Özet
Giriş ve Amaç Toraks cerrahisi sonrası kronik ağrı oldukça yaygın görünen bir durumdur. Post torakotomi ağrı sendromu (PTAS) olarak da adlandırılan bu durum hastaların günlük yaşam aktivitelerinde kısıtlamaya neden olabilmektedir. PTAS'a neden olan mekanizmalar tam olarak bilinmemekle birlikte intraoperatif sinir hasarı, önceden var olan nöropatiler ve operasyon teknikleri gibi çeşitli etkenlerin PTAS'a sebep olabileceği düşünülmektedir. İntravenöz analjezikler, opioidler, torakal epidural analjezi, erektör spina plan bloğu (ESPB) ve paravertebral blok (PVB) gibi rejyonel anestezi yöntemleri hem toraks cerrahisinin postoperatif analjezisini sağlamak hem de PTAS gelişmesini önlemek amacıyla kullanılan yöntemler arasında yer almaktadır. Rejyonal anestezi teknikleri son yıllarda toraks cerrahisinde analjezi sağlamak amacıyla sıklıkla kullanılarak kılavuzlarda da yerini almıştır. Bu çalışmada torakotomi yapılacak hastalara preoperatif uygulanan ESPB ve PVB yapılarak PTAS gelişimine etkisi amaçlandı. Materyal ve Metod 18-70 yaş aralığında, ASA I-III skoruna sahip, torakotomi ile lobektomi yapılan, ilaç alerjisi olmayan, kronik analjezik ilaç kullanımı/bağımlılığı olmayan, kronik ağrısı olmayan, daha önce torakotomi olmayan tüm erişkin hastalar çalışmaya dahil edilmiş olup her grupta 30'ar hasta olacak şekilde toplam 60 hasta ESPB ve PVB grupları olarak 2 gruba ayrılmış ve hastaların demografik özellikleri, intraoperatif hemodinamik parametreler, sıvı infüzyon miktarları, , ameliyat ve anestezi süreleri, intraoperatif ve postoperatif komplikasyonlar, postoperatif 0,2,6,12,24 saat, 2.hafta ve 2.ay'da Vizüel Analog Skala(VAS) ve Prince Henry Hospital Pain Score(PHHPS) kullanılarak değerlendirilen ağrı düzeyleri, gruplar arasında HKA toplam ortalama talep sayısı, ortalama toplam opioid kullanımı ve kurtarıcı analjezik gereksinimi parametrelerine ait değerler karşılaştırıldı. Bulgular Çalışmamızda her iki grubun değerleri karşılaştırıldığında; Postoperatif 0. Saat VAS skoru PVB grubunda ESPB grubuna göre istatistiksel olarak anlamlı şekilde düşük bulundu. Diğer zaman dilimlerinde VAS değerleri arasında istatistiksel olarak anlamlı fark bulunmadı. PHHPS takibinde PVB'nin 0,2,6 ve 12. saatlerde ESPB'ye göre anlamlı derecede daha düşük olduğu gözlenmiştir. 2.ayda PVB grubunda 1(%3,3), ESPB grubunda 2(%6,6) hastada olmak üzere toplam 3 hastada PTAS geliştiği gözlendi. PTAS; PVB grubunda ESPB gruna göre oransal olarak daha düşük oranda gözlenmekle birlikte istitatistiksel olarak gruplar arası anlamlı fark bulunmadı. PVB uygulanan hastalarda HKA kullanım miktarı ve kurtarma analjeziği kullanan hasta sayısı ESPB'a göre anlamlı derecede daha düşük bulunmuştur. Sonuç PTAS gelişimini önlemek amacıyla hem ESPB hem de PVB etkin bir şekilde kullanılabilir ve birbirlerine alternatif olarak görülebilirler.

Introduction and purpose Chronic pain after thoracic surgery is a very common condition. This condition, also called post thoracotomy pain syndrome (PTAS), can cause restrictions in patients' daily life activities. Although the mechanisms that cause PTAS are not fully known, it is thought that various factors such as intraoperative nerve damage, pre-existing neuropathies and operating techniques may cause PTAS. Regional anesthesia methods such as intravenous analgesics, opioids, thoracic epidural analgesia, erector spinae plane block (ESPB) and paravertebral block (PVB) are among the methods used to provide postoperative analgesia of thoracic surgery and to prevent the development of PTAS. Regional anesthesia techniques have been frequently used to provide analgesia in thoracic surgery in recent years and have taken their place in the guidelines. In this study, the effect of preoperative ESPB and PVB on the development of PTAS was aimed in patients undergoing thoracotomy. Material and Method All adult patients between the ages of 18-70, with ASA I-III scores, who underwent lobectomy with thoracotomy, who did not have drug allergies, who did not have chronic analgesic drug use/addiction, who did not have chronic pain, and who had not had a thoracotomy before, were included in the study, 30 in each group. A total of 60 patients were divided into 2 groups as ESPB and PVB groups and the demographic characteristics of the patients, intraoperative hemodynamic parameters, fluid infusion amounts, surgery and anesthesia durations, intraoperative and postoperative complications, postoperative 0, 2, 6, 12, 24 hours, Pain levels evaluated using the Visual Analog Scale (VAS) and Prince Henry Hospital Pain Score (PHHPS) at the 2nd week and 2nd month, and the values of the total average number of PCA requests, average total opioid use and rescue analgesic requirement parameters were compared between the groups. Results When the values of both groups were compared in our study, the postoperative 0th hour VAS score was statistically significantly lower in the PVB group than in the ESPB group. There was no statistically significant difference between VAS values at other time periods. In the follow-up of PHHPS, PVB was significantly lower than ESPB at 0,2,6 and 12 hours. PTAS developed in a total of 3 patients, 1 (3.3%) in the PVB group and 2 (6.6%) in the ESPB group at the 2nd month. Although PTAS was observed at a proportionally lower rate in the PVB group compared to the ESPB group, there was no statistically significant difference between the groups. The amount of PCA use and the number of patients using rescue analgesics were found to be significantly lower in patients with PVB compared to ESPB. Conclusion In order to prevent the development of PTAS, both ESPB and PVB can be used effectively and can be seen as alternatives to each other.
