Özet

Çalışmamızın amacı; inme hastalarında EXOPULSE Mollii Suit Giyilebilir Elektrik Stimülasyon uygulamasının spastisite şiddeti, ağrı, yorgunluk, üst ve alt ekstremite fonksiyonları, denge, yürüme, endurans ve el fonksiyonları üzerine akut etkilerini klinik kontrollü bir şekilde incelemekti. Çalışmada inme tanısı almış olan 27 birey tedavi (n=15, yaş: 57,00 ± 6,24 yıl) ve kontrol (n=12, yaş: 57,08 ± 14,06) gruplarından birine rastgele olarak atandı. Bireylerin üst ve alt ekstremite kaslarının spastisite şiddetleri Modifiye Ashworth Skalası (MAS) ile değerlendirildi. Sonuçlar Mollii Suit sisteminin bilgisayar arayüzüne kaydedildi. Sonrasında hastalara Mollii Suit sisteminin kıyafetleri giydirildi. Mollii grubuna sistemin öngördüğü şiddette elektrik stimülasyonu giyilebilir kıyafet aracılığı ile 1 saat süre ile uygulandı (kıyafet içerisinde 56 adet gömülü silikon elektrot aracılığı ile). Kontrol grubuna ise EXOPULSE Mollii sisteminin kıyafetleri giydirilerek sadece boyun ve sağlam ekstremitelerine minimum şiddette elektrik stimülasyon uygulaması 1 saat boyunca yapıldı. Değerlendirmeler kör bir araştırmacı tarafından Mollii Suit kıyafetini giymeden hemen önce ve uygulamadan hemen sonra olmak üzere toplam 2 kez yapıldı. Bireylerin ağrı, yorgunluk ve memnuniyet düzeyleri Görsel Analog Skalası (GAS), el performansı 9 Delikli Peg Testi, fonksiyonel kapasite STREAM Ölçeği, alt ekstremite fonksiyonel düzeyi 5 Kez Otur-Kalk Testi, fonksiyonel mobilite becerileri Zamanlı Kalk ve Yürü Testi, yürüme performansı ise 10 Metre Yürüme Testi ile değerlendirildi. Çalışmanın sonuçları incelendiğinde Mollii grubunda parmak fleksörleri ve plantar fleksör kasların spastisite şiddetinde azalma olduğu (p<0,05). STREAM ölçeğinde Mollii grubunda grup içi anlamlı iyileşme bulundu. Beş Kez Otur-Kalk Testi'nde her iki grupta gelişme olduğu görüldü (p<0,05). On Metre Yürüme Testinde Mollii grubunda, Zamanlı Kalk ve Yürü Testi'nde ise kontrol grubunda grup içi anlamlı iyileşme olduğu tespit edildi (p<0,05). Çalışma memnuniyeti değerlendirmesinde ise Mollii grubu lehine gruplar arasında anlamlı fark bulundu (p<0,05). Ağrı, yorgunluk ve üst ekstremite performansı ölçümlerinde her iki grupta, grup içi ve gruplar arasında istatistiksel olarak anlamlı fark olmadığı tespit edildi (p>0,05). Sonuç olarak, Exopulse Mollii Suit giyilebilir elektrik stimülasyon sisteminin spastisite, fonksiyonel kapasite ve yürüyüş performansı üzerine akut olarak olumlu etkiler gösterdiği; buna karşın ağrı, yorgunluk, üst ekstremite fonksiyonları ve gruplar arası genel alt ekstremite performansı üzerinde belirgin bir akut etkisinin olmadığı bulundu. Bu sonuçlar, EXOPULSE Mollii Suit'in inme rehabilitasyonunda tamamlayıcı bir müdahale olarak umut verici sonuçlar ortaya koyduğunu ancak etkinliğinin geniş hasta grupları, fonksiyonel çıktılar ve uzun dönem etkiler açısından değerlendirilmesi için daha kapsamlı çalışmalara ihtiyaç olduğunu göstermektedir.

The aim of this study was to clinically and in a controlled manner investigate the acute effects of the EXOPULSE Mollii Suit wearable electrical stimulation on spasticity severity, pain, fatigue, upper and lower extremity functions, balance, gait, endurance, and hand functions in patients with stroke.A total of 27 individuals diagnosed with stroke were randomly assigned to either the treatment group (n=15; age:57.00±6.24 years) or the control group (n=12; age:57.08±14.06years). Spasticity severity of the upper and lower extremity muscles was assessed using the Modified Ashworth Scale (MAS), and the results were recorded via the Mollii Suit system's computer interface. Subsequently, the Mollii Suit garments were worn by all participants.In the Mollii group, electrical stimulation was applied at the intensity prescribed by the system through the wearable garment for 1 hour via 56 embedded silicone electrodes. In the control group, the Mollii Suit garments were also worn; however, only minimal-intensity electrical stimulation was applied to the neck and unaffected extremities for 1 hour.All assessments were conducted by a blinded assessor immediately before wearing the Mollii Suit and immediately after the intervention. Pain, fatigue, and satisfaction levels were evaluated using the Visual Analog Scale (VAS). Hand performance was assessed with the Nine-Hole Peg Test, functional capacity with the STREAM Scale, lower extremity functional level with the Five Times Sit-to-Stand Test, functional mobility with the Timed Up and Go Test, and gait performance with the 10-Meter Walk Test.The results demonstrated a significant reduction in spasticity severity of the finger flexor and plantar flexor muscles in the Mollii group (p<0.05). A significant within-group improvement was observed in the Mollii group on the STREAM Scale. Improvements in the Five Times Sit-to-Stand Test were noted in both groups (p<0.05). Significant within-group improvements were found in the Mollii group for the 10-Meter Walk Test, and in the control group for the Timed Up and Go Test (p<0.05). In terms of treatment satisfaction, a significant between-group difference was found in favor of the Mollii group (p<0.05).No statistically significant within-group or between-group differences were observed in pain, fatigue, or upper extremity performance in either group (p>0.05).In conclusion, the EXOPULSE Mollii Suit wearable electrical stimulation system demonstrated acute positive effects on spasticity, functional capacity, and gait performance; however, no significant acute effects were found on pain, fatigue, upper extremity functions, or overall between-group lower extremity performance. These findings suggest that the EXOPULSE Mollii Suit may serve as a promising complementary intervention in stroke rehabilitation; nevertheless, further comprehensive studies with larger sample sizes, broader functional outcomes, and long-term follow-up are required to better determine its effectiveness.
