Özet

Amaç: Non-motor semptomların çeşitli hareket bozukluklarında yaşam kalitesi ve morbidite üzerinde önemli etkileri olduğu kabul edilmektedir. Bununla birlikte, bu semptomların hastalıklarda görülme sıklığı, etiyolojisi, patofizyolojisi ve tedavisi yeterince tanımlanmamıştır. Çalışmamızda servikal distoni, hemifasiyal spazm, blefarospazm tanıları ile takip edilen hastalarda psikiyatrik bozukluklar, bilişsel durum, uyku başta olmak üzere çeşitli non-motor semptomları değerlendirmeyi ve bu semptomların motor semptomların şiddeti ile ilişkili olup olmadığını araştırmayı amaçladık. Gereç ve Yöntem: Araştırma Mart 2021- Haziran 2021 tarihleri arasında Ankara Şehir Hastanesi Nöroloji Polikliniği'nde kesitsel, gözlemsel, vaka-kontrol çalışması olarak yapıldı. Araştırmaya katılmayı kabul eden ve hemifasiyal spazm (HFS), blefarospazm (BFS) ve servikal distoni (SD) tanılarından birini almış, 18-75 yaş arasındaki hastalar ile kontrol grubu olarak yaş, cinsiyet, eğitim düzeyi uyumlu olan ve hareket bozukluğu tanısı, klinik bulguları olmayan gönüllüler çalışmaya dahil edildi. Hastalık şiddetlerini belirlemek için her gruba özgü motor şiddet ölçekleri kullanıldı. Non-motor semptomları değerlendirmek için Türkçe versiyonunun geçerlilik ve güvenilirlik çalışması bulunan Montreal bilişsel değerlendirme ölçeği (MoCA), Pittsburgh uyku kalite indeksi (PSQI), Epworth uykululuk skalası (ESS), non-motor semptomlar skalası (NMSQ), Beck depresyon ölçeği (BDI), Beck anksiyete ölçeği̇ (BAI) hasta ve kontrol gruplarına uygulandı. Veriler IBM SPSS V 23 ile analiz edildi. Önem düzeyi p<0,05 alındı. Bulgular: Hasta gruplarında literatür ile benzer şekilde kadın cinsiyet hakimiyeti ve benzer başlangıç yaşı tespit edildi. Çalışmamızda SD hasta grubunda MoCA toplam skoru, 'lisan', 'soyut düşünme', 'yönelim' kontrol grubuna göre istatistiksel anlamlı olarak düşük; PSQI, ESS ve NMSQ puanları ise kontrollere göre istatistiksel anlamlı yüksek bulunmuştur. BFS hasta grubunda MoCA toplam skoru ve 'lisan' puanı kontrollere göre anlamlı düşük, NMSQ puanı ise kontrollere göre istatistiksel olarak anlamlı yüksek bulunmuştur. Hemifasiyal spazm hasta grubunda MoCA toplam puanı, 'yürütücü işlevler', 'lisan', 'soyut düşünme' puanları istatistiksel olarak kontollere göre anlamlı düşük; PSQI puanları kontrollere göre istatistiksel olarak anlamlı yüksektir. SD hasta grubunda şiddet skoru ile PSQI puanı arasında, BFS hasta grubunda şiddet skoru ile NMSQ puanı arasında pozitif yönlü ilişki bulunmuştur. Anlamlı bilişsel bozukluğun üç grupta da olduğu saptanmıştır. Bunun yanında uyku problemleri hastalar açısından sorun oluşturan başlıca non-motor semptomlardandır. Hastalık süreleri ile yapılan test skorları arasında bir ilişki tespit edilmemiştir. Sonuç: Servikal distoni, bleferospazm ve hemifasiyal spazm gibi hiperkinetik hareket bozukluklarında motor belirtilerin yanısıra non-motor belirtiler de gözardı edilemeyecek sıklıkta görülebilmektedir ve hastaların yaşam kalitelerinde bozulmalara neden olabilmektedir. Dolayısıyla hastaların takiplerinde, tedavilerinde non-motor semptom sorgulaması yapmak ve NMS bulunan hastaları tedavi etmek hastaların yaşam kalitelerine olumlu katkı sağlayacaktır. Non-motor semptomların kesin patofizyolojisinin saptanmasında yapılacak geniş hasta katılımlı nörofizyolojik, nörogörüntüleme, nörogenetik ve nörobiyolojik çalışmalar önem arz edecektir. Biz de çalışmamızla bu alanda yapılacak çalışmalara katkı sağlamak istedik.

Objective: Non-motor symptoms are considered as an important modifier of quality of life and morbidity in various movement disorders. However; the incidence, etiology, pathophysiology and treatment of these symptoms in diseases have not been adequately defined. In our study, we aimed to evaluate various non-motor symptoms, especially psychiatric disorders, cognitive status, and sleep, in patients followed up with the diagnoses of cervical dystonia, hemifacial spasm, and blepharospasm, and to investigate whether these symptoms are related to the severity of motor symptoms. Materials and Methods: The study was conducted as a cross-sectional, observational, case-control study in Ankara City Hospital Neurology Department between March 2021 and June 2021. Patients between the ages of 18-75 who accepted to participate in the study and had one of the diagnoses of hemifacial spasm (HFS), blepharospasm (BPS) and cervical dystonia (CD). Healthy volunteers who were matched for age, gender, educational background and had no movement disorder diagnosis and clinical findings as the control group were included in the study. Motor severity scales specific to each group were used to determine disease severity. To evaluate non-motor symptoms, the Turkish version of the Montreal cognitive assessment scale (MoCA), Pittsburgh sleep quality index (PSQI), Epworth sleepiness scale (ESS), non-motor symptoms questionnarie (NMSQ), Beck depression scale (BDI), Beck anxiety scale (BAI) were applied to the patient and control groups. Data were analyzed with IBM Statistical Package for Social Sciences (SPSS) program V23. The level of significance was assumed as p<0.05. Results: Similar to the literature, female predominance and a similar age of onset were detected in the patient groups. In our study, MoCA total score, 'language', 'abstract thinking', 'orientation' were statistically significantly lower in the CD patient group than in the control group; PSQI, ESS and NMSQ scores were statistically significantly higher than controls. In the BPS patient group, the MoCA total score and the 'language' score were significantly lower than the controls, and the NMSQ score was statistically significantly higher than the controls. In the hemifacial spasm patient group, MoCA total score, 'executive functions', 'language', and 'abstract thinking' scores were statistically significantly lower than in controls; PSQI scores are statistically significantly higher than controls. A positive correlation was found between the severity score and the PSQI score in the CD patient group, and between the severity score and the NMSQ score in the BPS patient group. Significant cognitive impairment was found in all three groups. In addition, sleep problems are among the main non-motor symptoms that cause issues for patients. No correlation was found between disease duration and test scores. Conclusion: In hyperkinetic movement disorders such as cervical dystonia, blepherospasm and hemifacial spasm; non-motor symptoms are apparent with an undeniable frequency in addition to common motor symptoms. Importantly, these non-motor symptoms may cause significant deterioration in the quality of life of the patients. Therefore, evaluating non-motor symptoms in the follow-up and treatment processes of the patients and intervening with appropriate treatments should contribute positively to the quality of life of the patients. Neurophysiological, neuroimaging, neurogenetic and neurobiological studies with large patient participation will be important in determining the exact pathophysiology of non-motor symptoms. We wanted to contribute to the literature in this field with our study.
