Özet
Amaç: Bell paralizisi tedavisinde fasiyal sinir dekompresyonu (FSD)'nun zamanlaması tartışmalıdır. Travma sonrası sinir rejenerasyonunu etkileyen faktörlerden biri de intranöral fibrozistir. Bu çalışmada ratlarda Bell paralizisi modeli oluşturularak, dekompresyon zamanlaması ve antifibrotik ajan topikal mitomisin-C (MMC)'nin sinir iyileşmesi üzerindeki etkilerinin incelenmesi amaçlanmıştır. Materyal ve method: Bell paralizisi modeli, deneklerin sol fasiyal sinir trunkusu 5/0 nylon sutür ile çapı %50 azalacak şekilde bağlanarak oluşturuldu. Toplam 25 denek 5'erli 5 gruba ayrıldı. Grup 1'e dekompresyon yapılmadı. Grup 2 ve 3'teki ratların fasiyal siniri 1 hafta, Grup 4 ve 5'teki ratlarınki ise 3 hafta sonra dekomprese edildi. Grup 3 ve 5'teki ratların siniri üzerine dekompresyon sonrası topikal MMC uygulandı. Grup 1 için çalışmanın başlangıcından itibaren 3.haftada, diğer gruplar için dekompresyondan 3 hafta sonra klinik skorlama, elektrofizyolojik ve histopatolojik değerlendirme yapıldı. Bulgular: Grup 1'deki toplam klinik skor ortancası diğer gruplara göre istatistiksel anlamlı olarak düşüktü (p=0.001). Diğer gruplar arasında istatistiksel anlamlı fark saptanmadı (p>0.05). Grup 1'deki deneklerin ortalama sol (cerrahi yapılan) /sağ (sağlam) amplitüd değeri diğer gruplara kıyasla istatistiksel anlamlı düşüktü (p=0,006). Diğer gruplar arasında istatistiksel olarak anlamlı fark yoktu (p>0.05). Latans değerleri açısından gruplar arasında istatistiksel anlamlı fark izlenmedi (p=0.105). Grup 1'deki deneklerde aksonal dejenerasyonun daha şiddetli olduğu (p<0.05), diğer gruplar arasında ise istatistiksel anlamlı fark olmadığı saptandı (p>0.05). Grup 1'deki deneklerde makrovakualizasyon daha şiddetliydi. Ancak istatistiksel olarak anlamlı bulunmadı (p>0.05). Miyelin morfolojisi bozukluğu grup 1'de grup 2 ve 3'e göre istatistiksel anlamlı olarak daha şiddetliydi (p=0.008, p=0.004). Diğer gruplar arasında istatiksel anlamlı fark gözlenmedi (p>0.05). Sonuç: Dekompresyon zamanlamasının (1. ve 3.hafta) ve MMC uygulamasının sinir iyileşmesi üzerine herhangi bir etkisi saptanmadı. Çalışmamız hem ratlarda ilk kez oluşturulan Bell paralizisi modelinin geliştirilmesi, hem de Bell paralizisinde dekompresyon zamanlamasının klinik etkilerinin daha iyi anlaşılması için ilerideki çalışmalara öncü olacaktır.

Objective: The timing of facial nerve decompression (FSD) in the treatment of Bell's palsy is controversial. Intraneurial fibrosis is one of the factors affecting post-traumatic nerve regeneration. This study aimed to investigate the effects of decompression timing and topical mitomycin-C (MMC), an antifibrotic agent, on nerve regeneration in rats for which Bell's palsy model was developed. Material and method: Bell's palsy model was developed by ligating the left facial nerve trunk of the subjects with a 5/0 nylon suture so that the diameter decreased by 50%. A total of 25 subjects were divided into five groups of 5 each. Group 1 was not decompressed. The facial nerve was decompressed after one week in Groups 2-3 and three weeks in Groups 4-5. Topical MMC was applied after decompression on the nerves of the rats in groups 3-5. Clinical scoring, electrophysiological and histopathological evaluations were performed at three weeks from the beginning of the study for group 1 and three weeks after decompression for the other groups. Results: The median total clinical score in group 1 was statistically significantly lower than the other groups (p=0.001). There was no statistically significant difference between the other groups (p>0.05). The mean left (operated)/right (undamaged) amplitude values of the subjects in group 1 were statistically significantly lower compared to the other groups (p=0.006). There was no statistically significant difference between the other groups (p>0.05). There was no statistically significant difference between the groups in terms of latency values (p=0.105). It was found that axonal degeneration was more severe in the subjects in group 1 (p<0.05), but there was no statistically significant difference between the other groups (p>0.05). The subjects in Group 1 had more severe macrovacuolization. However, it was not found statistically significant (p>0.05). Myelin morphology disruption was significantly more severe in group 1 than group 2 and 3 (p=0.008, p=0.004). There was no statistically significant difference between the other groups (p>0.05). Conclusion: Decompression timing (1 or 3 weeks) and MMC application did not affect nerve regeneration. This study will guide future studies both for the development of the rat model of Bell's palsy and for a better understanding of the clinical implications of timing of decompression in Bell's palsy.
