Özet

Bu çalışmanın amacı, servikal disk hernisine sahip olan hastalarda fasyal gevşetme teknikleri uygulayarak otonomik sinir sistemi üzerine etkilerini incelemektir. Çalışmaya alınma kriterlerini karşılayan servikal disk hernisine sahip ve yaşları 18 - 40 arası değişen 50 hasta üzerinde yapıldı. Çalışmaya dahil edilen 50 gönüllüye sadece 1 seans fasyal gevşetmeye yönelik tedavi uygulandı. Uygulama öncesi, ilgili anket ve form dolduruldu ve ardından otonomik ölçüm cihazı ile kalp hızı değişimi ölçüldü. Uygulama sonrası tekrar otonomik ölçüm cihazı ile kalp hızı değişimleri ölçülerek uygulama öncesi ve sonrası değerler kontrol edilerek değerlendirme yapıldı.İlgili anketler uygulamadan sonraki güne doldurulması istendi. Uygulama sonrası doldurulması istenen ilgili anketler: ağrıyı değerlendirmek için Vizüel Ağrı Skala (VAS), boyun problemleri sebebiyle günlük yaşamda meydana gelen özrü değerlendirmek için Boyun Özürlülük Skalası, emosyonel durumun değerlendirilmesi için Beck Depresyon Ölçeği, yaşam kalitesini değerlendirmek amaçlı ise Notthingham Sağlık Profili (NSP) anketleri kullanıldı. Katılımcıların boyun özürlülük anketi(BÖS) ile Notthingham sağlık profilinin(NSP) tedavi öncesi ile tedavi sonrasındaki sonuçları arasında anlamlı bir fark oluştu(p<0,05). Hafif ve orta düzey boyun özürlü olanlarda ise BÖS sonucu bakımından her ikisinde de tedavi öncesi ile tedavi sonrası sonuçları anlamlı bir fark oluştu(p<0,05). NSP skoru tedavi öncesi sonucu anlamlı bir fark oluştuğu(p<0,05), ancak hafif boyun özürlülerde(BÖS<15) NSP tedavi öncesi ile tedavi sonrası sonucunda anlamlı bir fark oluşmadı(p>0,05). PNS index, Beck depresyon ve VAS tedavi öncesi ile tedavi sonrasındaki sonuçları arasında anlamlı bir fark oluştu(p<0,05). Hafif ve orta seviye boyun özürlü olgularda otonomik PNS indeks, Nottingham sağlık profili dışında PNS index, Beck depresyon ve VAS tedavi öncesi ile tedavi sonrasındaki sonuçları arasında anlamlı bir fark oluştu(p<0,05). Çalışmanın sonucunda katılımcılarda tedavi sonrası PNS İndex, tedavi öncesi ve tedavi sonrası NSP, Beck depresyon, VAS ve BÖS skorları arasındaki ilişki tedavi sonrası sonucu pozitif yönlü yüksek düzey(p<0,05) belirlendi.

This study aims to examine the effects on the autonomic nervous system by applying fascial release techniques in patients with cervical disc herniation. The study was performed on 50 patients with cervical disc herniation, aged between 18 and 40, who met the inclusion criteria. Only one session of fascial release treatment was applied to 50 volunteers included in the study. Before the application, the relevant questionnaire and form were filled out, and then the change in heart rate was measured with an autonomic measuring device. After the application, the heart rate changes were measured again with the autonomic measuring device, and the values were checked before and after the application. The relevant questionnaires were asked to be filled in the day after the application. Relevant questionnaires to be filled in after the application were as followings: Visual Pain Scale (VAS) to assess pain, Neck Disability Scale to assess disability in daily life due to neck problems, Beck Depression Scale to assess the emotional state, Nottingham Health Profile (NHP) to assess the quality of life questionnaires. There was a significant difference between the results of the participants' Neck Disability Questionnaire (NDQ) and Nottingham Health Profile (NHP) before and after treatment (p<0.05). There was a significant difference between pre-treatment and post-treatment results in both patients with mild and moderate neck disability in terms of Neck Disability Questionnaire (NDQ) results (p<0.05). There was a significant difference in the NHP score before the treatment (p<0.05), but there was no significant difference in the NHP pre-treatment and post-treatment results in mild neck disabilities (NDQ<15) (p>0.05). There was a significant difference between the results of PNS index, Beck depression, and VAS before and after treatment (p<0.05). There was a significant difference among the results of autonomic PNS index, PNS index except for Nottingham Health Profile, Beck Depression, and VAS before and after treatment in cases with mild and moderate neck disability (p<0.05). As a result of the study, the correlation among the post-treatment PNS Index, pre-treatment and post-treatment NHP, Beck Depression, VAS, and NDQ scores was found to be positive and high(p<0.05).
