Özet
Amaç: Travmanın neden olduğu hemorajiye bağlı ölümler günümüzde hala en sık ölüm sebeplerinden birisidir ve aynı zamanda önlenebilir ölüm nedenleri arasında ön sıralarda yer almaktadır. Travma hastalarının kanamaya bağlı mortalitesini öngörmek için Bleeding Audit and Triage Trauma Score (BATT Skoru) tanımlanmıştır. Çalışmamızda BATT skorunun travma hastalarının hastane içi mortalitesini predikte etmedeki tanısal değerliliği incelenmiştir. İkincil olarak travma hastalarında acil operasyona gidiş, mekanik ventilatör ihtiyacı, yoğun bakım ihtiyacı, kan transfüzyon ihtiyacı gibi alt gruplar incelenerek, BATT skorunun bu sonlanımları predikte etmedeki tanısal değerliliği incelenmiştir. Gereç ve Yöntem: Bu çalışma prospektif, gözlemsel tanısal değerlilik çalışması olarak tasarlanmış olup tek merkezde yürütülmüştür. Hastanemiz erişkin acil servisine 01.04.2021-01.04.2022 tarihleri arasında başvuran travma hastaları, dahil etme ve dışlama kriterlerine göre çalışmamızda yer aldı. Akut travması olan 18 yaş üzerindeki hastalardan Injury Severity Score (ISS) 9 veya üzerinde olanlar çalışmaya dahil edildi. Acil servise gelmeden önce kardiyak arrest gelişenler, gebeler, izole kafa travması, izole ekstremite travması olan hastalar dışlandı. Majör travması olan ve dahil edilme kriterlerini taşıyan hastaların başvuru anındaki nabız, sistolik kan basıncı, solunum sayısı veya oksijen satürasyonu, Glasgow Koma Skalası Skoru, yaralının yaşı, penetran yaralanma varlığı ve yüksek enerjili travma varlığına göre BATT skoru hesaplandı. Elde edilen veriler uygun istatistiksel yöntemler ile karşılaştırıldı. Bulgular: Hastanemize getirilen 349 majör travma hastasından dışlanma kriterleri uygulanınca kalan 237 hasta çalışmamıza dahil edildi. Hastaların 36'sında (%15.2) birincil sonlanım olan hastane içi mortalite gerçekleşti. Hastane içi mortalitesi olan hastalarda BATT skoru ortancası 7 (5-11), hastane içi mortalitesi olmayan hastalarda BATT skoru ortancası 2 (1-4) olarak hesaplandı ve gruplar arasındaki fark istatistiksel olarak anlamlı bulundu (p<0.001). BATT skoru Receiver Operating Characteristic (ROC) eğrisi ile analiz edildiğinde; eğri altında kalan alan 0.842 (%95 GA=0.771-0.913) olarak hesaplandı. BATT skorunun duyarlılık ve özgüllüğünün toplamının en yüksek olduğu 3.5 değeri eşik değer olarak kabul edildiğinde, BATT skorunun duyarlılığı %83.3 (%95 GA=67.2-93.6), özgüllüğü %74.6 (%95 GA=68-80.5), doğruluğu %75,9 (%95 GA=70-81.5) olarak hesaplandı. BATT skorunun tanımlayıcı çalışmasında belirlenen değeri (2 ve üzeri skor alanlar yüksek riskli, <2 skor alanlar düşük riskli) eşik değer kabul ettiğimizde BATT skorunun duyarlılığı %97.22 (%95 GA=85.47-99.93), özgüllüğü %28.1 (%95 GA=22.58-35.48), doğruluğu %39.08 (%95 GA=32.84-45.59) olarak hesaplandı. İkincil sonlanım ölçütleri açısından BATT skoru değerlendirildiğinde, yoğun bakım ihtiyacı gelişen hastaların BATT skoru ortancası 5 (2-7), ihtiyaç olmayan hastaların BATT skoru ortancası 2 (0-3), mekanik ventilatör ihtiyacı gelişen hastaların BATT skoru ortancası 7 (5-10), gelişmeyen hastaların BATT skoru ortancası 2 (1-3), acil ameliyat ihtiyacı olan hastaların BATT skoru ortancası 5 (3-8), olmayan hastaların BATT skoru ortancası 2 (1-4), eritrosit transfüzyon ihtiyacı gelişen hastaların BATT skoru ortancası 8 (5-10), gelişmeyen hastaların BATT skoru ortancası 2 (1-4) hesaplandı ve her dört sonlanımda da gruplar arası fark anlamlı bulundu (sırayla p<0.001, p<0.001, p<0.001, p<0.001). İkincil sonlanım açısından ROC eğrisi analizinde, yoğun bakım ihtiyacı açısından AUC 0.730 (%95 GA=0.665-0.795), mekanik ventilatör ihtiyacı açısından AUC 0.874 (%95 GA=0.819-0.929), acil ameliyat ihtiyacı açısından AUC 0.762 (%95 GA=0.696-0.827), eritrosit transfüzyonu ihtiyacı açısından 0.864 (%95 GA= 0.807-0.921) olarak hesaplanmıştır. Çalışmamızda ayrıca hastane içi mortaliteyi ön gören değişkenlerin tespit edilmesi için multivaryant lojistik regresyon analizi yapıldı. BATT skoru ve ISS skorunun bağımsız prediktör olduğu gözlendi (sırayla p=0.002, p=0.003). BATT skorunun (OR 0.793; %95 GA: 0.684 – 0.919) ve ISS skorunun (OR 0.908; %95 GA: 0.851 – 0.968) olduğu gözlendi. Sonuç: Travma hastalarında mortaliteyi predikte etme amacıyla geliştirilmiş olan BATT skorunun tanısal test performansını araştırdığımız çalışmamızın sonuçları orijinal çalışmadaki sonuçlar kadar iyi olmasa da BATT skorunun tanısal test performansının iyi düzeyde olduğu söylenebilir. BATT skorunun performansını değerlendiren çok merkezli, daha büyük örneklem büyüklüğüne sahip prospektif çalışmalarla validasyona ihtiyacı olduğunu düşünmekteyiz.

Introduction and Aim of The Study: Deaths due to hemorrhage caused by trauma are still the most common causes of death today and it is also one of the leading causes of preventable death. Bleeding Audit and Triage Trauma Score (BATT Score) were defined to predict bleeding related deaths in trauma patients. In our study, the diagnostic value of BATT score in predicting in-hospital mortality of trauma patients was investigated. For the secondary outcome, subgroups such as going to emergency operation, need for mechanical ventilator, need for intensive care, need for blood transfusion in trauma patients trauma patients were examined and the diagnostic value of BATT score in predicting these outcomes was examined. Materials and Methods: Our study was designed as a single-center, prospective, observational study. Trauma patients admitted to the adult emergency department of our hospital between 01.04.2021 and 01.04.2022 were included in our study according to the inclusion and exclusion criteria. Patients over the age of 18 with acute trauma who had an Injury Severity Score (ISS) of 9 or higher were included in the study. Patients who developed cardiac arrest before coming to the emergency department, pregnant women, isolated head trauma, and isolated extremity trauma were excluded. BATT score calculated with at the time of admission heart rate, systolic blood pressure, the age of the injured, the presence of penetrating injury and the presence of high-energy trauma for patients with major trauma who met the inclusion criteria.. The obtained data were compared with appropriate statistical methods. Results: When the exclusion criteria were applied from 349 major trauma patients brought to our hospital, the remaining 237 patients were included in our study. The primary outcome, in-hospital mortality, occurred in 36 (15.2%) of the patients. The median BATT score was calculated as 7 (5-11) in patients with in-hospital mortality and 2 (1-4) in patients without in-hospital mortality, and the difference between the groups was statistically significant (p<0.001). When the BATT score is analyzed for the primary outcome with the Receiver Operating Characteristic (ROC) curve; area under the curve was calculated as 0.842 (95% CI=0.771-0.913). When the value of 3.5, The sum of the sensitivity and specificity of the BATT score is the highest, is accepted as the cut-off value, the sensitivity of the BATT score is 83.3% (95% CI=67.2-93.6), the specificity is 74.6% (95% CI=68-80.5), the accuracy is 75.9% (95% CI=70-81.25). When we accept the value determined in the descriptive study of the BATT score as the threshold value (high risk for those with a score of 2 and above, low risk for those with a score of <2) the sensitivity of the BATT score is 97.22% (95% CI=85.47-99.93), the specificity is 28.1% (95% CI=22.58-35.48), accuracy was calculated as 39.08% (95% CI=32.84-45.59). When the BATT score is evaluated in terms of secondary outcome measures, the median BATT score of the patients who need intensive care is 5 (2-7), the median of the BATT score of the patients who do not need it is 2 (0-3), the median of the BATT score of the patients who need mechanical ventilation is 7 (5-10), the median of the BATT score of the patients who do not develop is 2 ( 1-3), the median BATT score of the patients who need urgent surgery is 5 (3-8), the median of the BATT score of the patients who do not have it is 2 (1-4), the median of the BATT score of the patients who need blood transfusion is 8 (5-10), the BATT of the patients who do not the median score of 2 (1-4) was calculated and the difference between the groups was significant in all four outcomes (respectively, p<0.001, p<0.001, p<0.001, p<0.001). In the analysis of the ROC curve for secondary outcome, AUC 0.730 (95% CI=0.665-0.795) for need for intensive care, AUC 0.874 (95% CI=0.819-0.929) for need for mechanical ventilator, and AUC 0.762 (95% CI= 0.696-0.827) for need for emergency surgery, AUC 0.864 (95% CI= 0.807-0.921) for the need for blood transfusion. In our study, multivariate logistic regression analysis was also performed to determine the variables that predicted in-hospital mortality. BATT score and ISS score were observed to be independent predictors (respectively, p=0.002, p=0.003). BATT score (OR, 0.793; 95% CI: 0.684 - 0.919) and ISS score (OR, 0.908; 95% CI: 0.851 - 0.968) were observed. Conclusion: The results of our study, in which we investigated the diagnostic test performance of the BATT score, which was developed to predict mortality in trauma patients, were not as good as the results of the original study although it can be said that the diagnostic test performance of the BATT score is at a good level. We think that it needs validation with multicenter, larger sample size prospective studies evaluating the performance of the BATT score.
