Özet
Amaç: Zehirlenme, pediatrik yaş grubunda önemli bir halk sağlığı sorunudur. Çocuk veya adolesanların zehirlenme açısından değerlendirilmesindeki risk faktörleri arasında yaş ve cinsiyet ön plandadır. Coronavirüs hastalığı 2019 tüm dünyada olduğu gibi ülkemizde de yaşamı derinden etkilemiş olup, eğitim-öğretim süreci de bu pandemiden temel olarak etkilenen basamaklardan birisidir. Önlemler kapsamında, sınıfta maske takılması, sınıf içi öğrenme yerine hibrit veya online öğrenme gibi diğer yöntemlerin kullanılması da çocukların mental sağlıklarını etkileyen faktörler arasındadır. Çalışmamızda hastanemiz çocuk acil servisinde intoksikasyon tanısı alan olguların hem pandemi öncesi hem de pandemi dönemindeki demografik özellikleri ile, pandemi döneminde uygulanan kısıtlamaların intoksikasyon olgularına olan etkisinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışma, retrospektif olarak kurgulanmış kesitsel bir çalışmadır. 11 Mart 2018 ile 11 Mart 2022 tarihleri arasında intoksikasyon tanılı olgular, demografik ve klinik özelliklerine göre değerlendirilmiştir. Bununla birlikte, pandemi sırasında, olgular, ülkemizdeki sokağa çıkma kısıtlamaları ve eğitim-öğretim kısıtlamalarına göre de üç gruba (Okullar kapalı ve sokağa çıkma yasağı var; okullar kapalı ve sokağa çıkma yasağı yok ve okullar açık ve sokağa çıkma yasağı yok) ayrılmıştır. Çalışmamızda istatistiksel analiz için SPSS 22.0 programı kullanılmıştır. İstatistiksel anlamlılık düzeyi p<0,05 olarak kabul edilmiştir. Bulgular: Çalışma grubunun %49'u (n:266) erkektir. Okul öncesi dönemde intoksikasyon nedeni ile başvuran 384 (%83,3) olgu varken, adolesan olgular tüm çalışma grubunun %11,7'sini oluşturmaktadır (n:54). Etkenler incelendiğinde, en sık saptanan etkenin korozif madde'dir. (n:52, %11,3); Olguların %90'ında (n:415) kaza nedenli intoksikasyon saptanırken, %10,0'unda (n:46) intihar girişimi nedeni intoksikasyon kaydedilmiştir. Kısıtlama dönemlerine çocuk acil servisine başvurular arasında intoksikasyon tanı sıklığı, gruplar arasında istatistiksel olarak anlamlı derecede farklıdır. (p<0,001). Pandemi öncesi ve pandemi dönemindeki olguların başvuru dönemine göre etken yüzdeleri arasında istatistiksel olarak anlamlı farklılık bulunmaktadır. (p<0,001). Tartışma ve Sonuç: Çalışmamızda, pediatrik intoksikasyon ile ilgili bulgularımızın literatürle uyumlu olduğu, pandemi döneminde, dezenfektan nedenli intoksikasyon olgularının sıklığında artış olduğu ve okulların açık ve sokağa çıkma yasağının olmadığı dönemde, intoksikasyon tanılı olguların tüm acil başvurularına oranının diğer kısıtlama gruplarına göre daha düşük olduğu saptanmıştır

Objective: Intoxication is an important public health problem in the pediatric age group. Age and gender are among the risk factors in the evaluation of children or adolescents in terms of poisoning. The coronavirus disease 2019 has deeply affected life in our country as well as all over the world, and the education-teaching process is one of the steps that is mainly affected by this pandemic. As part of the precautions, wearing a mask in the classroom and using other methods such as hybrid or online learning instead of in-class learning are among the factors that affect children's mental health. In our study, it was aimed to evaluate the demographic characteristics of the cases diagnosed with intoxication in the pediatric emergency department of our hospital both before and during the pandemic, and to evaluate the effects of restrictions applied during the pandemic period on intoxication cases. Material and Methods: The study is a cross-sectional study designed retrospectively. Between 11 March 2018 and 11 March 2022, cases with a diagnosis of intoxication were evaluated according to their demographic and clinical characteristics. However, during the pandemic, the cases were divided into three groups (Schools are closed and there is a lockdown; schools are closed and there is no lockdown, and schools are open and there is no lockdown) according to the lockdown and education-training restrictions in our country. SPSS 22.0 program was used for statistical analysis in our study. Statistical significance level was accepted as p<0.05. Results: 49% (n:266) of the study group were male. While there were 384 (83.3%) cases presenting due to intoxication in the preschool period, adolescent cases constitute 11.7% of the entire study group (n:54). When the reasons of intoxications were examined, the most common poisoning agent was corrosive substance. (n:52, 11.3%); While accidental intoxication was detected in 90% (n:415) of the cases, intoxication due to suicide attempt was recorded in 10.0% (n:46). The frequency of diagnosis of intoxication among admissions to the pediatric emergency department during the restriction periods was statistically significant between the groups. (p<0.001). There was a significant difference between the poisoning agent percentages of the cases before the pandemic and during the pandemic period based on the admission group. (p<0.001). Conclusion: In our study, it was determined that our findings regarding pediatric intoxication were consistent with the literature, the frequency of disinfectant-induced intoxication cases increased during the pandemic period, and the ratio of cases with a diagnosis of intoxication to all emergency admissions was lower in the period when schools were open and there was no lockdown than the other restriction groups.
