Özet
Adipoz doku hücre sayısı ve büyüklüğü bakımından enerji ihtiyacı ve tüketimine bağlı olarak, yaşam boyu sürekli hacim değişkenliği gösteren bir dokudur. Adipoz doku salgıladığı enzim, sitokin, büyüme faktörü ve hormonlarla biyolojik fonksiyonlar ve özellikle enerji metabolizmasının düzenlenmesinde çok önemli bir yere sahiptir. Bu çalışmada, adipoz doku kaynaklı mezenkimal kök hücre tedavisi uygulanmış, rutin diz egzersizi verilmiş hastalarda diz fonksiyonelliğinin fiziksel test skorlamalarıyla etkisi değerlendirilmiştir. Çalışmaya kök hücre tedavisi almış 25 hastanın dosyaları, tedavi öncesi ve sonrası değerlendirmelerini içeren, Diz Değerlendirme Formu, Berg Denge Ölçeği, Zamanlı Kalk ve Yürü Testi, Western Ontarioand McMaster Universities Osteoarthritis Index (WOMAC) ve Visual Analogue Scale (VAS) gibi dizin fonksiyonel kapasitesini değerlendiren test skorlamaları ve egzersiz reçetesi kayıtlarına ulaşılarak toplanmıştır. Hastaların operasyon öncesi ve sonrası 1. ve 2. kez olmak üzere bir ay ara ile diz fiziksel fonksiyon ölçümleri karşılaştırıldı. Dominant ve non-dominat eklem hareket açıklığı operasyon öncesine göre anlamlı olarak gelişti (p<0.001). Diz ekleminde Ağrı ve Yorgunluk VAS değerlendirildiğinde, operasyon sonrası ölçümlerin, operasyon öncesine göre anlamlı bir şekilde azaldığı tespit edildi (p<0.001). 30 sn kalk-otur ve 6 dk yürüme testi sonuçları operasyon sonrasında anlamlı bir şekilde hızlanmıştır (p<0.001 ). BERG ve WOMAC toplam skorlarının takip eden ölçümlerde anlamlı olarak geliştiği tespit edildi (p<0.001). BERG ölçeğinde yönergenin gelişimi, ölçümler arasında 3. ve 10. yönergeler hariç, operasyon sonrası 1. ölçümde operasyon öncesi ölçüme göre anlamlı olarak gelişmiştir (p<0.001). Operasyon sonrası bazı BERG skorlaması yönergelerinde özellikle rotasyonel denge özelliklerinde anlamlı olarak gelişim göstermediği tespit edilmiştir (p>0.05). Sonuç olarak, kök hücre tedavisi uygulanmış hastalarda verilen fizik tedavi programının; diz eklem ağrısını azalttığı, genel denge özelliklerini ve diz eklem fonksiyonlarını olumlu yönde geliştirdiği tespit edilmiş, ancak rotasyonel denge özelliklerinde anlamlı bir gelişim sağlamamıştır.

Depending on its energy need and consumption, adipose tissue is a tissue that shows continuous volume variability throughout life in terms of cell number and size. Adipose tissue has a very important place in the regulation of biological functions and especially energy metabolism due to the enzymes, cytokines, growth factors and hormones it secretes. In the current study, the effect of knee functionality in patients who received adipose tissue-derived mesenchymal stem cell therapy and routine knee exercise was evaluated through physical test scores. Within the scope of the study, information including the files of 25 patients who received stem cell therapy, their evaluations before and after the treatment, test scoring tools assessing the functional capacity of the knee such as Knee Evaluation Form, Berg Balance Scale, Timed Up and Go Test, Western Ontario and McMaster University Osteoarthritis Index (WOMAC) and Visual Analogue Scale (VAS)and exercise prescription records were accessed and gathered. Knee physical function measurements of the patients were compared, once before and after the operation, with an interval of one month. Dominant and non-dominant range of motion improved significantly compared to the motion prior to the operation (p<0.001). Pain and Fatigue VAS in the knee joint were evaluated and postoperative measurements were found to be significantly reduced compared to preoperatively (p<0.001). 30 sec stand-sit and 6 min walk test results were significantly accelerated after the operation (p<0.001). BERG and WOMAC total scores have been observed to develop significantly in the following measurements (p<0.001). The development of the items in the BERG scale was significantly improved at the 1st postoperative measurement compared to the preoperative measurement, except for the 3rd and 10th items between measurements (p<0.001). It was determined that there was no significant improvement in some of the post-operative BERG scoring guidelines, especially in rotational balance characteristics, after the operation (p>0.05). In conclusion, it was determined that the physical therapy program applied to the patients who received stem cell therapy reduced knee joint pain, improved general balance characteristics and knee joint functions positively, but did not provide a significant improvement in rotational balance characteristics.
