Özet
Bu araştırma COVID-19 pandemisi döneminde gebelerin prenatal distres düzeylerini ve bunu etkileyen faktörleri belirlemek amacıyla yapılmıştır. Araştırma, analitik-kesitsel olarak 15 Şubat 2021-15 Temmuz 2021 tarihleri arasında, Dokuz Eylül Üniversitesi Hastanesi Kadın Hastalıkları ve Doğum Anabilim Dalı gebe polikliniğine başvuran 549 gebe ile yürütülmüştür. Veriler, 'Gebe Bilgi Formu' ve 'Prenatal Distres Ölçeği' kullanılarak toplanmıştır. Verilerin değerlendirilmesinde tanımlayıcı istatistikler (yüzde, frekans, ortalama ve standart sapma), Student-t testi, tek yönlü varyans analizi (ANOVA), Mann-Whitney U testi, Kruskall-Wallis tek yönlü varyans analizleri ve post-Hoc Bonferroni testleri kullanılmıştır. Araştırmaya katılan gebelerin %11,5'i gebelik öncesinde ve %5,5'i gebelik sırasında COVID-19 geçirdiğini ifade etmiştir. Gebelerin %5,1'i COVID-19 hakkındaki bilgi düzeyini, %23,3'ü ise COVID-19'un gebelik üzerindeki etkisi hakkındaki bilgi düzeyini yetersiz bulduğunu belirtmiştir. Çalışmadaki gebelerin %37'si COVID-19'un psikolojik sağlıklarını etkilediğini, %33'ü ise COVID-19'a bağlı sosyal izolasyonda olmanın psikolojik sağlıklarını etkilediğini, %19,1'i pandemide eş ile ilişkisinin çok olumsuz etkilendiğini, %79,6'sı COVID-19'a maruziyetin gebeliğe zarar vereceğini ve %79,2'si ise virüsün bebeğine zarar vereceğinden endişe duyduğunu bildirmiştir. Araştırmaya katılan gebelerin %5,3'ü gebeliklerinde düzenli kontrole gidemediğini ve bu durumun en önemli nedeninin“pandemi”olduğunu söylemiştir. Gebelerin Prenatal Distres Ölçeği puan ortalaması 10,31±4,12 olarak bulunmuştur. Özel sektörde çalışan, gelir durumu kötü olan, gebelik haftası 35 ve üzeri olup, şu anki gebeliğinde sağlık sorunu yaşan gebelerin prenatal distres düzeylerinin yüksek olduğu belirlenmiştir. Gebelerin COVID-19'un ve COVID-19'a bağlı sosyal izolasyonun psikolojik sağlık üzerindeki etkisi ile PDÖ puan ortalamaları karşılaştırıldığında; gruplar arasında istatistiksel olarak anlamlı fark olduğu saptanmıştır (p<0,001). COVID-19'a maruz kalmanın gebeliğe ve bebeğe zarar vereceğini düşünen gebelerin prenatal distres düzeyleri yüksektir. Ayrıca COVID-19 sürecinden dolayı gebelik takipleri için hastaneyi/doktoru ziyaret eden gebelerin PDÖ puan ortalamaları karşılaştırıldığında gruplar arasında istatistiksel olarak anlamlı fark olduğu saptanmıştır (p<0,001). Bu çalışmada gebelerin prenatal distresinin hafif düzeyde olduğu belirlenmiştir. Prenatal distres puanını özel sektörde çalışma, gelir durumu, sosyal izolasyon ve gebelik haftasının ileri olmasının etkilediği görülmüştür. Ayrıca gebelerin büyük bir kısmının COVID-19'a maruz kalmanın gebeliğine ve doğacak bebeğine zarar vereceği konusunda endişeli olduğu belirlenmiştir.

This research was conducted to determine the prenatal distress levels of pregnant women and the factors affecting it during the COVID-19 pandemic period. The study was carried out analytically and cross-sectionally with 549 pregnant women who applied to the pregnancy polyclinic of Dokuz Eylül University Hospital, Department of Obstetrics and Gynecology between February 15, 2021 and July 15, 2021. Data were collected using the 'Pregnant Information Form' and the 'Prenatal Distress Scale'. Descriptive statistics (percentage, frequency, mean and standard deviation), Student's t test, one-way analysis of variance (ANOVA), Mann-Whitney U test, Kruskall-Wallis one-way analysis of variance and post-Hoc Bonferroni tests were used to evaluate the data. 11.5% of the pregnant women participating in the study stated that they had COVID-19 before pregnancy and 5.5% during pregnancy. 5.1% of the pregnant women stated that they found the level of knowledge about COVID-19 insufficient, while 23.3% found the level of knowledge about the effect of COVID-19 on pregnancy insufficient. 37% of the pregnant women in the study stated that COVID-19 affected their psychological health, 33% of them said that being in social isolation due to COVID-19 affected their psychological health, 19.1% said that their relationship with their spouse was affected very negatively during the pandemic, 79%, Six of them reported that exposure to COVID-19 would harm the pregnancy and 79.2% were concerned that the virus would harm their baby. 5.3% of the pregnant women who participated in the study said that they could not go to regular check-ups during their pregnancies and that the most important reason for this situation was the“pandemic”. Prenatal Distress Scale mean score of pregnant women was found to be 10.31±4.12. It was determined that the prenatal distress levels of pregnant women who work in the private sector, have a low income, have a gestational week of 35 and above and have health problems in their current pregnancy are high. When the effects of COVID-19 and social isolation due to COVID-19 on the psychological health of pregnant women were compared with the mean PBL scores; It was found that there was a statistically significant difference between the groups (p<0.001). Prenatal distress levels of pregnant women who think that exposure to COVID-19 will harm the pregnancy and the baby are high. In addition, when the mean PDS scores of the pregnant women who visited the hospital/doctor for pregnancy follow-up due to the COVID-19 process were compared, it was found that there was a statistically significant difference between the groups (p<0.001). In this study, it was determined that prenatal distress of pregnant women was mild. It has been observed that working in the private sector, income status, social isolation, and advanced gestational week affect the prenatal distress score. In addition, it has been determined that most of the pregnant women are worried that exposure to COVID-19 will harm their pregnancy and the baby to be born.
