Özet

Amaç: SARS-CoV-2 virüsünün ortaya çıkmasından sonra tüm dünyayı etkisi altına alan pandemi süreci, bireyler üzerinde fiziksel ve ruhsal birçok etkiye neden olmuştur. Özellikle COVID-19 hastalığı geçiren kişilerde, bu etkilerin daha belirgin olduğu gözlemlenmiştir. Bu çalışmada, COVID-19 tanısı alan bireylerin; sosyodemografik özellikler, depresyon, stres, anksiyete düzeyleri, obsesif kompulsif belirtiler, rüya, kabuslarda meydana gelen değişiklikler, uyku ile ilişkili problemler ve bunların kronotiplerle ilişkisi açısından, COVID-19 hastalığını geçirmeyen bireylerle karşılaştırması, aynı zamanda hastalığı geçiren bireylerin, 3. ay, 6. ay izlemlerinde, yukarıda bahsedilen psikopatolojilerin ve özelliklerin seyri açısından değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmada, hasta grubuna, KSÜ Tıp Fakültesi Hastanesi COVID-19 kliniklerinde ayaktan veya yatarak takip edilen, COVID-19 pozitif 18-65 yaş arası 70 yetişkin, kontrol grubuna ise sağlıklı 18-65 yaş arası 70 yetişkin dahil edilmiştir. Katılımcılarla görüşülerek onam alınmıştır ve veriler katılımcıların ölçekleri doldurmasıyla yoluyla elde edilmiştir. Katılımcılar tarafından; Sosyodemografik Veri Formu, Depresyon, Anksiyete ve Stres Ölçeği-21 (DASÖ-21), PADUA Envanteri- Revize, Uykusuzluk Şiddet İndeksi (UŞİ), UYKU-50 Ölçeği, Van Rüya Bunaltı Ölçeği, Sabahçı Akşamcı Ölçeği Kısa Formu doldurulmuştur. Hasta grubuna, pozitifliklerinin 3. ve 6. ayında tüm ölçekler tekrarlanmıştır. Çalışmada, hem hasta ile kontrol grubu; hem de hasta grubunun pozitifliklerinin 1. ay verileri ile 3.ay, 6. ay verileri bahsedilen ölçekler ve özellikler açısından karşılaştırılmıştır. Analizler SPSS (Statistical Package for Social Sciences; SPSS Inc., Chicago, IL) 22 paket programında değerlendirilmiştir. Bulgular: Çalışmaya, 70 hasta ve 70 kontrol olmak üzere toplam 140 katılımcı dahil edilmiştir. Gruplar arasında; yaş, cinsiyet, eğitim düzeyi, gelir düzeyi, çalışma durumu açısından anlamlı farklılık tespit edilmemiştir. Hasta grubunun evli olma oranı kontrol grubunun evli olma oranından anlamlı şekilde yüksek görülmüştür (p=0,023). Hasta grubunda çocuk sahibi olma oranı, kontrol grubunda çocuk sahibi olma oranından anlamlı şekilde yüksek bulunmuştur(p=0,004). Hasta grubundakilerin karantinada kalma oranı, kontrol grubundakilerin karantina da kalma oranından anlamlı şekilde yüksek bulunmuştur (p<0,001). Hasta grubundakilerin ailesinde COVID geçirme oranı, kontrol grubundakilerin ailesinde COVID geçirme oranından anlamlı şekilde yüksek bulunmuştur(p=0,022). Hasta ve kontrol grubu karşılaştırıldığında; hasta grubunun, PADUA Envanteri-R ölçeğinin alt boyutlarından temizlik alt boyutu puanı ve Uyku-50 ölçeğinin alt boyutlarından uyku apne puanı kontrol grubunun puanından anlamlı şekilde yüksek bulunmuştur. Bunun dışında hasta ve kontrol grubu arasında; Depresyon Anksiyete Stres Ölçeği (DASÖ-21) ölçek alt boyutları ve toplam puanı, Uyku Şiddet İndeksi (UŞİ) ölçeği, PADUA Envanteri-R temizlik alt boyutu dışındaki diğer alt boyutları ve toplam puanı, Uyku-50 ölçeğinin uyku apne alt boyutu dışındaki diğer alt boyutları ve toplam puanı, Van Rüya Bunaltı Ölçeği ölçek alt boyutları ve toplam puanı, Sabahçıl Akşamcıl Ölçeği Kısa Formu açısından anlamlı farklılık görülmemiştir. Hasta grubunda yaptığımız izlem değerlendirmelerinde ise başlangıç değerlendirmesine (1. ay) kıyasla 3. ve 6. aylar da; UŞİ ölçeğinde orta şiddette ve ciddi insomni oranlarında, PADUA Envateri-R ölçeği temizlik alt boyutunda, Van Rüya Bunatlı ölçeği rüya hatırlama sıklığı alt boyutunda zamanla anlamlı derecede azalma olduğu bulunmuştur. Bunların dışında, 3. ve 6. ay izlemlerde; Depresyon Anksiyete Stres Ölçeği-21, Uyku-50 ölçeği, Sabahçıl Akşamcıl Ölçeği Kısa Formu, Van Rüya Bunaltı Ölçeğinde rüya hatırlama sıklığı dışındaki alt boyutları ve toplam puanı, PADUA Envanteri-R ölçeğinde temizlik alt boyutu dışındaki diğer alt boyutları ve toplam puanı açısından anlamlı farklılık görülmemiştir. Sonuç: Çalışmanın sonuçlarına göre; COVID-19 hastalığını geçiren kişilerin, sağlıklı kontrollere göre OKB semptomatolojisinden temizlikle ilişkili belirti görülme riskinin daha fazla olduğu söylenebilir. Ve literatürle uyumlu olarak, COVID-19 geçiren kişilerde daha yüksek oranda obstruktif uyku apnesi görüldüğü söylenebilir. Hasta ve kontrol grubu arasında depresyon, anksiyete, stres, uyku apnesi dışındaki uyku bozuklukları, rüya ve kabusla ilişkili değişikler açısından anlamlı bir fark bulunmaması; örneklem büyüklüğünün yetersiz olması, hasta grubunun çok büyük oranda (%80) hastalığı hafif geçiren bireylerden oluşması, çalışmamızın COVID-19 hastalığı ile ilgili bilinmezliğin oldukça azaldığı, COVID-19'un daha bilinir bir hastalık olduğu dönemde yapılması gibi nedenlerden kaynaklanmış olduğu söylenebilir. Ayrıca izlemler değerlendirmelerinde, yine literatürle uyumlu olarak COVID-19 hastalığı geçiren kişilerde ortaya çıkan; temizlik/kontaminasyon belirtilerinin, insomni belirtilerinin ve rüya hatırlama sıklığı gibi belirtilerin zamanla azalacağı bu çalışmada da desteklenen bir bulgudur. Ancak COVID-19 geçiren bireylerde oluşan psikopatolojiler ve seyirleri konusunda daha geniş hasta örnekleminde yapılan çalışmalar, tartışmalara önemli katkı sağlayacaktır.

Aim: The pandemic process, which affected the whole world after the emergence of the SARS-CoV-2 virus, caused many physical and mental effects on individuals. These effects have been observed to be more pronounced, especially in people with COVID-19 disease. In this study it was aimed to compare individuals diagnosed with COVID-19 with individuals who haven't had COVID-19 in terms of sociodemographic characteristics, depression, stress, anxiety levels, obsessive-compulsive symptoms, changes in dreams, nightmares, sleep-related problems and their relationship with chronotypes, at the same time evaluation of the individuals diagnosed with COVID-19 for observation of psychopathologies and features metioned above in their 3rd and 6th month follow-ups. Materials and Methods: In this study, patient group was included with 70 adults between the ages of 18-65 who were positive for COVID-19, and followed up as outpatient or inpatient in the COVID-19 clinics of KSU Medical Faculty Hospital; control group was included with 70 healthy adults between the ages of 18-65.Consent was obtained by interviewing the participants, and the data were obtained via the participants filling out the scales. By the participants; Sociodemographic Data Form, Depression, Anxiety and Stress Scale-21(DASÖ-21), PADUA Inventory- Revised, Insomnia Severity Index (ISI), SLEEP-50 Scale, Van Dream Anxiety Scale, Morning and Evening Scale Short Form were filled. All scales were repeated in the patient group in the 3rd and 6th months of COVID-19 positivity. In this study, both patient group, control group and 1st, 3rd and 6th month data of patient group were compared in terms of scales and features mentioned above. Analyzes, were evaluated in SPSS (Statistical Package for Social Sciences; SPSS Inc., Chicago, IL) 22 package programs. Results: A total of 140 participants, 70 patients and 70 controls, were included in the study. No significant difference was found in terms of age, gender, education level, income level, and employment status among the groups. The rate of being married in the patient group was found to be significantly higher than the rate of being married in the control group (p=0.023). The rate of having children in the patient group was found to be significantly higher than the rate of having children in the control group (p=0.004). The rate of staying in quarantine in the patient group was found to be significantly higher than the rate of staying in quarantine in the control group (p<0.001). The rate of having COVID in the family of the people in the patient group was found to be significantly higher than the rate of having COVID in the family of the people in the control group (p=0.022). When the patient and control groups were compared, the washing sub-dimension score of the PADUA Inventory-R scale and the sleep apnea score, which is the sub-dimension of the Sleep-50 scale of patient group were found to be significantly higher than the scores of control group. Apart from that, there was no significant difference defined between the patient and control group in terms of Depression Anxiety Stress Scale (DASÖ-21) scale sub-dimensions and total scores, Sleep Severity Index (UŞİ) scale, sub-dimensions and total score of PADUA Inventory-R other than washing sub-dimension, sub-dimensions and total score of Sleep-50 scale other than sleep apnea sub-dimension, Van Dream Anxiety Scale(RBÖ) scale sub-dimensions and total score, Morning-Evening Scale Short Form (SAÖ). In the follow-up evaluations we made in the patient group at the 3rd and 6th months compared to the initial evaluation (1st month), it was found that there was a significant decrease over time in moderate and severe insomnia rates of UŞİ scale, PADUA Inventory- R scale washing sub-dimension, Van Dream Anxiety Scale dream recall frequency sub-dimension. Apart from these, in the 3rd and 6th month follow-ups; there was no significant difference defined in terms of, Depression Anxiety Stress Scale-21, Sleep-50 scale, Morning Evening Scale Short Form, the sub-dimensions and total score of the Van Dream Anxiety Scale, excluding the frequency of dream recall, the sub-dimensions and total score of the PADUA Inventory-R scale except for the washing sub-dimension. Conclusions: According to the results of the study; It can be said that people who have had COVID-19 disease have a higher risk of showing symptoms related to washing from OCD symptomatology compared to healthy controls. And in line with the literature, it can be said to seen that people with COVID-19 have a higher rate of obstructive sleep apnea. The fact that there was no significant difference between the patient and control groups in terms of changes related to sleep disorders, dream and nightmare except depression, anxiety, stress, sleep apnea can be said to derive from the reasons like insufficient sample size, the fact that patient group mostly (%80) consists of individuals with a mild illness, and that our study was conducted during the period when the information about the COVID-19 disease increased and COVID-19 was a more known disease. In addition, in the follow-up evaluations in accordance with the literature, It is also a supported finding in this study that symptoms such as cleaning/contamination symptoms, insomnia symptoms, and frequency of dream recall occurring in people who had COVID-19 disease will decrease over time. However, studies on the psychopathologies and prognosis of individuals with COVID-19 in a larger patient sample will contribute significantly to the discussions.
