Özet
Spinal deformity is the most common problem in children and adolescents with the share of 48%. (HCUP-AHRQ 2011) Only in United states, estimated number of patient who are suffer from spinal deformity is more than 4 million. Treatment requires from patient to wear the spinal brace for 10 to 23h a day for years which could be from 2 to 6 years. A patient needs to join everyday life and socialize with people while wearing the spinal brace. Since patient needs to spend such a long time in brace, they do not want to be seen wearing 'medical device'. The appearance of the spinal brace is the most important factor for the user. Theoretically look of the brace does not have an effect on function but for the patient, it is a decision maker. Medical devices only work with the cooperation of the patient. Besides that, this long process has a potential to affect the patient not only physically but also psychologically. It has been proven that spinal brace treatment has a negative impact on quality of life. While medical devices aim to improve patient's health it should not cause new problems in the processes. This thesis paper investigates the reasons for failure in spinal brace treatment from the design point of view and try to understand the user experience also relationship between brace and the user. Besides, exploring the possibilities to improve the effectiveness of spinal brace. In this study, people who are taken part in the treatment process (patient, family, doctors) are included to research paper. The focus of the research is directed to understanding patient's design linked reasons to not follow the treatment. The discomfort that patient is experiencing, both physically and emotionally while he/she is wearing the brace creates a compliance problem. Patient compliance issue is the reason of ineffectiveness of the spinal brace. Based on multiple case studies, experimental works and literature surveys, the research is proposing possible ways of increasing patient compliance through design and highlighting possible features/developments to make the spinal brace more efficient. After detailed research, I came to the conclusion that product needs to be in balance between what patient medically needs and what patient willing to wear. This can be achieved by customizing the product aesthetically to improve the appearance and help the user to engage with the brace. However, improving appearance is not enough by itself. Patient requires emotional and social support which can be provided by sharing the experience with other patients. As last there must be constant communication between medical staff about patient's process so they can be more aware of patient's progress.

La deformazione della spina dorsale è la patologia più comune tra bambini e adolescenti, con una diffusione di circa il 48% (HCUP-AHRQ 2011). Solo negli Stati Uniti si stima che il numero di persone affette da questo disturbo sia oltre 4 milioni. La cura prevede che il paziente indossi un tutore dalle 10 alle 23 ora al giorno per un periodo che può variare dai 2 ai 6 anni. I pazienti sono costretti ad affrontare la vita di tutti i giorni ed interagire con altre persone indossando il tutore, e poiché è necessario che lo portino per un tempo molto prolungato, non vogliono mostrarsi in pubblico con addosso un“dispositivo medico”. L'estetica del tutore è uno degli elementi più importanti per l'utente. Benché l'aspetto del tutore non ne influenzi teoricamente la funzione, è comunque un fattore di scelta: i dispositivi medici funzionano anche grazie alla cooperazione del paziente. Oltre a questo, la lunga durata del processo di cura può potenzialmente affliggere il paziente non solo fisicamente ma anche psicologicamente. È stato provato che il trattamento delle patologie legate alla deformazione della colonna vertebrale può avere un impatto negativo sulla qualità della vita del paziente, e lo scopo dei dispositivi medici è quello di migliorare la sua salute, e non causargli ulteriori problemi. Questa tesi indaga sulle ragioni della mancata riuscita del trattamento di correzione della deformazione della spina dorsale dal punto di vista del design, e cerca di cogliere la user experience e la relazione tra utente e tutore, esplorando, inoltre, le possibilità di ottimizzare l'efficienza di quest'ultimo. In questo studio giocheranno un ruolo fondamentale le persone coinvolte nel processo del trattamento (paziente, famiglia, dottori). Il punto nevralgico della ricerca è incentrato sulla comprensione delle ragioni - legate al design - per cui il paziente decide di non proseguire la cura. Il disagio che il paziente prova sia fisicamente sia emotivamente nel momento in cui indossa il tutore crea problemi relativi all'acquiescenza, che sono la causa dell'inefficacia dello stesso. Sulla base di molteplici casi studio, lavori sperimentali e sondaggi, la ricerca propone alcuni modi possibili per incrementare l'acquiescenza del paziente attraverso il design, ed evidenzia alcuni potenziali sviluppi e caratteristiche per rendere il tutore più efficiente. Dopo una ricerca dettagliata, si è giunti alla conclusione che il prodotto necessita di un equilibrio tra quello di cui il paziente ha bisogno da un punto di vista medico e quello che vorrebbe indossare. Ciò può essere ottenuto da una personalizzazione estetica del prodotto per migliorarne l'aspetto e aiutare l'utente ad accettare il tutore. Ad ogni modo, migliorare l'estetica non è abbastanza. Al paziente serve un supporto emotivo e sociale che può essere dato dalla condivisione dell'esperienza con altri pazienti, ed è infine opportuno un implemento dello scambio di informazioni all'interno dello staff medico, in modo da avere una maggiore consapevolezza dei progressi del paziente
