Özet

Bu çalışma, Huzursuz bacak sendromu (HBS) olan gebelere yapılan sıcak ve soğuk su uygulamasının gebenin yakınmalarına ve uyku kalitesine etkisini belirlemek amacıyla randomize kontrollü deneysel bir çalışma olarak yürütülmüştür. Çalışma Eylül 2020-Mayıs 2021 tarihleri arasinda bir üniversite hastanesinin Kadın-Doğum Polikinliği'ne başvuran ve araştırmaya dahil edilme kriterlerine uyan toplam 80 gebe (sıcak su grubu=26, soğuk su grubu=27, kontrol grubu=27) ile yapılmıştır. Müdahale gruplarındaki gebeler bulundukları gruba göre yedi gün boyunca uykudan bir saat önce bacaklarına 20 dk sıcak (37-40 oC) veya soğuk (15-18 oC) su uygulaması yaparken kontrol grubuna rutin takip ve bakım dışında bir uygulama yapılmamıştır. Müdahale gruplarında bulunan gebeler yedi günlük uygulamayı tamamladıktan sonraki üç hafta botunca ihtiyaç duymadıkları sürece tekrar uygulama yapmamıştır. Veriler Kişisel Bilgi Formu, HBS Tanı Kriterleri Anket Formu, HBS Şiddeti Derecelendirme Skalası, Pittsburgh Uyku Kalitesi İndeksi, Uygulama Sonrası Takip Çizelgesi ve Uygulama Memnuniyet Formu ile ilk görüşme ile sonraki 8. ve 29. günlerde toplanmıştır. Veri analizleri SPSS Statistics 24.0 programında, Shapiro Wilk testi ve Q-Q grafikleri, Levene testi Bağımlı Örneklem t testi, Bağımsız Örneklem t testi, Ki-kare testi, One Way Annova testi, Repeated Measures Annova testi ve Bonferroni testi ile yapılmıştır. İstatistiksel anlamlılık düzeyi için p<0.05 değeri kabul edilmiştir. Araştırma öncesi Erciyes Üniversitesi Klinik Araştırmalar Etik Kurulu'ndan onay ve kadınlardan yazılı olurları alınmıştır. Müdahale ve kontrol grupları sosyodemografik ve obstetrik özellikleri bakımından birbirine benzerdir (p>0.05). Çalışmada grupların HBS şiddet derecelendirme skalası ve Pittsburgh Uyku Kalitesi İndeksi ön test puanları benzerken (p>0.05), son test puanları müdahale gruplarında kontrol grubundan daha düşüktür (p<0.001). Sıcak ve soğuk su uygulamalarının etkisi ortalama ikinci günde başlarken, her iki gruptaki gebelerin uygulama memnuniyetleri ortalama 8 puandır (p<0.05). Yapılan takipte müdahale gruplarının HBS şiddet puanları benzerken (p>0.05) soğuk su grubunun uyku kalitesi sıcak su grubuna göre daha iyiydi (p<0.05) ve her iki grupta da gebelerin çoğu tekrar uygulama yapmaya gereksinim duymadı (p<0.05). Sonuç olarak, yapılan sıcak ve soğuk su uygulamaları gebelelerin HBS şiddetini azaltarak, uyku kalitesini artırmakta ve yapılan müdahalelerin etkisi uygulama bırakıldıktan sonra da devam etmektedir.

This study was carried out as a randomized controlled experimental study to determine the effect of hot and cold water application on pregnant women with restless legs syndrome (RLS) on their complaints and sleep quality. The study was conducted with a total of 80 pregnant women (hot water group=26, cold water group=27, control group=27) who applied to the Obstetrics and Gynecology Polyclinic of a university hospital between September 2020 and May 2021 and met the inclusion criteria of the study. In the study, according to the group, the pregnant women in the intervention groups applied hot (37-40 oC) or cold (15-18 oC) water to their legs for 20 minutes one hour before sleep for seven days, while the control group wasn't applied anything than routine follow-up and care. The pregnant women in the intervention groups did not re-apply for three weeks after completing the seven-day application, unless they needed it. Data were collected on the 8th and 29th days after the first interview with the Personal Information Form, RLS Diagnostic Criteria Questionnaire Form, RLS Severity Rating Scale, Pittsburgh Sleep Quality Index, Post-Application Follow-up Chart, and Implementation Satisfaction Form. Data analyzes were performed using SPSS Statistics 24.0 program, Shapiro Wilk test and Q-Q charts, Levene test Dependent Sample t test, Independent Sample t test, Chi-square test, One Way Annova test, Repeated Measures Annova test and Bonferroni test. A p<0.05 value was accepted for the statistical significance level. Before the study, approval from the Erciyes University Clinical Research Ethics Committee and written consent from the women were obtained. Intervention and control groups were similar to each other in terms of sociodemographic and obstetric characteristics (p>0.05). In the study, while the RLS severity rating scale and Pittsburgh Sleep Quality Index pretest scores of the groups were similar (p>0.05), the posttest scores were lower in the intervention groups than in the control group (p<0.001). While the effect of hot and cold water applications started on the second day on average, the application satisfaction of the pregnant women in both groups was 8 points on average (p<0.05). During the follow-up, the RLS severity scores of the intervention groups were similar (p>0.05), while the sleep quality of the cold water group was better than the hot water group (p<0.05), and most of the pregnant women in both groups did not need reapplication (p<0.05). As a result, hot and cold water applications increase the sleep quality of pregnant women by reducing the severity of RLS, and the effect of the interventions continues after the application is stopped.
