, with CB showing a statistically significant higher detection rate compared to other methods (p<0.001). For patients where cancer was detected via TB, it was demonstrated that taking at least four cores was sufficient for PI-RADS 3 lesions, at least three cores for PI-RADS 4 lesions, and at least two cores for PI-RADS 5 lesions. Conclusion : The results of this study indicate that instead of taking the standard four samples from each lesion in all fusion biopsies, sampling two cores for PI-RADS 5 lesions and three cores for PI-RADS 4 lesions, which are considered larger and more prominent, may be sufficient to reach an accurate diagnosis. These findings serve as a guide for optimizing prostate biopsy protocols but highlight the need for further studies involving larger patient populations and long-term follow-up to support these conclusions.
