Özet
Septorinoplasti hastalar tarafından talep ve kabul gören, sık uygulanan kozmetik ve fonksiyonel bir cerrahi prosedürdür. Lateral ve median osteotomi rinoplasti operasyonlarındaki önemli basamaklardandır. Nazal dorsum düzeltilmesi sonrasında oluşan açık çatıyı kapatmak için kullanılan osteotomide ortaya çıkan vibratuar basınç kemik yolu ile işitsel ve vestibüler sistem üzerinde indirekt etki oluşturabilir. Bizim çalışmamızın amacı septorinoplasti operasyonlarında sık kullanılan osteotominin işitsel ve vestibüler sistem üzerine etkisini incelemektir. Çalışmamıza, çalışma grubuna septorinoplasti sırasında osteotomi yapılan 30 hasta (14 erkek, 16 kadın) ve kontrol grubuna ise başka nedenle burun ameliyatı olup osteotomi yapılmayan 10 (5 erkek, 5 kadın) hasta dahil edilmiştir. Tüm olgulara preoperatif ve postoperatif saf ses odyometrisi, otoakustik emisyon, VEMP ve VHİT testi uygulandı. Elde edilen veriler SPSS 22.0 programı kullanılarak istatistiksel olarak karşılaştırıldı. P<0.05 değeri anlamlı olarak kabul edildi.. Çalışma ve kontrol grubunda sağ ve sol kulakta preop-postop hava/kemik yolu eşikleri 250,500,1000,2000,4000,6000 ve 8000 Hz değeri anlamlı (p ˃ 0.05) farklılık göstermemiştir.. Çalışma ve kontrol grubunda sağ ve sol kulakta postop hava/kemik yolu eşik değerleri 250,500,1000,2000,4000 ve 8000 Hz' ta preop döneme göre anlamlı değişim (p ˃ 0.05) göstermemiştir. Postoperatif dönemde otoakustik emisyon, VEMP ve VHİT testi ile elde edilen sonuçlar preoperatif dönemde elde edilen sonuçlar ile istatistiksel olarak anlamlı farklılık göstermedi (p ˃ 0.05). Bu çalışmada septorinoplasti ameliyatının aşamalarından biri olan osteotominin gerek koklear gerekse vestibüler sistem üzerine belirgin bir etkisinin olmadığı sonucuna varıldı. Çalışmamızın bu konuda yapılan ilk kapsamlı prospektif kontrollü çalışma olmasından dolayı verilerimizin yapılacak yeni çalışmalarla desteklenmeye ihtiyacı vardır.

Septorhinoplasty is a commonly applied cosmetic and functional surgical procedure that's requested and accepted by the patients. Lateral and median osteotomies are among the important steps of rhinoplasty operations. The vibratory pressure produced by osteotomy that's used to close the opened roof which exists following the correction of nasal dorsum, may exert an indirect effect on auditory and vestibular systems via bone conduction. The aim of the present study is to investigate the effects of osteotomy, which is commonly used during septorhinoplasty operations, on the auditory and vestibular systems. In our study, the study group included 30 patients (14 males, 16 females) who underwent osteotomy during septorhinoplasty, and the control group included 10 patients (5 males, 5 females) who underwent nasal operations for other reasons and did not undergo osteotomy. All cases were applied the pure tone audiometry, otoacoustic emission, and the VEMP and VHIT tests pre- and post-operatively. The data were compared statistically using the SPSS 22.0 software. A p value <0.05 was accepted to be significant. The pre- and post-operative threshold values of the air/bone conduction hearings in the right and the left ears at the frequencies of 250, 500, 1000, 2000, 4000, 6000 and 8000 Hz did not differ significantly between the study and the control groups (p>0.05). In the study and the control groups, the post-operative threshold values of the air/bone conduction hearings in the right and the left ears at the frequencies of 250, 500,1000,2000,4000 and 8000 Hz did not differ significantly compared to the pre-operative values (p>0.05). The results of the otoacoustic emission, and the VEMP and VHIT tests obtained in the post-operative period did not show statistically significant differences when compared to the values assessed in the pre-operative period (p>0.05). The study results revealed that osteotomy, one of the stages of the septorhinoplasty operation, does not exert any marked effect on the cochlear and vestibular systems.
