Özet

Giriş ve Amaç: Meme koruyucu cerrahide etkili postoperatif analjezi yönetimi ile erken iyileşme ve kronik ağrının önlenebilmektedir. Erektör Spina Plan (ESP) blok ilk kez 2016 yılında postoperatif analjezi amacıyla tanımlanmıştır. Pektoral Blok Tip II (PECS II) blok ise 2012 yılından itibaren etkin bir şekilde meme cerrahilerinde postoperatif analjezi amacıyla kullanılmaktadır. Biz çalışmamızda PECS II blok ile ESP bloğun meme koruyucu cerrahi prosedürlerinde, postoperatif analjezi üzerine etkinliklerini karşılaştırmayı amaçladık. Gereç ve Yöntem: Gülhane Eğitim ve Araştırma Hastanesi Tıp Fakültesi Etik Kurulu'nun 2021/261 sayılı etik kurul onayı alındıktan sonra çalışmaya başlandı. Çalışmaya, elektif şartlarda genel anestezi altında meme koruyucu cerrahi ile Sentinel Lenf Nodu Biyopsisi (SLNB); meme koruyucu cerrahi, SLNB ile aksilla diseksiyonu operasyonu olacak, ASA I-II-III, 18-70 yaş arası çalışmayı kabul eden toplam 75 hasta dahil edildi. Cerrahi genel anestezi altında gerçekleştirildi. Operasyon bitiminde hastalar kapalı zarf tekniği ile ESP (Grup E), PECS II (Grup P) ve kontrol grubu (Grup K) olmak üzere rastgele 3 gruba ayrıldı. Cerrahi sürenin bitiminin ardından, hastalardan E grubuna 40 ml %0,25 bupivakain ile ultrasonografi eşliğinde ESP blok, P grubuna 40 ml %0,25 bupivakain ile ultrasonografi eşliğinde PECS II bloğu yapıldı, kontrol grubuna ise herhangi bir düzlem bloğu uygulanmadı, 1 mg/kg intravenöz tramadol ile analjezi sağlandı. Kontrendikasyon bulunmayan tüm hastalara intraoperatif 1 gr parasetamol uygulandı. Bütün hastalara anestezi sonrası bakım ünitesinde tramadol içeren intravenöz hasta kontrollü analjezi (HKA) cihazı bağlandı. Postoperatif dönemde hastalar 0. dk, 15. dk, 30. dk 1. saat 3. saat, 6. saat, 9. saat ve 24. saatte NRS ile ağrı skorları, toplam tüketilen tramadol miktarı ve komplikasyonlar açısından değerlendirildi. İstatistiksel analizler için“IBM SPSS Statistics 20”programı kullanıldı ve p<0,05 altındaki değerler istatistiksel olarak anlamlı kabul edildi. Bulgular: Hasta gruplarının demografik verilerinde, istatistiksel olarak anlamlı fark görülmedi. Grupların postoperatif NRS skorları değerlendiğinde postoperatif 0. dk da Kontrol grubunda ESP ve PECS II grubuna göre istatistiksel olarak anlamlı yükseklik saptandı. 24 saatlik toplam tramadol tüketimleri karşılaştırmasında ise ESP grubunda 120,8 mg, PECS II grubunda 160,8 mg, Kontrol grubunda ise 257,2 mg hesaplandı. İstatistiksel olarak ESP ve PECS II grubunda tramadol tüketimi kontrol grubuna göre anlamlı düzeyde düşük saptandı (p<0,05). ESP ve PECS II gruplarında toplam opioid tüketimleri açısından fark yoktu. Çalışmaya alınan hastaların hiçbirinde komplikasyon gelişmedi. Bulantı, kusma ve diğer yan etkiler açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmadı. Sonuç: Bu çalışma sonucunda meme koruyucu cerrahi operasyonlarında ESP ve PECS II bloklarının kontrol grubuna göre daha az tramadol tükettiğini saptadık. ESP ve PECS II blok açısından tramadol tüketimlerinin istatistiksel anlamlılık oluşturmamıştır. Meme koruyucu cerrahide postoperatif analjezi sağlamak amacıyla her iki düzlem bloğu da tercih edilebilir.

ABSTRACT Introduction and Aim: Early recovery and chronic pain can be prevented with effective postoperative analgesia management in breast conserving surgery. Erector Spina Plan (ESP) block was first defined in 2016 for postoperative analgesia. Pectoral Block Type II (PECS II) block has been effectively used for postoperative analgesia in breast surgeries since 2012. In our study, we aimed to compare the efficacy of PECS II block and ESP block on postoperative analgesia in breast conserving surgical procedures. Materials and Methods: The study was initiated after the ethics committee approval numbered 2021/261 of Gülhane Training and Research Hospital Medical Faculty Ethics Committee. The study included breast-conserving surgery and Sentinel Lymph Node Biopsy (SLNB) under general anesthesia under elective conditions; A total of 75 patients who will undergo breast conserving surgery, axilla dissection with SLNB, ASA I-II-III, between the ages of 18-70, who agreed to the study were included. Surgery was performed under general anesthesia. At the end of the operation, the patients were randomly divided into 3 groups using the closed envelope technique: ESP (Group E), PECS II (Group P) and control group (Group K). After the end of the surgical period, ESP block was performed with 40 ml of 0,25% bupivacaine under ultrasound guidance in the E group, PECS II block was performed with 40 ml of 0,25% bupivacaine in the P group, and no plane block was applied to the control group, 1 mg/kg. Analgesia was provided with intravenous tramadol. All patients without contraindications were administered 1 g of paracetamol intraoperatively. Intravenous patient-controlled analgesia (PCA) device containing tramadol was connected to all patients in the post-anesthesia care unit. In the postoperative period, patients were evaluated in terms of pain scores, total amount of tramadol consumed, and complications with NRS at 0th min, 15th min, 30th min, 1st hour, 3th hour, 6th hour, 9th hour, and 24th hour. The“IBM SPSS Statistics 20”program was used for statistical analysis and values below p<0,05 were considered statistically significant. Results: There was no statistically significant difference in the demographic data of the patient groups. When the postoperative NRS scores of the groups were evaluated, a statistically significant increase was found in the control group at the postoperative 0th minute compared to the ESP and PECS II groups. In the comparison of 24-hour total tramadol consumption, 120,8 mg in the ESP group, 160,8 mg in the PECS II group, and 257,2 mg in the Control group were calculated. Statistically, the consumption of tramadol in the ESP and PECS II group was significantly lower than in the control group (p<0,05). There was no difference in total opioid consumption in the ESP and PECS II groups. No complications developed in any of the patients included in the study. There was no statistically significant difference between the groups in terms of nausea, vomiting and other side effects. Conclusion: As a result of this study, we determined that ESP and PECS II blocks consumed less tramadol in breast conserving surgery operations compared to the control group. Tramadol consumptions were not statistically significant in terms of ESP and PECS II block. In breast-conserving surgery, both plane blocks can be preferred to provide postoperative analgesia.
