Özet
AMAÇ: Postoperatif pulmoner komplikasyonlar (PPK) ve postoperatif atelektazi insidansı, abdominal cerrahilerde yüksektir. Sıklığını azaltmak üzere koruyucu ventilasyon stratejileri yaygın şekilde uygulanmaktadır. Bu prospektif, randomize, kontrollü çalışmada, abdominal cerrahilerde, akciğer koruyucu ventilasyon stratejilerine ek olarak, ekstübasyon öncesi artımlı yeniden kazandırma manevrasının (KM;“recruitment”) postoperatif solunum derlenmesi üzerine etkisi incelenmiştir. GEREÇ ve YÖNTEM: Etik kurul onayını takiben, çalışma onamı alınan abdominal cerrahi hastalarından, hariç edilme kriterleri sonrası kalan 136 olgudan; kontrol grubuna (GK) standart intraoperatif akciğer koruyucu ventilasyon (n=68), çalışma grubuna (G-KM) ise standart akciğer koruyucu ventilasyon ile birlikte ekstübasyon öncesi artımlı KM (n=68) uygulandı. Standart koruyucu ventilasyonda, pozitif end-ekspiratuar basınç (PEEP) 6 cm-H2O, düşük tidal hacim (6-8 ml/kg) ve SpO2<%%90 izlenmesi kurtarıcı vital kapasite manevrası uygulandı. G-KM'de, bu standart koruyucu ventilasyona ek olarak, ekstübasyon öncesi tek seferlik, 2'şer cm-H2O PEEP artışlarıyla 20 cm-H2O'ya dek ulaşan bir artımlı KM uygulandı. Manevrayı takiben, PEEP artımı esnasında en yüksek dinamik kompliyans elde edilen“optimal PEEP”, ekstübasyona dek korundu. Tüm hastalar, postoperatif derlenme odasında ilk 30 dakikalık maske oksijen tedavisinin ardından, beş dakika boyunca oda havasında izlendi. Oda havasındaki 5. dakikadaki SpO2 değerleri ≥%97 ve <%97 olmak üzere kaydedildi. Literatürde postoperatif atelektazi oluşumuyla ilişkilendirilen, postoperatif dönemde oda havasında SpO2<%97 izlenen grup,“postoperatif oda havası testi pozitif”olarak değerlendirildi. GK ve G-KM gruplarında, birincil çıktı olarak, postoperatif oda havası testi pozitifliği incelendi. Diğer çıktılar arasında, gruplar arasında postoperatif ilk yedi günde PPK insidansı; artımlı KM ile ilişkili dinamik kompliyans değişimleri ile manevranın hemodinamik etkileri bulunmaktaydı. BULGULAR: GK ve GK-M grupları arasında, postoperatif oda havası testi pozitifliği benzer bulunmuştur (GK: 23/68 [%33,8], GK-M: 16/68 [%23,5]; p>0,05). İki grup arasında postoperatif pulmoner komplikasyonlar (PPK) sıklığı arasında da fark izlenmemiştir (GK: 12/68 [%17,6], G-KM: 7/68 %10,3; p>0,05). Bununla birlikte, artımlı KM uygulamasıyla, operasyon sonu düşüş gösteren akciğer dinamik kompliyansında %50'ye yakın bir düzelme sağlanmıştır. Artımlı KM uygulaması, hemodinamik instabiliteye sebep olmamıştır. Her iki grupta da PPK gelişen hastaların postoperatif oda hava testi pozitif çıkma oranı anlamlı olarak daha yüksek yüksek bulunmuştur (p<0,05). SONUÇ: Standart koruyucu ventilasyona ek olarak uygulanan, ekstübasyon öncesi artımlı KM'nin, postoperatif oda havası testi ve PPK gelişimi üzerine etkisi gösterilememiştir. Ancak operasyon sonu dinamik kompliyanslar üzerine olumlu etkisi izlenen bu KM uygulamasının, PPK tahmini riski daha yüksek olan bir hasta grubunda araştırılması gerektiğini düşünmekteyiz.

OBJECTIVE: Postoperative pulmonary complications (PPC) and postoperative atelectasis have an elevated incidence following abdominal surgeries. Lung protective ventilation strategies are widely devoted to reducing their occurrence. This prospective, randomized, controlled study investigated the effect of incremental recruitment maneuver (RM) prior to extubation in adjunct to lung protective ventilation strategies on postoperative respiratory outcomes in abdominal surgery. MATERIAL and METHODS: Following the ethics committee approval, of the 136 abdominal surgery patients who consented to the study and ruled out the exclusion criteria, the control group (GC) received standard intraoperative lung protective ventilation (n=68), and the study group (G-RM) received standard lung protective ventilation + incremental RM before extubation (n=68). The standard protective ventilation consisted of positive end-expiratory pressure (PEEP) of 6 cm-H2O, low tidal volume (6-8 ml/kg) and rescue vital capacity maneuver when SpO2<90%. In G-RM, patients received a single incremental RM reaching 20 cm-H2O with each 2 cm-H2O PEEP increment before extubation, besides standard protective ventilation. Following the maneuver, the“optimal PEEP”, which achieved the highest dynamic compliance during the PEEP increment, was maintained until extubation. In the postoperative recovery room, all the patients received mask oxygen for the first 30 minutes. They are followed up in the room air for five minutes thereafter. SpO2 values in room air at the 5th minute were recorded as ≥97% and <97%. Patients with SpO2<97% were titled postoperative room air test (+), associated with the formation of postoperative atelectasis per the current reference literature. This study assesses the postoperative room air test results as the primary outcome among the GC and G-RM groups. Further outcomes included the incidence of PPC in the first seven days postoperatively among the groups, changes in the dynamic compliance associated with incremental RM and the hemodynamic effects of RM. RESULTS: Postoperative room air test results were found to be similar between GC and G-RM groups (GC: 23/68 [33.8%], G-RM: 16/68 [23.5%]; p>0.05). There was no difference in the incidence of PPCs between the two groups (GC: 12/68 [17.6%], G-CM: 7/68 10.3%; p>0.05). However, after the incremental RM, an improvement of about 50% was achieved in dynamic pulmonary compliance, which decreased at the end of the surgery. Incremental RM did not cause hemodynamic instability. In both groups, the positive postoperative room air test rate in patients with PPC was significantly higher (p<0.05). CONCLUSION: The effect of incremental RM prior to extubation as an adjunct to standard protective ventilation on postoperative room air test and PPCs has not been demonstrated. However, we assume that this RM application, which positively affects end-operative dynamic compliance, should be investigated in a larger patient group with a higher predicted risk for PPC.
