Özet
Araştırmanın amacı afiliasyon uygulanan bir kurum olan Afiliye Üniversite-Eğitim Araştırma Hastanesinde çalışan hekimlerin ve öğretim üyelerinin afiliasyon algıları ve iş doyumları arasındaki ilişkinin incelenmesidir. Gözlemsel ve tanımlayıcı tipte bir araştırmadır. Veri toplama yöntemi ankettir. Çalışma evrenini Mayıs-Haziran 2016 tarihlerinde Marmara Üniversitesi Pendik Eğitim ve Araştırma Hastanesi bünyesindeki 876 hekim oluşturmuş olup 388'ine anket uygulanmıştır. Verilerin istatistiksel analizinde; sıklık dağılımlarını yanı sıra Ki-Kare, Fisher, Mann Whitney U, Kruskal Wallis, Spearman korelasyonlarından yararlanılmıştır. Afiliasyondan önce Marmara Üniversitesi kadrosunda çalışan hekimler afiliasyonun hastaneye yarar boyutunda faydalı olduğunu, afiliasyondan önce Sağlık Bakanlığı kadrosunda çalışan hekimler ise afiliasyonun hastaneye faydasız olduğunu düşünmektedirler (p=0.008). Hekimlerin çoğunluğu afiliasyonun hekim gelirine olumlu etkisi olduğunu belirtmişlerdir (p=0.775). Anket tarihindeki durumlarına göre Sağlık Bakanlığı kadrosunda çalışan hekimler afiliasyonu akademik olarak faydalı (%40.00) bulurken, Marmara Üniversitesi kadrosunda çalışanlar faydasız bulmuşlardır (%17.50) (p=0.002). Mesleğe ilk defa Marmara Üniversitende başlayan genç hekimler, afiliasyonun eğitim boyutunda diğer hekimlere göre daha yüksek oranda faydasız olduğunu düşünmektedir (%39,4) (p=0.019). Afiliasyonun çeşitli boyutlarda faydalı olduğunu düşünen hekimlerin genellikle iş doyum skorları anlamlı şekilde daha yüksek saptanmıştır (p <0,001). Çalışma sonucunda hekimlerin afiliasyondan memnuniyet düzeylerinin iş doyumlarını etkilediği görülmüştür. Genel anlamda hekimler, afiliasyonu hastane ve ülkenin sağlık sisteminde ne faydalı ne de faydasız olarak görmektedirler. Afiliasyonun eğitim ve işletme alanında olumlu bir uygulama olabilmesi için hekimlerin eğitimini kısıtlamaması ve hizmet kalitesine olumsuz etkilerinin önüne geçilmesi gerekmektedir.

The purpose of this research is to analyze the relation between the job satisfaction and the impression of affiliation among the doctors who work at the affiliated Marmara University Training and Research Hospital. The purpose of this research is to analyze the relation between the job satisfaction and the impression of affiliation among the doctors who work at the affiliated Marmara University Training and Research Hospital. It is an observational and descriptive study. Data collection method is survey. The study population consisted of 876 physicians at the Marmara University Training and Research Hospital and 388 of them were surveyed May and June 2016. In the statistical analysis of data; number, frequency, percentage, Fisher's exact test, Chi-square test, Mann Whitney U. test, Kruskal Wallis test and Spearman correlation were used. While the physcians working at Marmara University before affiliation thought that affiliation benefits the hospital; the physcians who were working at Ministry of Health before affiliation didn't. The majority of physicians stated that affiliation was beneficial to their income. Forty percent of the physicians who were working at the Ministry of Health found affiliation academically beneficial, while those who were employed in Marmara University found it useless (p=0.002). Young physicians -who started their career at Marmara University for the first time- think that affiliation is more useless in education dimension than other physicians (39.4%) (p=0.019). The physicians who thought that affiliation was beneficial in various dimensions were found to have significantly higher job satisfaction scores than the others. As a result of the study, it was seen that physicians' satisfaction levels were effective in their job satisfaction. In general terms, physicians think that affliation is beneficial and useful for the hospital and for the healthcare system of the country. In order for affiliation to be a positive application in the field of education and management; it is necessary not to restrict the education of physicians, and in order to provide more services, efforts should be made not to compromise service quality.
