Özet
Amaç: Bu çalışmanın amacı, pilonidal sinüs hastalığının tedavisinde kristalize fenol uygulaması ve unroofing küretaj yöntemlerinin sonuçlarını karşılaştırmaktır. Gereç ve Yöntem: Ocak 2023 ile Kasım 2023 arasında İstanbul Eğitim ve Araştırma Hastanesi polikliniğine başvuran 48 hasta çalışmaya alınmıştır. Çalışma prospektif randomize kohort çalışma olarak planlanmıştır. Hastalar 3 gruba eşit sayıda randomize olarak dağıtılmıştır. 1. Gruba Kristalize fenol uygulaması, 2. Gruba unroofing küretaj yöntemi ve 3. Gruba kombine yöntem uygulanmıştır. Tüm işlemler lokal anestezi altında gerçekleştirilmiştir. Bulgular: Kristalize fenol (n=16), unroofing küretaj (n=16) ve kombine yöntem (n=16) grupları arasında işe dönüş süresi analiz edildiğinde, kristalize fenol grubunda unroofing küretaj grubuna kıyasla daha kısa bulunmuştur (p<0.001), kristalize fenol grubunda kombine yöntem grubuna kıyasla daha kısa bulunmuştur (p=0.011). VAS skorları karşılaştırıldığında, 2. ve 5. günlerdeki hissedilen en şiddetli ağrı analiz edildiğinde gruplar arasında istatistiksel olarak anlamlı farklılık saptanmıştır (p=0.037 ve p=0.03). Altı aylık takip süresinde, 48 hastanın hiçbirinde nüks tespit edilmedi. Sonuç: Çalışmamızda kristalize fenol uygulaması ile unroofing küretaj yöntemleri karşılaştırıldığında ağrı ve işe dönüş süresi açısından kristalize fenol yöntemi daha avantajlı bulunmuştur.

Aim: This study aimed to compare the results of the crystalized phenol treatment and unroofing curettage techniques in the treatment of pilonidal sinus disease. Materials and Methods: Between January 2023 and November 2023, 48 patients who applied to the Istanbul Training and Research Hospital outpatient clinic were enrolled in the study. The study was planned as a prospective randomized cohort study. Patients were randomly allocated into three groups with equal numbers. The 1st group received crystallized phenol application, the 2nd group underwent unroofing curettage method, and the 3rd group received combined method. All procedures were performed under local anesthesia. Results: When the return to work duration was analyzed among the groups of crystallized phenol (n=16), unroofing curettage (n=16), and combined method (n=16), it was found to be shorter in the crystallized phenol group compared to the unroofing curettage group (p<0.001), and shorter in the crystallized phenol group compared to the combined method group (p=0.011). When comparing the VAS scores, statistically significant differences were observed between the groups in terms of the most severe pain felt on the 2nd and 5th days of analysis (p=0.037 and p=0.03). In the 6-month follow-up, no recurrence was detected in any of the 48 patients. Conclusions: In our study, when crystallized phenol application was compared with the unroofing curettage methods, crystallized phenol was found to be more advantageous in terms of pain and return to work duration.
