Özet

Giriş ve amaç: Kötü haber vermek, bir doktorun en zor görevlerinden biridir ancak tıp eğitimi genellikle bu göz korkutucu görev için çok az resmi hazırlık sunmaktadır. Uygun eğitim olmadan, kötü haber vermenin getirdiği rahatsızlık ve belirsizlik, doktorların hastalardan duygusal olarak uzaklaşmasına neden olabilir. Kötü haberlerin verilmesini kolaylaştırmak için iletişim becerileri ve tekniklerine odaklanan eğitimin hasta memnuniyetini ve doktor konforunu artırdığı kanıtlanmıştır. Çalışmamızda Ankara Şehir Hastanesinde çalışan araştırma görevlisi doktorların kötü haber verme ile ilgili bilgi düzeylerini ölçmek amaçlanmıştır. Gereç ve Yöntem: Araştırmanın yapıldığı Haziran-Aralık 2022 tarihleri arasında Ankara Şehir Hastanesinde çalışan 361 araştırma görevlisine ulaşılmıştır. Katılımcılara iki kısımdan oluşan araştırmacı tarafından hazırlanan anket gözlem altında ve Google forms aracılığıyla uygulanmıştır. Anketin birinci kısmında katılımcıların tanımlayıcı bilgilerini sorgulayan 14 soru ve kötü haber vermenin iletişim becerisi olup olmadığını sorgulayan 2 soru vardı. Anketin ikinci kısmında ise kötü haber verme modeli olarak kabul edilen SPIKES modelinin 6 alt başlığına göre araştırmacı tarafından hazırlanan 22 soruluk anket yer almaktaydı. Bu sorular katılımcıların SPİKE protokolüne göre kötü haber verme hakkında bilgi düzeylerini ölçmek amaçlı beşli likert tarzında cevabı olan sorulardan oluşmaktaydı. Verilerin istatistiksel analizinde SPSS 22.0 programı kullanılmıştır. Tanımlayıcı istatistiklerin sunumunda nitel veriler için sayı ve yüzde kullanılmıştır. Nicel veriler için ortalama ve standart sapma, ortanca (minimum; maksimum) kullanılmıştır. Değişkenlerin normal dağılıma uygunluğu histogram ve olasılık grafikleri ve Shapiro-Wilk testi ile incelenmiştir. Bağımlı değişkenlerin analizinde nitel veriler için Pearson Ki-kare testi, nicel veriler için Student T-Testi ve One-Way ANOVA testi kullanılmıştır. İstatistiksel anlamlılık düzeyi p<0,05 olarak kabul edilmiştir.Bulgular: Araştırmaya katılan hekimlerin %58,4'ü kadındı, %58,7'si bekardı, %68,4'ü dahili branştaydı. Katılımcıların %85,9'u kötü haber verme deneyimi olduğunu belirtmişti. En çok verdikleri üç kötü haber konusu sırasıyla ölüm haberi (%73,1), kronik hastalık tanısı (%42,7), bulaşıcı hastalık tanısı (%36,8) olmuştu. Katılımcıların %80,1'i kötü haber vermeyi stres sebebi olarak görmekteydi, %51,0'ı mezuniyet öncesi konu hakkında eğitim almıştı, %14,1'i mezuniyet sonrası konu hakkında eğitim almıştı. Katılımcıların %78,4'ü kendisi veya yakını için kötü haber alma deneyimi yaşamıştı, kötü haber alma deneyimi yaşayan katılımcıların %44,2'sini bu deneyim sonrasında kötü haber verme yönünden etkilemişti. Katılımcıların %45,7'si kendisini kötü haber verme konusunda yeterli görmekteydi, %99,2'si kötü haber vermeyi iletişim becerisi olarak görmekteydi, %98,9'u kötü haber vermenin sonradan öğrenilebilir bir beceri olarak görmekteydi. Genel puan ortalaması 3,8 ± 0,4'dür. Protokolde katılımcıların en yüksek puana ulaştıkları kısım hastanın bilgilendirildiği kısımken en düşük puanı ise hastanın ne kadarını bilmek istediğinin sorgulandığı kısımda almışlardır. Sonuç ve Öneriler: Çalışmamız sonucunda Ankara Şehir Hastanesi'nde çalışan araştırma görevlisi hekimlerin kötü haber verme konusundaki bilgi düzeyi ortalaması 5 üzerinden 3,8 ± 0,4 bulunmuştur. Kötü haber verirken dikkat edilmesi gereken durumlar içinde hastanın bilgilendirilme aşamasına özen gösterilirken, hastanın ne kadarını bilmek istediğinin sorgulandığı kısmının göz ardı edildiği, mevcut bilginin davranışa dönüştürülmesinde ve uygulanmasında eksiklikler olduğu saptanmıştır. Bunun yanında mezuniyet öncesi ve sonrası konu hakkında alınan eğitimler yetersiz bulunmuştur. Çalışmamıza katılan hekimlerin çoğu kötü haber verme konusunda stres yaşıyor ve kendisini yeterli görmüyordu. Kötü haber vermenin öğrenilen bir beceri olduğu ve bir protokole uygun şekilde verildiğinde hem hasta hem de hekim için bu zor durumla daha rahat başa çıkılabileceği saptanmıştır. SPİKES protokolü bu konuda dünyada en sık uygulanan ve kabul gören protokollerden biridir. Hekimlerin mezuniyet öncesi ve sonrasında pratik uygulamaya yönelik eğitimlerinde bu protokolün bir araç olarak kullanılması faydalı olacaktır.

Introduction and Aim: Delivering bad news is one of a doctor's most difficult tasks, but medical training often offers little formal preparation for this daunting task. Without proper training, the discomfort and uncertainty of reporting bad news can cause doctors to become emotionally distant from patients. Training that focuses on communication skills and techniques to facilitate the delivery of bad news has been proven to increase patient satisfaction and physician comfort. It was aimed to measure the knowledge level of the research assistant doctors working in Ankara City Hospital about giving bad news. Materials and Methods: Between June and December 2022, when the research was conducted, 361 research assistants working at Ankara City Hospital were reached. The questionnaire prepared by the researcher, consisting of two parts, was applied to the participants face-to-face and via Google forms. In the first part of the questionnaire, there were 14 questions questioning the descriptive information of the participants and 2 questions questioning whether giving bad news is communication skills. In the second part of the questionnaire, there was a questionnaire consisting of 22 questions prepared by the researcher according to the 6 sub-headings of the SPIKES model, which is accepted as a model of giving bad news (Annex-2). These questions consisted of questions with five-point likert style answers to measure the knowledge level of participants about giving bad news according to the SPİKE protocol. SPSS 22.0 program was used in the statistical analysis of the data. In the presentation of descriptive statistics, numbers and percentages were used for qualitative data. For quantitative data, mean and standard deviation, median (minimum; maximum) were used. The conformity of the variables to the normal distribution was examined by histogram and probability plots and the Shapiro-Wilkinson test. Pearson Chi-square test was used for qualitative data, Student T-Test and One-Way ANOVA test were used for quantitative data in the analysis of dependent variables. Statistical significance level was accepted as p<0.05. Results: 58.4% of the participating physicians were women, 58.7% were single, and 68.4% were in internal departments. 85.9% of the participants stated that they had bad news experience. The three most common bad news topics were death news (73.1%), chronic disease diagnosis (42.7%), and infectious disease diagnosis (36.8%), respectively. 80.1% of the participants consider giving breaking bad news as a cause of stress, 51.0% received training on the subject before graduation, 14.1% received training on the subject after graduation. 78.4% of the participants had the experience of receiving breaking bad news for themselves or their relatives, and 44.2% of the participants who had a bad news experience affected them in terms of giving breaking bad news after this experience. 45.7% of the participants consider themselves sufficient to deliver breaking bad news, 99.2% see giving breaking bad news as a communication skill, 98.9% see giving breaking bad news as a skill that can be learned later. The overall mean score is 3.8 ± 0.4. In the protocol, the participants got the highest score in the section where the patient was informed, while the lowest score was in the section where the patient was questioned how much they wanted to know. Conclusion and Suggestions: As a result of our study, the average knowledge level of the research assistant physicians working at Ankara City Hospital about reporting breaking bad news was found to be 3.8 ± 0.4 out of 5. While attention is paid to the stage of informing the patient, among the situations that should be considered when giving breaking bad news, it has been determined that the part where the patient is questioned about how much they want to know is ignored, and there are deficiencies in transforming the existing information into behavior and applying it. In addition, the training received on the subject before and after graduation was found to be insufficient. Most of the physicians participating in our study were stressed about giving breaking bad news and did not consider themselves sufficient. It has been determined that giving bad news is a learned skill and when given in accordance with a protocol, this difficult situation can be handled more easily for both the patient and the physician. SPİKES protocol is one of the most frequently applied and accepted protocols in this regard. It would be beneficial to use this protocol as a tool in the training of physicians for practical application before and after graduation.
