Özet
Bu çalışma infertil çiftlere PLISSIT modeli doğrultusunda verilen cinsel danışmanlığın cinsel işlev ve cinsel doyuma etkisini belirlemek amacıyla yapılan randomize kontrollü deneysel bir çalışmadır. Çalışma İstanbul Üniversitesi İstanbul Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı Reprodüktif Endokrinoloji ve İnfertilite Bilim Dalı İnfertilite Kliniği'nde Ağustos 2019 - Şubat 2020 tarihleri arasında yürütülmüştür. Örnekleme cinsel işlev bozukluğu olan toplam 60 infertil çift dahil edilmiştir. Deney grubu 30 çift ile toplam 6 görüşme (ilk görüşme, en az birer hafta ara ile PLISSIT modeline dayalı 3 cinsel danışmanlık, ikinci ayda 1 telefon görüşmesi ve üçüncü ayda son görüşme), kontrol grubu 30 çift ile toplam 3 görüşme (ilk görüşme- ikinci ayda 1 telefon görüşmesi ve üçüncü ayda son görüşme) yapılmıştır. Verilerin toplanmasında Tanıtıcı Bilgi Formu, Kadın Cinsel Fonksiyon İndeksi (KCFİ), Arizona Cinsel Yaşantılar Ölçeği Erkek Formu (ACYÖ), Beck Depresyon Ölçeği (BDÖ), Yeni Cinsel Doyum Ölçeği (YCDÖ) ve İnfertilite Stresi Ölçeği (İSÖ) kullanılmıştır. Kadın ve erkeklerin ön görüşmedeki BDÖ toplam ortalama puanları deney ve kontrol grubunda benzerdi (p>0,05). Cinsel danışmanlık alan deney grubu kadınların son değerlendirmelerinde KCFİ ve YCDÖ toplam ölçek puanları ve alt boyut puanları kontrol grubundan yüksek, İSÖ toplam ölçek puanları ve alt boyut puanları kontrol grubuna göre daha düşük bulundu (p<0,05). Erkeklerde ise, deney grubundaki son değerlendirmelerde ACYÖ puan ortalamasının kontrol grubuna göre olumlu şekilde daha düşük olduğu, YCDÖ toplam ölçek ve alt boyut puanlarının kontrol grubuna göre daha fazla olduğu ve İSÖ toplam ölçek ve alt boyut puanlarının kontrol grubu erkeklere göre daha düşük olduğu saptandı. (p<0,05). Sonuç olarak, cinsel işlev bozukluğu olan ve cinsel doyum düzeyi düşük olan infertil çiftlere PLISSIT modeli kapsamında verilen cinsel danışmanlığın çiftlerin cinsel fonksiyonlarını ve cinsel doyumlarını olumlu yönde etkilediği gözlenmiştir. İnfertil çiftlere yönelik cinsel danışmanlıkta PLISSIT modelinin kullanılması önerilir.

This study is a randomized controlled experimental study to determine the effect of sexual counseling given to infertile couples in line with the PLISSIT model on sexual function and sexual satisfaction. The study was conducted between August 2019 - February 2020 in the Infertility Clinic of Istanbul University, Istanbul Medical Faculty, Department of Obstetrics and Gynecology, Reproductive Endocrinology and Infertility. A total of 60 infertile couples with sexual dysfunction were included in the sampling. A total of 30 couples were interviweved in 6 sessions (first interview, 3 sexual counseling sessions based on the PLISSIT model at least one week apart, 1 phone call in the second month and the last interview in the third month), and 30 couples in the control group were interviewed in 3 sessions (first interview - 1 phone call in the second month and the last interview in the third month). Descriptive Information Form, Female Sexual Function Index (FSFI), Arizona Sexual Experiences Scale Male Form (ASEX), Beck Depression Inventory (BDI), New Sexual Satisfaction Inventory (NSSI) and Infertility Stress Scale (ISS) were used to collect the data. BDI total mean scores of women and men in pre-interviews were similar in the experimental and control groups (p>0.05). In the final evaluations of the experimental group women who received sexual counseling, the total scale scores and sub-dimension scores of FSFI and NSSI were found to be higher than the control group, and the ISS total scale scores and sub-dimension scores were lower than the control group (p <0.05). In the final evaluations in the experimental group, it was determined that the mean ASEX score was positively lower than the control group, the total scale and sub-dimension scores of the NSSI were higher than the control group, and the ISS total scale and sub-dimension scores were lower than the control group. (p<0.05). As a result, it was observed that sexual counseling given to infertile couples with sexual dysfunction and low sexual satisfaction within the scope of the PLISSIT model positively affected the sexual functions and sexual satisfaction of the couples. It is recommended to use the PLISSIT model in sexual counseling for infertile couples.
