Özet

Amaç: Multiple Skleroz Hastalarında denge, postür ve yürüyüş performansı özürlülüğün değerlendirilmesi, prognozun belirlenmesinde önemli parametrelerdir. Çalışmamızda MS hastalarında denge ve vestibüler sisteminin; Aktiviteye Özgü Güven (ABC), Berg Denge, 12 madde MS yürüyüş testleri (MSWS-12), Baş dönmesi engellik testi (DHI), Uluslararası Düşme Anketi (FES-1) ve bilgisayarlı dinamik postürografi (BDP) ile değerlendirilmesini, elde edilen sonuçların denge, postür ve ilişkili özürlülüğün öngörülmesinde kullanılıp kullanılamayacağını değerlendirmeyi amaçladık. Gereç ve Yöntem: Çalışmaya 2017 McDonald Kriterleri'ne göre kesin RRMS tanısı alan, 18 -55 yaş arası, EDSS skoru 3 ve altında, son 3 ay içinde atak öyküsü olmayan, kranial ve spinal manyetik rezonans görüntülemelerinde denge ile ilgili alanlarda lezyonu olmayan 35 hasta ile Nöroloji Polikliniğine baş ağrısı şikayeti ile başvuran nörolojik muayene ve kranial görüntülemeleri normal 35 kontrol dahil edildi. Her iki grubun ayrıntılı nörolojik muayeneleri, Berg Denge ve ABC testleri, DHI, FES-1ve postürografik değerlendirmeleri yapıldı. MS hastalarına, MSWS-12 anketi uygulandı. Tüm hastaların atak sıklığı, atak tipi, EDSS skorları, kranial ve spinal MRG lezyon dağılımları kayıt edildi. Hasta ve kontrol grubunun arasındaki postürografik skorları ve anket sonuçları karşılaştırıldı. Bulgular: Çalışmamızda hasta ve kontrol grubu arasında yaş, cinsiyet, boy, kilo ve VKI açısından anlamlı fark izlenmedi (p>0.05). Hasta grubunda değerlendirilen Postürografi sonuçlarındaki tüm skorlar (-SOM, -VİS, -VEST, -PREF ve -TOTAL) kontrol grubuna göre istatistiksel olarak anlamlı düşük saptandı (p<0,05). En belirgin skor düşüklüğü Postürografi-VEST basamağında gözlendi. EDSS skorlarının dağılımı ile Postürografi skorları arasında anlamlı ilişki saptamazken hastalık süresi ile Postürografi-VEST skoru açısından istatistiksel olarak anlamlı ilişki saptandı (p<0,05). ABC ve Berg Denge Ölçeklerinin ortalamaları hasta grubunda istatistiksel olarak anlamlı düşük, DHI ve FES-1 puanları anlamlı yüksek saptandı (p<0,05). MS hasta grubunda Berg Denge Ölçeği ile Postürografi-VİS, Postürografi-VEST ve Postürografi-Total arasında, FES-1 ile Postürografi-VEST arasında anlamlı ilişki saptandı (p=0,033). MS hasta grubunda Romberg pozitif, tandem yürüyüş yapamayanlarda Postürografi – VEST ve -Total sonuçları istatistiksel olarak anlamlı yüksekti (p < 0,05). Sonuç: Bu çalışmada, EDSS düşük ve görüntülemelerinde belirgin bulgunun olmadığı MS hastalarında hem denge ile ilgili sübjektif testlerde hem objektif posturografik değerlendirmede (özellikle vestibüler basamağında) etkilenmenin daha belirgin olduğu saptandı. Benzer şekilde, Romberg ve tandem yürüyüşte etkilenmesi olan hastalarında posturografi incelemelerinde vestibüler skorlarında düşüklük gözlendi. Bu sonuçlar, belirgin özürlülüğü olmayan MS hastalarında dengenin özellikle vestibüler basamağında etkilenmeyi göstermesi ve hastaların erken dönemden itibaren denge testleri ve nörolojik muayene ile takibinin önemini vurgulaması açısından değerlidir.

Objective: To evaluate balance and vestibular system in MS patients we used ABC test, Berg Balance test, 12-item MS gait tests, DHI (dizziness handicap inventory), Fall Questionnaire and dynamic computerized posturography. We aimed if it is possible to use dynamic computerized posturography to evaluate balance and vestibular system of MS patients in the absence of complaints about imbalance and dizziness. Materials and Methods: The study was conducted in Neurology Clinic of Istanbul Training and Research Hospital between September 15, 2022 and August 15, 2023, patients who were diagnosed with MS according to the 2017 McDonald Criteria, over 18 years old, under 55 years old, had an EDSS score of 3 and below, volunteered to participate in the study, without the history of attacks in the last 3 months, magnetic resonance imaging in the system, no obvious lesions in the areas related to balance in cranial and spinal magnetic resonance imaging.And we include 35 controls without disease and without complaints of dizziness and imbalance. ABC test, Berg Balance test, DHI (dizziness handicap inventory), Fall Questionnaire and posturographic evaluation were performed on 70 cases. An additional 12-item MS gait questionnaire was administered to 35 patients with MS. Posturographic scores and survey results between the patient and control groups were compared, and the posturography score according to the EDSS value of the patients in the MS group, and the correlation between balance questionnaires and posturography scores were evaluated. Results: In our study no significant difference was observed between the patient and control groups in terms of age, gender, height, weight and BMI (p>0,05). In MS patient group all scores of the Posturography results (-SOM, -VIS, -VEST, -PREF and -TOTAL) were found to be statistically significantly lower than the control group (p<0,05). The most significant decrease in score was observed in the Posturography-VEST step. While there was no significant relationship between the EDSS scores and Posturography scores, a statistically significant relationship was found between disease duration and Posturography-VEST score (p<0,05). Averages of ABC and Berg Balance Scales were found to be statistically significantly lower in the patient group, and DHI and FES-1 scores were significantly higher (p<0,05). In the MS patient group, a significant relationship was observed between the Berg Balance Scale and Posturography-VIS, Posturography-VEST and Posturography-Total, and between FES-1 and Posturography-VEST scores (p=0,033). In the MS patient group, Posturography-VEST and Posturography-Total results were statistically significantly lower in those who were Romberg positive and could not walk in tandem (p<0,05). Conclusion: It was observed that MS patients with low EDSS and no obvious findings on imaging have more affected scores in both subjective balance-related tests and objective posturographic evaluation (especially in the vestibular step) than the control group. Similarly, low vestibular scores in posturography were observed in MS patients with impaired tandem gait and/or Romberg positiveness. These results are valuable because they show that balance is affected (especially in the vestibular stage) in MS patients without significant disability. And this study emphasize the importance of monitoring patients by using balance tests and neurological examination from the early stages.
