Özet

Bu tanımlayıcı kesitsel çalışmada, COVID-19 pandemisinde, ameliyathanelerde kişisel koruyucu ekipman (KKE) kullanımına bağlı cerrahi ekip üyelerinin yaşadığı zorluklar ve bu zorlukların şiddetinin belirlenmesi amaçlanmıştır. Çalışmamız İstanbul Bakırköy Dr. Sadi Konuk Eğitim ve Araştırma Hastanesi'nde, etik kurul izni, Sağlık Bakanlığı bilimsel araştırma onayı ve kurum izni sonrası, 203 cerrahi ekip üyesinin katılımıyla gerçekleştirildi. Araştırma verileri anket yöntemiyle toplandı. Katılımcıların yaş ortalaması 32.6±8.4 yıl, %55.7'si kadın, %43.8'i doktor, %33.0'ü hemşire ve %23.2'si anestezi teknikeriydi. KKE'lara bağlı sorunlar değerlendirildiğinde; katılımcıların %91.6'sı solunum, %94.1'i görme, %61.6'sı işitme kalitesinde, %93.6'sı ekip içi iletişimde azalma ve %89.2'si buğulanma sorunu yaşadıklarını ifade etti. Terleme, sıcaklık hissi, susama ve cilt sorunlarında artış bildirildi (sırasıyla % 96.1, % 95.1, % 91.6, % 85.2). Katılımcıların %88.2'si hareket kabiliyetinde, %80.3'ü taktil duyuda azalma olduğunu belirtti. Fiziksel güç, konsantrasyon ve psikolojik tolerensta azalma sırasıyla %79.3, %78.8, %85.2 idi. Katılımcıların 90.1'i açık havada dinlenme ve %86.7'si duş olanaklarının olmadığını bildirdi. Katılımcıların yorgunluk artışı 95.1 ve başağrısı %82.3 oranındaydı. Katılımcıların % 73.9'u en çok zorlandıkları KKE'nın tulum olduğunu, %60.1'i KKE'larla verimli çalışabilecekleri sürenin <2 saat olduğunu bildirdi. Görme kalitesinde azalma, termal stres, terlemede artış ve psikolojik toleransta azalma hekimlerde daha yüksekti (p<0.05). İşitmede sınırlılık hemşirelerde ve 10 yıldan fazla mesleki deneyimi olanlarda daha yüksekti (p<0.05). Cilt sorunları, fiziksel ve psikolojik toleransta azalma erkeklerde daha fazlaydı ( p<0.05). Çalışma sonuçları“COVID-19 Ameliyathane KKE”bileşenlerinin çalışanlar üzerinde pek çok alanda, olumsuz etkiler oluşturduğunu gösterdi. Gelecekte yaşanabilecek benzer durumlar açısından KKE'ın yalnızca enfeksiyon geçişi yönü ile değil aynı zamanda çalışan sağlığına uygun yapı ve tasarımda olması önerilir.

This descriptive cross-sectional study aimed to determine the challenges and the severity of these challenges experienced by surgical team members due to the use of personal protective equipment (PPE) in operating rooms during the COVID-19 pandemic. Our study was carried out after Istanbul Bakirkoy Dr. Sadi Konuk Training and Research Hospital ethics committee approval, Ministry of Health scientific research approval, and institution permission, with the participation of 203 surgical team members. Research data were collected by survey methodology. The mean age of the participants was 32.6±8.4 years, 55.7% were women, 43.8% were doctors, 33.0% were nurses and 23.2% were anesthesia technicians. When the problems related to PPE were evaluated, 91.6% of the participants stated that they had respiratory problems, 94.1% vision, 61.6% hearing quality, 93.6% communication within the team, and 89.2% condensation problems. The participants reported increased sweating, feeling of warmth, thirst, and skin problems (96.1%, 95.1%, 91.6%, 85.2%, respectively). 88.2% and 80.3% of the participants stated that there was a decrease in mobility and tactile sensation, respectively. The decrease in physical strength, concentration and psychological tolerance was 79.3%, 78.8% and 85.2%, respectively. 90.1% of the participants reported that they did not have the opportunity to rest in the open area and 86.7% of them had no shower facilities. The rate of fatigue increase of the participants were 95.1% and the headache were 82.3%. 73.9% of the participants reported that the most difficult PPE was overall, and 60.1% reported that the time they could work efficiently with PPE was lesser than 2 hours. Decreased vision quality, thermal stress, increased sweating and decreased psychological tolerance were higher among physicians (p<0.05). The hearing limitation was higher in nurses and those with more than 10 years of professional experience (p<0.05). The skin problems and decreased physical and psychological tolerance were greater in male participants (p<0.05). The study results showed that the components of“COVID-19 Operating Room PPE”negatively affect employees in many areas. In terms of similar situations that may occur in the future, it is recommended that PPE should not only be in terms of infection transmission but also a structure and design suitable for employees' health.
