Özet
Amaç: Çin'de Aralık 2019'ta SARS-CoV-2 virusunun ortaya çıkışı ve tüm dünyaya yayılımıyla dünyada olduğu gibi ülkemizde de COVID-19 pandemisi yaşanmıştır. Pandemi başlangıcında küresel düzeyde olduğu gibi ülkemizde de yoğun ve uygunsuz bir antibiyotik tüketimi olmuştur. COVID-19 pandemisinde erken dönemde artan antibiyotik tüketiminin uzun vadede durumu belirsizdir. Bu çalışmada Gülhane Eğitim ve Araştırma Hastanesi'nde yatan erişkin hastalarda pandeminin ikinci yılında antimikrobiyal ilaç kullanım durumunun belirlenmesi, pandemi öncesi durum ile kıyaslanarak COVID-19 pandemisinin antibiyotik kullanımına etkisinin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Çalışma retrospektif nokta prevalans çalışması olup çalışmada 11.02.2020 ve 03.01.2022 tarihlerinde Gülhane Eğitim ve Araştırma Hastanesinde yatmakta olan 18 yaş ve üzeri antibiyotik alan hastalar değerlendirildi. Hastaların yaş, cinsiyet, klinikleri, komorbid durumları kaydedildi. Avrupa Hastalık Önleme ve Kontrol Merkezi (ECDC)'nin belirlediği komorbid hastalıklara göre Mc Cabe skorları kaydedildi. Kullanılan antimikrobiyal ilaç adı, veriliş yolu, kullanım nedeni, kullanım şekli, varsa kültür sonucu ve tedaviye duyarlılık durumu, enfeksiyon kaynağı, Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji konsültasyon durumu, konsültasyon önerilerine uyum, tedavi uygunsuzluk durumları ve nedenleri dosya veya otomasyon sistemi üzerinden kaydedildi. Antibiyotikler Dünya Sağlık Örgütü (DSÖ) Temel İlaçların Seçimi ve Kullanımı Uzman Komitesi tarafından geliştirilen AWaRe(Access/erişim, Watch/izleme, Reserve/rezerv) sınıflamasının 2021 güncellemesine göre kategorize edilerek klinikler arası değişim değerlendirildi. Elde edilen veriler Statistical Packages for the Social Science (SPSS; IBM; Chicago, IL) 23 istatistik programıyla analiz edilerek pandemi öncesi ve pandeminin ikinci yılındaki antibiyotik kullanımı ve uygunsuzluk durumları değerlendirildi. Bulgular: Gülhane Eğitim ve Araştırma Hastanesi'nde pandemi öncesi dönemde yatan erişkin hastaların 323(%36,4)'ü, pandemi döneminde ise 361(%50,1)'i en az bir antimikrobiyal ilaç kullanmıştı (p<0,001). COVID-19 kliniklerinde yatan 102 hastanın ise 47(%46)'si en az bir antimikrobiyal ilaç kullanmaktaydı. Pandemi öncesi 503 antimikrobiyalin 343(%68,2)'ü parenteral kullanılırken pandemi döneminde 557 antibiyotiğin 449(%80,6)'u parenteral kullanılmıştı (p<0,001). Antimikrobiyaller her iki dönemde de en sık ampirik olarak solunum sistemi enfeksiyonlarına yönelik kullanılmıştı ve cilt/yumuşak doku enfeksiyonlarında artış mevcuttu (p:0,008). Pandemi öncesi ve pandemi döneminde en sık kullanılan ilk iki antibiyotik sırasıyla piperasilin tazobaktam ve imipenem/meropenem idi. Üçüncü sıklıkla pandemi öncesi seftriakson kullanılırken pandemi döneminde florokinolonlar kullanılmıştı. AWaRe sınıflamasına göre değerlendirildiğinde pandemi döneminde“Access”grubunda azalma görülürken“Watch”ve“Reserve”grubunun kullanımında artış görüldü. Pandemi öncesi 209(%64,7) hastanın Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji (EHKM) konsültasyonu mevcut olup konsültasyon önerilerine uyum %100'dü. Pandemi döneminde ise 247(%68,4) hastanın konsültasyonu mevcut olup önerilere uyum 227(%91,9) ile azalmıştı (p<0,001). Pandemi öncesi dönemde yatan hastaların aldığı tedavilerin 64(%19,8)'i uygunsuz iken pandemi döneminde bu oran 100(%27,7) ile artmıştı (p:0,016). En sık uygunsuzluk nedeni her iki dönemde de uzamış cerrahi profilaksilerdi (p:0,2). Uygunsuz antibiyotik kullanımının en sık görüldüğü klinikler pandemi öncesi dönemde genel cerrahi ve ortopedi iken pandemi döneminde genel cerrahi ve kadın hastalıkları/doğum kliniği idi. Sonuç: COVID-19 pandemisinin erken dönemlerinde yoğun ve uygunsuz antibiyotik kullanımı olmuştur. Pandeminin ilerleyen dönemlerinde bu oran azalsa da halen yüksekliğini korumaktadır. Pandemiyle birlikte uygunsuz antibiyotik kullanımındaki artışın uzun vadede etkileri belirsiz olmakla birlikte antimikrobiyal dirence olumsuz katkısı kaçınılmazdır. Antibiyotik kullanım sürecinin doğru bir şekilde anlaşılması, antibiyotik uygulamalarını iyileştirmeye yönelik müdahalelere rehberlik edebilir.

Aim: With the emergence of the SARS-CoV-2 virus in China in December 2019 and its spread to the whole world, COVID-19 pandemic has occurred in our country as well as in the World. At the beginning of the pandemic, there was an intense and inappropriate consumption of antibiotics in our country as well as most other countries. The long-term status of increased antibiotic consumption in the early phase of the COVID-19 pandemic is uncertain. In this study, it was aimed to determine the antimicrobial drug using in the second year of the pandemic in adult patients hospitalized in Gülhane Training and Research Hospital, and to evaluate the effect of the COVID-19 pandemic on antibiotic use by comparing it with the pre- pandemic situation. Materials and Methods: The study is a retrospective point prevalence study, and patients aged 18 years and over who were hospitalized in Gülhane Training and Research Hospital on 11.02.2020 and 03.01.2022 were evaluated. Age, gender, clinics and comorbid conditions of the patients were recorded. Mc Cabe scores were recorded according to the comorbid diseases determined by the European Center for Disease Prevention and Control (ECDC). The name of the antimicrobial drug used, route of administration, reason for use, administration route, culture result, source of infection, Infectious Diseases and Clinical Microbiology consultation status, compliance with consultation recommendations, inappropriate treatment and reasons were recorded through the file or automation system. Inter-clinical variation was evaluated by categorizing antibiotics according to the 2021 update of the AWaRe(Access, Watch, Reserve) classification developed by the WHO Expert Committee on the Selection and Use of Essential Medicines. The data were analyzed using 23 statistical programs from Statistical Packages for the Social Science (SPSS; IBM; Chicago, IL) and antibiotic use and inappropriateness were evaluated before the pandemic and in the second year of the pandemic. Results: In Gülhane Training and Research Hospital, 323 (36.4%) of the adult patients hospitalized in the pre-pandemic period and 361 (50.1%) during the pandemic period used at least one antimicrobial drug (p<0.001). Of the 102 patients hospitalized in COVID-19 clinics, 47 (46%) were using at least one antimicrobial drug. While 343 (68.2%) of 503 antimicrobials were used parenterally before the pandemic, 449 (80.6%) of 557 antibiotics were used parenterally during the pandemic period (p<0.001). Antimicrobials were most commonly used for respiratory system infections in both periods, and there was an increase in skin/soft tissue infections (p:0.008). The first two antibiotics most frequently used before and during the pandemic were piperacillin tazobactam and imipenem/meropenem, respectively. While ceftriaxone was used third frequently before the pandemic, fluoroquinolones were used during the pandemic period. When evaluated according to the AWARe classification, there was a decrease in the“Access”group during the pandemic period, while the use of the“Watch”and“Reserve”groups increased. Before the pandemic, 209 (64.7%) patients had Infectious Diseases and Clinical Microbiology (EHCM) consultation, and compliance with the consultation recommendations was 100%. During the pandemic period, 247 (68.4%) patients had consultations, and compliance with the recommendations decreased by 227 (91.9%) (p<0.001). While 64 (19.8%) of the treatments received by inpatients in the pre- pandemic period were inappropriate, this rate increased to 100 (27.7%) during the pandemic period (p:0.016). The most common reason for non-compliance was prolonged surgical prophylaxis in both periods (p:0.2). While the clinics where inappropriate antibiotic use was most common were general surgery and orthopedics in the pre-pandemic period, general surgery and gynecology/obstetrics clinics during the pandemic period. Conclusion: There has been extensive and inappropriate use of antibiotics in the early stages of the COVID-19 pandemic. Although this rate decreases in the later stages of the pandemic, it still maintains its high level. Although the long-term effects of the increase in inappropriate antibiotic use with the pandemic are uncertain, its negative contribution to antimicrobial resistance is inevitable. A proper understanding of the antibiotic use process can guide interventions to improve antibiotic practice.
