Özet
Diyetin seboreik dermatit gelişimi ve şiddeti üzerine etkisi Amaç: Seboreik dermatit (SD) etyopatogenezine ilişkin çok sayıda suçlu faktör araştırılsa da diyetin hastalığın seyrindeki rolünü araştıran çalışma sayısı yetersizdir. Bu çalışmada SD ile diyet arasındaki ilişkinin araştırılması, diyetin SD şiddeti ve SD atak sıklığına olan etkisinin tespit edilmesi amaçlanmıştır. Materyal ve metod: Çalışmaya dermatoloji polikliniğine başvuran 12-65 yaş arası olan 150 SD'li birey ile 150 sağlıklı gönüllü birey dahil edilmiştir. SD'li grup ile kontrol grubu gıda tüketim sıklığı bakımından -belirlenen gıda grupları ve ölçütleri esas alınarak-incelenerek karşılaştırıldı. SD'li hastalardaki hastalık şiddeti, SD atak sıklığı ile gıda tüketim sıklığı arasındaki korelasyon araştırıldı. Bulgular: SD'li grubunda ortalama vücut kitle indeksi 24,88 ± 5,54 kg/m2 iken kontrol grubunda 22,48 ± 1,06 kg/m2 idi (p<0,001). Seboreik dermatit şiddet indeks skoru ortanca değeri 1,2 (minimum 0,2- maksimum 16) iken SD'li gün sayısı ortanca değeri 30 gündü. SD'li hastalarda (%85,3) haftada ≥ 3 yağlı süt tüketimi kontrol grubuna (%32,7) göre istatistiksel olarak anlamlı derecede azdı (p<0,001). SD'li grupta hafta ≥ 3 porsiyon kırmızı et (p=0,001), tavuk eti (0,008), yumurta (p=0,020) tüketimi ve haftada ≥ 1 porsiyon fıstık (p=0,016) ve fındık (p=0,015) tüketimi kontrol grubuna göre istatistiksel olarak anlamlı derecede daha fazlaydı. Öte yandan, kontrol grubunda hafta ≥ 3 porsiyon mercimek çorbası tüketimi ve haftada ≥ 1 porsiyon çekirdek tüketimi istatistiksel olarak anlamlı derecede daha fazlaydı. SD atak sıklığı ile portakal tüketim miktarı arasında istatistiksel olarak anlamlı negatif korelasyon saptanırken (p=0,049, K=-0,161) kırmızı et tüketim miktarı ile SD atak sıklığı arasında istatistiksel olarak anlamlı derecede pozitif korelasyon saptandı (p=0,004, KK=0,23). Sonuç: Bugüne kadar SD ile diyet arasındaki ilişkinin incelendiği çalışma sayısı oldukça azdır. SD'li hastalarda kanıt yetersizliği nedeniyle herhangi bir diyet önerisi günlük pratikte sunulmamaktadır. Bu çalışmada ilk defa çok sayıda gıda tüketim sıklığının SD oluşumu ve şiddeti üzerindeki ilişki incelendi. Kırmızı et, tavuk eti ve yumurta gibi inflamatuar indeksi yüksek gıdaların ve fındık, fıstık gibi doymamış yağ asidinden zengin gıdaların SD'yi tetikleyebileceği görüldü. Öte yandan, yağlı sütün SD oluşumuna karşı koruyucu olabileceği görüldü. Diyet ve SD arasındaki ilişkinin gelecekteki çalışmalarla aydınlatılması ürtiker gibi bazı hastalıklarda verilen diyet önerilerini SD için de gündeme getirebilecektir.

Effect of diet on the development and severity of seborrheic dermatitis Objective: Although many culprit factors related to the etiopathogenesis of seborrheic dermatitis (SD) have been investigated, the number of studies investigating the role of diet in the course of the disease is insufficient. In this study, it was aimed to investigate the relationship between SD and diet and to determine the effect of diet on SD severity and and SD flare-up frequency. Material and methods: The study included 150 individuals with seborrheic dermatitis (SD) aged between 12 and 65 years, who presented to the dermatology clinic, and 150 healthy volunteer individuals. The SD group was compared to the control group in terms of food consumption frequency based on predetermined food groups and criteria. The correlation between disease severity, SD flare-up frequency and food consumption frequency in patients with SD was investigated. Results: The mean body mass index was 24.88 ± 5.54 kg/m2 in the SD group, while it was 22.48 ± 1.06 kg/m2 in the control group (p<0.001). The median value of the seborrheic dermatitis severity index score was 1.2 (minimum 0.2 - maximum 16), and the median value of the number of SD days was 30 days. In SD patients, the consumption of ≥ 3 servings of whole milk per week was significantly lower (85.3%) compared to the control group (32.7%) (p<0.001). In the SD group, the consumption of ≥ 3 servings of red meat (p=0.001), chicken meat (p=0.008), eggs (p=0.020), and ≥ 1 serving of peanuts (p=0.016) and hazelnuts (p=0.015) per week was significantly higher compared to the control group. On the other hand, in the control group, the consumption of ≥ 3 servings of lentil soup per week and ≥ 1 serving of sunflower seeds per week was significantly higher. A statistically significant negative correlation was found between SD flare-up frequency and orange consumption (p=0.049, r=-0.161), while a statistically significant positive correlation was observed between red meat consumption and SD flare-up frequency (p=0.004, r=0.23). Conclusion: There have been limited studies examining the relationship between diet and SD to date. Due to insufficient evidence, no specific dietary recommendations are currently provided in daily practice for SD patients. This study, for the first time, investigated the relationship between the frequency of food consumption and the development and severity of SD. It was observed that inflammatory foods with high inflammatory index such as red meat, chicken meat, and eggs, as well as foods rich in unsaturated fatty acids such as hazelnuts and peanuts, could trigger SD. On the other hand, it was found that whole milk could have a protective effect against the development of SD. Further studies elucidating the relationship between diet and SD in the future may open up discussions for dietary recommendations in SD, similar to those provided for certain conditions like urticaria.
