Özet
Bu çalışmada, v-HIT ve kalorik test kullanarak diyabetik nöropatisi olan Tip 2 diabetes mellitus (DM) hastalarında vestibüler sistem fonksiyonlarını objektif ölçekler kullanılarak değerlendirilmesini amaçladık. GEREÇ VE YÖNTEM Bu çalışmaya 30-55 yaş arasında herhangi bir nöro-otolojik şikâyeti olmayan diyabetik nöropatisi olan 30 Tip 2 DM hasta, diyabetik nöropatisi olmayan 30 Tip 2 DM hasta ve 26 sağlıklı gönüllü ile gerçekleştirildi. Tüm katılımcılara saf ses odyometri testi yapıldıktan sonra v-HIT ve kalorik test yapıldı. BULGULAR Yapılan v-HIT testi sonucuna göre, diyabetik nöropati olan Tip 2 DM grubu ile diyabetik nöropatisi olmayan Tip 2 DM grubu ve kontrol grubu arasında sağ ve sol lateral kanal vestibülo-oküler refleks (VOR) kazançları istatistiksel olarak anlamlı şekilde düşük olarak saptandı (sırasıyla; p=0,004, p=0,01). Kalorik test sonucunda diyabetik nöropatisi olan Tip 2 DM grubu ile diyabetik nöropatisi olmayan Tip 2 DM grubu ve kontrol grubu arasında kanal parezisi (UW) ve yön üstünlüğü (DP) parametreleri açısından istatistiksel olarak anlamlı fark görüldü (sırasıyla; p=0,03, p=0,004). SONUÇ Çalışmamızda diyabetik nöropatisi olan Tip 2 DM hastalarda vestibüler disfonksiyonun erken dönem değerlendirmesinde v-HIT ve kalorik testin kullanılabileceğini düşünmekteyiz.

The aim of this study was to evaluate the vestibular system in patients with Type 2 diabetes mellitus (DM) and diabetic neuropathy by using video head impulse test (v-HIT) and caloric test. MATERIALS AND METHODS Patients between the ages of 30-55 with diabetic neuropathy (n=30), type 2 DM without diabetic neuropaty (n=30) and healthy volunteers (n=26) were included in this study. After the audiological test was performed on all participants, the video head impulse test (v-HIT) and the caloric test were performed. RESULTS In our study, right and left lateral canal vestibülo-ocular reflex (VOR) gains were found to be statistically significantly lower between the Type 2 DM group with diabetic neuropathy, the Type 2 DM group without diabetic neuropathy and the control group, according to v-HIT test results (respectively; p=0.004, p. =0.01). According to the caloric test results, a statistically significant difference was found between the Type 2 DM group with diabetic neuropathy, the Type 2 DM group without diabetic neuropathy and the control group in terms of unilateral weakness (UW) and directional preponderance (DP) parameters (respectively; p=0.03, p=0.004). CONCLUSION In our study, we suggest that v-HIT and caloric test can be used in the early evaluation of vestibular dysfunction in Type 2 diabetes mellitus patients with diabetic neuropathy.
