Özet

Günümüzde, yardımcı üreme tekniklerinin (YÜT) giderek yaygınlaşması ile birçok infertil bireyin, çocuk sahibi olabilmesine olanak sağlanmıştır. Çalışmamız yardımcı üreme teknikleri ile klinik gebelik oranlarını iyileştirmek üzere, sperm elde etme yöntemleri olan ejakülat ve m-TESE kullanımlarını kıyaslayarak fertilizasyon oranları üzerine etkilerini incemeleyi amaçlamaktadır. Çalışmamızda, İstanbul Eurofertil Tüp Bebek Merkezine, 2017-2022 tarihlerinde, çocuk sahibi olabilmek üzere başvurmuş olan, toplam 1925 çiftten, kadın yaşı 25-38 yaş aralığında olup, ICSI uygulamasında 1407 ejekülat spermi, 140 ise m-TESE ile elde edilen sperm kullanılan toplam 1547 hasta dahil edildi. Çalışmamızda ejekülat spermi kullanılan 9663 oosit, m-TESE spermi kullanıllan 888 oosit'in fertilizasyon ve anomalileri değerlendirildi. Her iki gurup, oosit bazında kıyaslandığı zaman ejakülat gurubunun %85'inde, m-TESE gurubunun %75'inde ICSI sonrası 2PN görülmüştür ve bu istatistiksel olarak anlamlı olarak bulunmuştur (p=0,001). 2PN sonrası klivaj değerlendirilmiş ancak iki grupta farklılık görülmemiştir. 1PN oranı ve fertilize olmama oranları, ejakülat gurubuna göre kıyaslandığında m-TESE grubunda istatistiksel olarak anlamlı bulunmuştur (p=0,001). Sonuç olarak, çalışmamızda m-TESE yöntemi ile elde edilem spermlerin, fertilizasyon oranının daha düşük, fertilizasyon yetersizliklerinin daha fazla, 1PN fertilizasyon anomalilerinin daha yüksek olduğu görüldü. Bu sonucun sperm nükleer ve sitoplazmik maturasyonu ile ilişkili olabileceği konusunun detaylı incelenmesi gerektiği düşünüldü. NOA hasta gurubunda bu sürecin epididimal maturasyonun tamamlanmama'sına rağmen, başarılı gebelikler ve canlı doğumun olması, günümüz infertilite hastalarında, m-TESE yönteminin hala altın standart olarak kullanılmasını desteklediği görülmektedir.

Nowadays, with the widespread use of assisted reproductive techniques (ART), it has become possible for many infertile individuals to have children. Our study aims to examine the effect of assisted reproductive techniques on fertilization rates by comparing the use of ejaculate and m-TESE, which are sperm retrieval methods, to improve clinical pregnancy rates. Our study evaluated the ICSI results of a total of 1547 patients, women aged 25-38, from 1925 couples who applied to the Istanbul Eurofertil In Vitro Fertilization Center between 2017 and 2022 to have a child. In 1407 of the applications, sperm obtained by ejaculate was used, and in 140 cases, m-Tese sperm was used. In our study, fertilization, and anomalies of 9663 oocytes using ejaculated sperm and 888 oocytes using m-Tese sperm were evaluated. When both groups were compared on an oocyte basis, 2PN was observed in 85% of the post-ICSI ejaculate group and 75% of the m-TESE group, and this result was statistically significant (p = 0.001). The division was evaluated after 2PN, but no difference was seen in the two groups. The 1PN rate and non-fertilization rates were statistically significant in the m-TESE group compared to the ejaculate group (p=0.001). As a result, our study observed that the sperm obtained by the m-TESE method had a lower fertilization rate, more fertilization deficiencies, and a higher rate of 1PN fertilization anomalies. This result might be related to sperm nuclear and cytoplasmic maturation and should be examined in detail. Although the epididymal maturation of this process has not yet been completed in the NAO patient group, successful pregnancies and live births seem to support the use of the m-TESE method as the gold standard in today's infertility patients.
