Özet

Amaç: Çalışmamızın amacı, ön çapraz bağ cerrahisinde sadece semitendinosus tendonunun greft olarak kullanılmasının diz ekleminin fleksiyon kuvvetine, tibial iç rotasyon kuvvetine, diz ekleminin stabilitesine etkisinin semitendinosus ve gracilis tendonlarının birlikte kullanıldığı hastalarla karşılaştırılarak değerlendirilmesidir. Materyal ve metod: Çalışmamızda hamstring tendon otogrefti kullanılarak hepsi içerde tekniğiyle primer artroskopik ön çapraz bağ rekonstrüksiyonu (ÖÇBR) yapılan hastalar geriye dönük olarak değerlendirildi. Hastalar; ameliyat esnasında semitendinosus (St) ve gracilis (G) tendonu kullanılan grup (StG grubu) ve sadece semitendinosus tendon kullanılan grup (St grubu) olmak üzere iki gruba ayrıldı. Hastaların diz eklemi fleksiyon, ekstansiyon, iç ve dış rotasyon kuvvet ve dayanıklılıkları izokinetik dinamometre kullanılarak ölçüldü. Kuvvet ve dayanıklılık kayıpları hesaplandı. Hastaların dinamik postüral stabiliteleri Biodex® denge cihazı ile değerlendirildi. Hastalara ayrıca fonksiyonel skorlamalardan IKDC skoru, Lysholm, KOOS-QOL skoru, ACL-RSI skorları yapıldı. Hastaların diz eklemi anteroposterior stabilitesi KT-1000 cihazı kullanılarak ölçüldü. Hastaların sonuçları iki grup arasında karşılaştırıldı. Bulgular: StG grubun greft kalınlığı, St grubundan anlamlı olarak daha yüksek bulundu (p=0,02). Her 2 gruptaki hastaların diz eklemi fleksiyon, ekstansiyon ve iç rotasyon kuvvet kayıpları arasında istatistiksel olarak anlamlı farklılık saptanmamıştır (fleksiyon (p=0,98), ekstansiyon (p=0,41) ve iç rotasyon (p=0,1)). StG grubunun 89N ve 134N kuvvetle yapılan anteroposterior tibial translasyon miktarı St grubundan anlamlı olarak daha fazla bulunmuştur (sırasıyla, p=0,01 ve p<0,001). Hastaların fonksiyonel skorları ve dinamik postüral stabilitelerinin her iki grupta da benzer olduğu tespit edilmiştir. Sonuç: ÖÇBR cerrahisinde St tendonuna ek olarak G tendonunun da alınmasının tibianın anterior translasyonunu anlamlı olarak artırsa bile postop dönemde diz ekleminin diz eklemi fleksiyon ve ekstansiyon kuvveti, iç rotasyon kuvveti ve dinamik postüral stabiliteye olumsuz etkisi saptanmamıştır.

Objective: The aim of our study is to evaluate the effect of harvesting gracilis (G) tendon in addition to the semitendinosus (St) tendon in anterior cruciate ligament recontruction (ACLR) on muscle strength around the knee and the stability of the knee joint. Material and methods: Patients who underwent primary arthroscopic ACLR using the all-inside technique were evaluated retrospectively in our study. Patients were divided into two groups as the group in which semitendinosus (St) and gracilis (G) tendons were harvested (StG group) and the group in which only semitendinosus tendon was harvested (St group). Flexion, extension, internal and external rotation strength and endurance of the knee joint were measured using an isokinetic dynamometer. Strength and endurance deficits were calculated. The dynamic postural stability of the patients was evaluated with the Biodex® balance device. IKDC score, Lysholm, KOOS-QOL score, ACL-RSI scores were also evaluated as functional and clinical results. Anteroposterior stability of knees was measured using the KT-1000 device. The results of the patients were compared between the two groups. Results: The graft thickness of the StG group was found to be significantly higher than that of the St group (p=0.02). No statistically significant difference was found between the knee joint flexion, extension and internal rotation strength losses of the patients in both groups (flexion (p=0,98), extension (p=0,41) and internal rotation (p=0,1)). The amount of anteroposterior tibial translation performed with 89N and 134N force in the StG group was found to be significantly higher than in the St group (p=0.01 and p<0.001, respectively). The functional scores and dynamic postural stability of the patients were found to be similar in both groups. Conclusion: Although the harvesting of the G in addition to the St tendon in ACLR surgery significantly increased the anterior translation of the tibia, it had no negative effects on the knee joint flexion, extension and internal rotation stgrength and dynamic postural stability in the postoperative period.
