Prostat kanserli bireylerde tümör baskılayıcı (GSTP1, PTEN ve NKX3.1) genlerin ekspresyonu ile beslenme ve yaşam tarzı belirleyicilerinin ilişkisinin değerlendirilmesi

Özet
Bu çalışma prostat kanseri (PCa) hastalarında beslenme ilişkili epigenetik faktörler ve yaşam biçimi değişikliklerinin tümör baskılayıcı genlerin (GSTP1, PTEN ve NKX3.1) ekspresyonuna etkilerini değerlendirmek amacıyla planlanıp yürütülmüştür. Çalışmaya, 40 yaş üzeri patoloji sonucuna göre 41 prostat kanseri (malign prostat) olan ve 42 prostat kanseri olmayan (benign prostat) toplam 83 erkek birey dahil edilmiştir. Bireylere yüz yüze görüşme yöntemi ile tanımlayıcı bilgiler, fiziksel aktive düzeyleri, antropometrik ölçümleri, beslenme alışkanlıkları ve yaşam tarzı belirleyicilerinin sorgulandığı anket formu uygulanmıştır. Ayrıca bireylerin üç günlük besin tüketim kayıtları ve özelleştirilmiş (210 madde) besin tüketim sıklığı sorgulanmıştır. Benign prostat grubunda GSTP1, PTEN ve NKX3.1 gen ekspresyon düzeyleri malign prostat grubuna göre daha yüksek olup, bunlardan GSTP1 ve NKX3.1 gen ekspresyon düzeyleri ise istatistiksel olarak önemli düzeyde yüksektir (p≤0,001). Her iki grubu oluşturan bireylerde ise PTEN gen ekspresyonu, GSTP1 gen ekspresyonu ile pozitif yönde önemli bir ilişki göstermiştir (p<0,05). Bireylerin bel çevresi, Bel/Boy Oranı oranı ve BKİ sınıflamalarına göre gruplar arasında istatistiksel olarak önemli farklılık görülmüştür (p<0,05). Malign prostat grubun sirkadiyen ritim yaşam tarzı değişkenin optimizasyonu ile PTEN gen ekspresyonu arasında pozitif yönlü önemli ilişki gözlenmiştir (p<0,05). Benign prostat grubun ise stres düzeyi ve sirkadiyen ritim yaşam tarzı değişkenlerinin optimizasyonu ile NKX3.1 gen ekspresyonu arasında pozitif ilişki bulunmuşken, uyku kalitesi ile GSTP1 gen ekspresyonu arasında negatif yönde anlamlı bir ilişki saptanmıştır (p<0,05). Malign (%85,4) ve benign (%83,3) grup bireylerin çoğunluğunun Akdeniz diyet uyumunun düşük olduğu saptanmıştır (p>0.05). Malign prostat grubu bireylerin bitkisel protein, toplam posa ve B12 vitamini alımı benign prostat grubundaki bireylerden daha düşükken, hayvansal protein, rafine tahıllar ve ekmek, yumurta, şeker ve şeker eklenmiş içeceklerinin tüketim miktarının istatistiksel olarak önemli düzeyde yüksek olduğu saptanmıştır (p<0,05). Bununla birlikte diyetle ile makro ve mikro besin ögeleri alımları ile her iki grupta GSTP1ve NKX3.1 gen ekspresyon düzeyleri arasında anlamlı ilişki bulunmuştur (p<0,05). Sonuç olarak, GSTP1, PTEN ve NKX3.1 koruyucu genlerin ekspresyon düzeylerinin değişimi beslenme ve yaşam tarzı belirleyicileri ile ilişkili olduğu saptanmıştır. Diyetin bitkisel protein, posa, B12 vitamini gibi besin ögelerinin ve besin gruplarından özellikle meyve ve sebzelerin tüketiminin PCa kanserine karşı koyucu etki gösterdiği değerlendirilmiştir. Prostat kanserinin önlenmesi ve tedavisi için beslenme ve yaşam tarzı bileşenleri önem arz etmektedir.

This study was planned and conducted out to assess the effects of dietary-related epigenetic mechanisms and lifestyle modifications on the expression of tumor suppressor genes (GSTP1, PTEN, and NKX3.1) in prostate cancer patients. The study included 83 males over the age of 40, 41 of whom had prostate cancer (malign prostate) and 42 of whom did not (benign prostate), per the findings of the pathology. In order to collect data for the study, a questionnaire form was used to conduct in-person interviews with participants and ask them about descriptive information, physical activity levels, anthropometric measurements, dietary habits, and lifestyle determinants. Also included were the subjects' three-day food consumption records and a specially created Food Frequency Questionnaire (210 items). GSTP1, PTEN and NKX3.1 gene expression levels were higher in the benign prostate group than in the malign prostate group, and GSTP1 and NKX3.1 gene expression levels were statistically significantly higher (p≤0.001). PTEN gene expression and GSTP1 gene expression were significantly positively correlated in people belonging to both groups (p<0,05). In terms of anthropometric measurement classifications (waist circumference, waist/height ratio, and BMI) of individuals forming the malign and benign groups, there was a statistically significant difference between the groups (p<0,05). The malign prostate group's ptimization of the circadian rhythm lifestyle variable and PTEN gene expression were found to be positively and significantly correlated (p<0.05). Optimization of stress levels, circadian rhythm lifestyle factors, and NKX3.1 gene expression were all found to be positively correlated in the benign prostate group, whereas sleep quality and GSTP1 gene expression were found to be significantly negatively correlated (p<0,05). The majority of people in both the malign (85.4%) and benign (83.3%) groups were found to have a poor adherence to the Mediterranean diet (p>0,05). While the individuals in the malign prostate group consumed less total fiber, vitamin B12, and vegetable protein than those in the benign prostate group, they consumed statistically significantly more animal protein, refined gains and bread, eggs, sugar, and energy drinks (p<0,05). At the same time, a significant association between dietary intake of macro and micronutrients and GSTP1 and NKX3.1 gene expression levels of volunteers in the malign and benign prostate group was discovered (p<0,05). In conclusion, the levels of expression of the protective genes GSTP1, PTEN, and NKX3.1 were found to be correlated with dietary and lifestyle factors. The consumption of nutrients like vegetable protein, fiber, and vitamin B12 in the diet, especially the consumption of fruits and vegetables from food groups, has been found to have a protective effect against PCa cancer. Prostate cancer may be prevented and treated with the right diet and lifestyle.
