Özet

GİRİŞ: Kanser fiziksel, sosyal, ekonomik ve ruhsal yönlerden ağır yük oluşturan ciddi bir hastalıktır. Günümüzde kansere yönelik verilen tedaviler ve bu tedavilere bağlı yaşanan yan etkiler, birçok kanser hastasının acil servisler başta olmak üzere sağlık hizmetlerinin kullanımının artmasında etkili olmuştur. 2019 yılında Çin'in Wuhan kentinde ortaya çıkan ve kısa sürede tüm dünyada etkileri görülen COVİD-19 pandemisinin, kanser hastalarının acil servis başvuruları ile ilişkisi hakkında literatürde sınırlı sayıda çalışma mevcuttur. AMAÇ: Bu çalışmada, kanser hastalarının acil servis başvurularının özellikleri ve COVİD-19 pandemisinin bu başvurular üzerine olan etkileri araştırılmıştır. GEREÇ VE YÖNTEM: Çalışmamıza Dokuz Eylül Üniversitesi Hastanesi Acil Servis kliniğine 01/04/2018-01/07/2018 ve 01/04/2020-01/07/2020 tarihleri arasında başvuran ve Tıbbi Onkoloji bölümüne konsültasyon istenen, kanser tanılı 279 hasta dahil edildi. Bu hastaların hastane veri tabanı üzerindeki anamnez verileri, laboratuvar ve görüntüleme tetkikleri retrospektif olarak incelendi. BULGULAR: Acil servise başvuran hastaların %62,4'ü erkekti. Hastaların yaş ortalaması 62.5±11,63 idi. COVİD-19 pandemisi döneminde (%34,6) kanserli hastaların acil servis başvurularında, COVİD-19 öncesi döneme (%65,4) göre belirgin ölçüde azalma görüldü. Acil servise başvuran hastalar en sık (%24,7) solunum sistemi semptomları ile başvurmakta idi. Başvuru semptomları arasında kanama grubunda COVİD-19 döneminde artış (%12,5) görülmüş olup; istatistiksel açıdan anlamlı idi (p<0,05). Acil servise başvuran hastaların %49,1'inin önlenebilir bir semptomla başvurduğu görüldü; en sık önlenebilir neden ateş (%36,5) idi. Acil servise başvuran hastaların %67,4'üne yatış endikasyonu konulmuş idi ve bunların %83'ü metastatik evrede idi. Acil servise önlenebilir nedenlerle başvuran hastaların %54,7'sine yatış endikasyonu konulmuş idi. COVİD-19 döneminde önlenebilir nedenlerle başvurup yatış endikasyonu konulan hastaların (%48,9) oranında bir düşüş görülmüş olup; istatistiksel açıdan anlamlı idi (p<0,05). Acil servise başvuran ve yatış endikasyonu konulan hastaların CAR (CRP/Albümin) oranlarının mediyanı (38,78); taburcu olan hastalara (26,58) göre daha yüksekti ve bu fark istatiksel açıdan anlamlı idi. SONUÇ: Çalışmamızda, literatür ile benzer şekilde COVİD-19 pandemisi döneminde kanserli hastaların acil servis başvurularında belirgin azalma olduğu görüldü. Kanser hastalarının acil servis başvurularındaki önlenebilir nedenleri belirlemek, hem finansal hem de acil servislerin etkin kullanımının yönetimi açısından oldukça değerli olacaktır. Acil servise önlenebilir nedenlerle başvuran hastaların COVİD-19 döneminde yatış oranlarında azalma görülmesi, bu başvuruların önemli bir kısmının ayaktan tedavi edilebileceğini ve buna yönelik ülkemizde çalışmalar yapılması gerektiği gerçeğini bizlere göstermektedir.

INTRODUCTION: Cancer is a serious illness that poses a significant burden in terms of physical, social, economic, and psychological aspects. The treatments provided for cancer and the associated side effects have contributed to the increased utilization of healthcare services, including emergency departments, by many cancer patients. There are limited studies in the literature regarding the relationship between the COVID-19 pandemic, which emerged in Wuhan, China in 2019 and quickly spread worldwide, and the emergency department visits of cancer patients. OBJECTIVE: In this study, aimed to investigate the characteristics of emergency department visits by cancer patients and the effects of the COVID-19 pandemic on these visits. MATERIAL AND METHOD: Our study included 279 patients diagnosed with cancer who presented to the Emergency Department clinic of Dokuz Eylül University between 01/04/2018-01/07/2018 and 01/04/2020-01/07/2020, and were consulted to the Department of Medical Oncology. The medical history, laboratory, and imaging test results of these patients were retrospectively reviewed from the hospital database. RESULTS: 62.4% of the patients who presented to the emergency department were male. The mean age of the patients was 62.5±11.63. During the COVID-19 pandemic, there was a significant decrease in emergency department visits of cancer patients (34.6%) compared to the pre-COVID-19 period (65.4%). The most common reason for patients to present to the emergency department was respiratory system symptoms (24.7%). Among the presenting symptoms, there was a statistically significant increase in bleeding cases during the COVID-19 period (12.5%, p<0.05). It was observed that 49.1% of the patients who presented to the emergency department had a preventable symptom, with fever (36.5%) being the most common preventable reason. Among the patients who presented to the emergency department, 67.4% were hospitalized, and 83% of these patients were in the metastatic stage. Among the patients who presented to the emergency department with preventable reasons, 54.7% were hospitalized. During the COVID-19 period, there was a significant decrease in the rate of hospitalization for patients who presented with preventable reasons (48.9%, p<0.05). The median CAR (CRP/Albumin ratio) of the patients who were hospitalized and presented to the emergency department was higher (38,78) compared to those who were discharged (26,58), and this difference was statistically significant. CONCLUSION: In our study, similar to the literature, a significant decrease was observed in emergency department visits of cancer patients during the COVID-19 pandemic. Identifying the preventable reasons for cancer patients' emergency department visits would be valuable in terms of both financial management and effective utilization of emergency services. The decrease in hospitalization rates among patients who presented to the emergency department with preventable reasons during the COVID-19 period indicates that a significant portion of these visits can be managed on an outpatient basis, highlighting the need for studies in our country focusing on this aspect.
