Özet

Bu araştırma, organ nakli hastalarında termal bakım paketinin istenmeyen perioperatif hipotermi (İPH) gelişimine etkisinin incelenmesi amacıyla yapılan prospektif paralel olmayan kohort çalışmasıdır. Araştırma kapsamına, 1 Eylül 2020-31 Mayıs 2022 tarihleri arasında Ege Üniversitesi Organ Nakli Merkezi'ne yatan, 18 yaş ve üzeri, araştırmaya katılmayı kabul eden 71 organ nakli alıcısı dahil edildi. Araştırma protokolü üç aşamadan oluştu. Birinci aşamada, kanıta dayalı uygulamalar göz önüne alınarak var olan sorunun belirlenmesine yönelik gözlem yapıldı ve uygulamalarda aksayan basamaklar saptandı. İkinci aşamada, aksayan basamaklar doğrultusunda bakım paketine dahil edilecek parametreler belinlendi, cerrahi ekip üyelerine İPH yönetimi ve termal bakım paketi hakkında eğitim verildi. Üçüncü aşamada ise, organ nakli hastalarında termal bakım paketinin İPH görülme sıklığına etkisi incelendi. Elde edilen verilen analizi SPSS (Statistical Package for Social Science) Versiyon 25.0 ve R (R software) paket programı kullanılarak yapıldı. Verilerin değerlendirilmesinde tanımlayıcı istatistikler, Shapiro Wilk normal dağılıma uygunluk, Pearson Ki Kare, Fisher Exact, Mann Whitney U ve bağımsız iki örnek t testi, Brunner-Langer model (F1-LD-F1 dizayn) kullanıldı. İstatistiksel anlamlılık p<0.05 kabul edildi. Araştırmada, cerrahi ekip üyelerinin ön ısıtma parametresinin tüm alt basamaklarına uyumu bakım paketi öncesi dönemde bulunmazken, bakım paketi sonrasında %89.2 bulundu. Bakım paketi sonrası cerrahi ekip üyelerinin ön ısıtma parametresinin tüm alt basamaklarına uyumu, bakım paketi öncesine göre arttı (p<0.01). Cerrahi ekip üyelerinin vücut sıcaklığı ölçüm ve kaydı parametresinin tüm alt basamaklarına uyumu bakım paketi öncesi dönemde bulunmazken, bakım paketi sonrasında %89.2 bulundu. Bakım paketi sonrası cerrahi ekip üyelerinin vücut sıcaklığı ölçüm ve kaydı parametresinin tüm alt basamaklarına uyumu bakım paketi öncesine göre arttı (p<0.05). Cerrahi ekip üyelerinin aktif ısıtma parametresinin tüm alt basamaklarına uyumu bakım paketi öncesi %11.8 iken, bakım paketi sonrasında %89.2 bulundu. Bakım paketi sonrası cerrahi ekip üyelerinin aktif ısıtma parametresinin tüm alt basamaklarına uyumu bakım paketi öncesi döneme göre arttı (p<0.01). Cerrahi ekip üyelerinin termal bakım paketi parametrelerine tam uyumu %78.4 saptandı. Ekip üyelerinin bakım paketi sonrası paket parametrelerine tam uyumu, bakım paketi öncesine göre yüksekti (p<0.01). Bakım paketi öncesinde hastaların %100'ünde İPH görülürken, bakım paketi sonrası bu oran %61.1'di. Bakım paketi sonrası İPH görülme oranı bakım paketi öncesine göre azaldı (p<0.01). Bakım paketi sonrası intraoperatif en düşük vücut sıcaklığı (p<0.05), yoğun bakım ünitesine varış sıcaklığı (p<0.01) ve postoperatif en düşük vücut sıcaklığının (p<0.01) bakım paketi öncesi döneme göre yüksek olduğu bulundu. Bu araştırmanın sonucunda, kanıta dayalı uygulamalar doğrultusunda hazırlanan termal bakım paketinin İPH görülme oranını azalttığı ve vücut sıcaklıklarını yükselttiği belirlendi. Bakım paketine uyumun sürdürülebilir olması için kliniklerde geri bildirim ve denetim mekanizmasının oluşturulması; zaman içerisinde önceliklerinin değişimi ile kılavuzlara uyumun etkilenebileceği düşünülerek hatırlatma sistemlerinin oluşturulması önerilmektedir. Aynı zamanda, İPH gelişimi yönünden yüksek riskli grupta yer alan organ nakli hastalarında daha fazla araştırma yapılmasına gereksinim vardır. This research is a non-parallel cohort study to investigate the effect of inadvertent perioperative hypothermia (IPH) of a thermal care bundle in transplant recipients. 71 organ transplant recipients (liver, kidney) aged 18 and over, who were admitted to the Ege University Organ Transplant Center between September 1, 2020 and May 31, 2022, who agreed to participate in the study, were included in the study. The research protocol consisted of three phases. In the first phase, observations were made to determine the existing problem by considering the evidence-based practices to prevent IPH and the steps that were disrupted in the practices were determined. In the second phase, the parameters to be included in the care bundle were determined, and education was given to perioperative team members regarding IPH management and thermal care bundle. In the third phase, the effect of thermal care bundle on IPH in organ transplant recipients was examined. Data analysis was performed by SPSS (Statistical Package for Social Science) Version 25.0 and R (R software) package program. Descriptive statistics, Shapiro Wilk conformity to the normal distribution, Pearson Chi-Square, Fisher Exact, Mann Whitney U and independent two-sample t-test, Brunner-Langer model (F1-LD-F1 design) were used to evaluate the data. Statistical significance was accepted as p<0.05. In the study, the compliance of the perioperative team members with all the sub-items of the prewarming element was not found in the pre-bundle period, while it was 89.2% in the post-bundle. The compliance of perioperative team members with all sub-items of the prewarming parameter post-bundle increased compared to the pre-bundle (p<0.01). The compliance of the perioperative team members with all the sub-items of the body temperature measurement and recording element was not found in the pre-bundle period, while it was 89.2% in the post-bundle. The compliance of perioperative team members with all sub-items of the body temperature measurement and recording element post-bundle increased compared to the pre-bundle (p<0.05). While the compliance of perioperative team members with all sub-items of the active warming parameter was 11.8% in the pre-bundle period, it was 89.2% in post-bundle. The compliance of perioperative team members with all sub-items of the active warming element in the post-bundle period increased compared to the pre-bundle (p<0.01). The full compliance of the perioperative team members with all the parameters was determined as 78.4%. The full compliance of the perioperative team members with all the parameters in the post-bundle period was higher than in the pre-bundle (p<0.01). While IPH was observed in 100% of the patients in the pre-bundle, this rate was 61.1% in the post-bundle. The incidence of IPH in the post-bundle decreased compared to the pre-bundle(p<0.01). It was found that the intraoperative lowest body temperature (p<0.05), arrival temperature to the intensive unit (p<0.01), and postoperative lowest body temperature (p<0.01) in the post-bundle period were higher than the pre-bundle. As a result of this research, it was determined that the thermal care bundle prepared in line with evidence-based practices decreases the rate of IPH and increased body temperatures. Establishing a feedback and inspection mechanism in clinics in order to ensure compliance with the care bundle; it is recommended to create reminder systems, considering that the change in priorities over time may affect compliance with the guidelines. Furthermore, more research is needed on organ transplant recipients who are in the high-risk group for the development of IPH.
