Özet

Giriş: Gelişimsel kalça displazisi günümüzde hala sık görülen ve tedavi edilmediği durumda morbiditesi yüksek bir hastalıktır. Hastalığın tedavisinde yaşa göre farklı seçenekler bulunmaktadır. 18 ayın üzerindeki çocuklarda açık redüksiyon ve pelvik osteotomi ameliyatları yapılmaktadır. Salter innominate osteotomisi de bu cerrahilerden birisidir. Bu ameliyatlardan sonraki dönem hasta takiplerinde femur başı avasküler nekrozu gelişmesi olası önemli komplikasyonlardan birisidir. Tespit, takip ve tedavi edilmediği takdirde de yapılan cerrahinin başarısını önemli ölçüde azaltan bir komplikasyondur. Bu sebeple avasküler nekroza sebep olabilecek faktörlerin belirlenmesi ve bunlardan kaçınılması önem arz etmektedir. Bu çalışmamızda gelişimsel kalça displazisine sahip 18 aydan büyük çocuklarda aynı seansta yapılan açık redüksiyon ve Salter innominate osteotomisi sonrası gelişen femur başı avaküler nekrozuna sebep olabilecek faktörlerin belirlenmesi amaçlanmaktadır. Gereç ve Yöntem: Çalışmada 2013-2022 yılları arasında Gaziantep Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim dalı tarafından gelişimsel kalça displazili, 18 aydan büyük, aynı seansta Salter innominate osteotomisi ve açık redüksiyon ameliyatı yapılan hastalar ameliyat sonrası dönemde femur başı avasküler nekroz gelişimi açısından incelendi. Ameliyat sonrası en az 2 yıl takip süresine sahip 95 hasta üzerinde çalışma gerçekleştirildi. Hastaların ameliyat öncesi ve sonrası dönemde pelvis grafileri, poliklinik, ameliyat ve epikriz notları incelendi. Yapılan ölçümler, demografik veriler, biyokimyasal ve radyolojik parametreler açısından gruplar birbirleri arasında istatistiksel olarak kıyaslandı. Bulgular: Çalışmaya dahil edilen hasta sayısı 95 idi. Hastaların 18 tanesi erkek, 77 tanesi kızdı. Hastaların 43 tanesinde postoperatif dönem takiplerinde femur başı avasküler nekrozu geliştiği tespit edildi. Bu durumla ilişkili olabilecek toplam 22 parametre değerlendirildi. Bu parametrelerden femoral osteotomi uygulaması, pelvipedal alçı abdüksiyon derecesi, preoperatif Tönnis evresi, preoperatif asetabular indeks açısı, addüktör tenotomi, femur başı epifiz çekirdeği ile Hilgenreiner çizgisi arası mesafe ve femur başı epifiz çekirdeği ile y kıkırdağı arası mesafe ile femur başı avasküler nekrozu gelişimi arasında istatistiksel olarak anlamlı bir ilişki olduğunu saptadık (p<0,05). Sonuç: Salter innominate osteotomisi ve açık redüksiyon ameliyatı gelişimsel kalça displazili hastalarda sık uygulanan ve sonuçları yüz güldürücü bir cerrahidir. Femur başı avasküler nekrozu bu cerrahinin başarısını olumsuz etkileyen en önemli komplikasyonlardan birisidir. Sık görülen bir komplikasyon olması sebebiyle predispozan faktörlerin belirlenmesi ve bunlardan kaçınılması ile ilgili çözüm arayışları devam etmektedir. Başarılı bir cerrahi ve sonrasında avasküler nekrozdan kaçınmak adına, cerrah kalça eklemi anatomisini iyi bilmeli, uygun hastaya uygun teknikle ameliyatı yapmalıdır. Hastaya ait ve cerrahiden bağımsız olan risk faktörlerini tespit etmeli ve ameliyat sonrası dönemde yakın takip ile avasküler nekroz gelişimi açısından dikkatli olunmalıdır. Ameliyat öncesi her hasta kendi içerisinde detaylı olarak değerlendirilmeli, ameliyat sırasında yumuşak dokuya saygılı davranıp gerekli yumuşak doku gevşetmeleri ve kemik kesileri yapılmalıdır. Kalça eklemi içerisinde basınç artışına neden olarak femur başı avasküler nekrozu gelişimine katkı sağlayan faktörlerden kaçınılması durumunda bu komplikasyonun en aza indirileceği görüşündeyiz. Ancak mevcut bulguların geniş katılımlı ve uzun süreli çalışmalardan elde edilecek verilerle desteklenmeye ihtiyacı vardır.

Purpose: Developmental hip dysplasia is a disease that is still common today and has high morbidity if left untreated. There are different options for the treatment of the disease depending on age. Open reduction and pelvic osteotomy surgeries are performed in children over 18 months. Salter innominate osteotomy is one of these surgeries. Avascular necrosis of the femoral head is one of the possible important complications in the follow-up period after these surgeries. If it is not detected, followed up and treated, it is a complication that significantly reduces the success of the surgery. For this reason, it is important to identify factors that may cause avascular necrosis and avoid them. In this study, we aim to determine the factors that may cause avascular necrosis of the femoral head after open reduction and Salter innominate osteotomy in children older than 18 months with developmental hip dysplasia. Material and Method: In the study, patients with developmental hip dysplasia, older than 18 months, who underwent Salter innominate osteotomy and open reduction surgery in the same session by Gaziantep University Faculty of Medicine, Department of Orthopedics and Traumatology, between 2013 and 2022, were examined for the development of femoral head avascular necrosis in the postoperative period. The study was conducted on 95 patients with a follow-up period of at least 2 years after surgery. The patients' preoperative and postoperative pelvic radiographs, outpatient clinic, surgery and epicrisis notes were examined. The groups were compared statistically among each other in terms of measurements, demographic data, biochemical and radiological parameters. Results: The number of patients included in the study was 95. 18 of the patients were boys and 77 were girls. It was determined that femoral head avascular necrosis developed in 43 of the patients during postoperative follow-up. A total of 22 parameters that could be related to this condition were evaluated. Among these parameters, femoral osteotomy application, pelvipedal cast abduction degree, preoperative Tönnis stage, preoperative acetabular index angle, adductor tenotomy, distance between the femoral epiphyseal nucleus and Hilgenreiner line, and the distance between the femoral epiphyseal nucleus and y cartilage have no statistical relationship with the development of femoral head avascular necrosis. We found that there was a significant relationship (p<0,05). Conclusion: Salter innominate osteotomy and open reduction surgery is a frequently performed surgery with satisfactory results in patients with developmental hip dysplasia. Femoral head avascular necrosis is one of the most important complications that negatively affects the success of this surgery. Since it is a common complication, a solution has been sought to identify and avoid predisposing factors. In order to have a successful surgery and avoid subsequent avascular necrosis, the surgeon must know the anatomy of the hip joint well and perform the vii surgery with the appropriate technique for the appropriate patient. Risk factors that belong to the patient and are independent of surgery should be identified and close follow-up should be followed in the postoperative period to be careful about the development of avascular necrosis. Before the surgery, each patient should be evaluated in detail and the soft tissue should be treated respectfully during the surgery and necessary soft tissue releases and bone incisions should be made. We believe that this complication will be minimized if factors that contribute to the development of femoral head avascular necrosis by causing increased pressure within the hip joint are avoided. However, current findings need to be supported by data obtained from large-scale and long-term studies.
