Özet
Amaç: Hiponatremi en yaygın elektrolit bozukluğudur. Hiponatremide nedeni saptayabilmek ve tedaviyi bu nedene göre şekillendirmek için idrar sodyumuna, idrar osmolalitesine ve volüm durumunu değerlendirebilmek için fizik muayene bulgularına ihtiyaç vardır. Biz bu çalışmada volüm durumunu değerlendirebilmek için yatak başı ultrasonografi ile inferior vena cavanın kollapsibilitesinin etkinliğini belirlemeyi amaçladık. Gereç ve Yöntem: 1 Mayıs 2021-1 Şubat 2022 tarihleri arasında acil servisimize sodyum değeri 125 mEq/L'nin altında olan, hipertonik ve izotonik hiponatremik olmayan, en az bir hiponatremi semptomu olan, invaziv ventilasyon desteği almayan, 18 yaş üstü, akut hiponatremisi olan 110 hasta çalışmaya dahil edildi. Tedavi öncesinde, Mindray marka M5 model ultrasonografi cihazı kullanılarak subkostal pencereden, konveks prob ile görüntüler elde edildi (Mindray, NJ, ABD). M-modda inferior vena cavanın sağ atriyuma döküldüğü yerin 2 cm proksimaline imleç yerleştirilerek solunum siklusu ile değişen vena cava çapı kaydedildi. Volüm grupları geleneksel yöntemlerle belirlenerek IVC çapları karşılaştırıldı. İstatistiksel analiz için SPSS 23.0 for Windows programı kullanıldı ve anlamlılık seviyesi p<0,05 olarak kabul edildi. Bulgular: Tüm örneklem grubunda 40 hipovolemik, 40 övolemik ve 30 hipervolemik hasta vardı. Hipovolemik hasta grubunun IVC CI (inferior vena cava kollapsibilite indeksi) değerini 53,15±11,56 bulduk. Övolemik grupta IVC CI değeri 40,57±10,76 idi. Hipervolemik grupta IVC CI değerini 25,09±12,1 bulduk. Üç volüm durumunu (hipovolemik, övolemik, hipervolemik) kendi arasında karşılaştırdığımızda fark istatiksel olarak fark anlamlıydı (p<0.001). Sonuç: Bu çalışmayla hipotonik hiponatremide volüm durumunun değerlendirilmesinde USG'nin IVC ölçümleri ile kullanılabileceğini ve fizik muayeneden üstün olduğunu saptadık.

Objective: Hyponatremia is the most common electrolyte disorder. Urine sodium and urine osmolality are needed to detect the cause of hyponatremia and to organise the treatment accordingly and assessment of volume status in patients relies on physical examination findings. The aim of this study is to determine the effectiveness of inferior vena cava collapsibility with bedside ultrasonography to assess volume status. Patients and methods: This prospective, investigator-initiated, single-center, randomized clinical trial enrolled 110 patients older than 18 years with serum sodium level below 125 mEq/L, who were not hypertonic or isotonic hyponatremic, without invasive ventilation support, with acute hyponatremia, had at least one symptom of hyponatremia. Recruitment took place from May 1, 2021 until February 1, 2022, to the emergency department of Ankara Diskapi Yildirim Beyazit Training and Research Hospital. Before the treatment, images were obtained from the subcostal window with a convex probe using the Mindray brand M5 model ultrasonography device (Mindray, NJ, ABD). In M-mode, the diameter of the vena cava, which changes with the respiratory cycle, was recorded by placing the cursor 2 cm proximal to the point where the inferior vena cava empties into the right atrium. SPSS 23.0 for Windows was used for statistical evaluations and p<0.05 was accepted as statistical significance limit. Results: In the whole sample, there were 40 hypovolemic, 40 euvolemic, and 30 hypervolemic patients. We found that IVC CI value of the hypovolemic patient group was 53.15±11.56. The IVC CI value in the euvolemic group was 40.57±10.76 and in the hypervolemic group was 25,09±12,1. When we compared the three volume conditions (hypovolemic, euvolemic, hypervolemic) among themselves, there was a was statistically significant difference (p<0,001). Conclusion: In this study, we concluded that USG can be used with IVC measurements in the evaluation of the volume status in hypotonic hyponatremia and is superior to physical examination.
