Özet

Amaç: Kadınların erkeklere göre ağrı algılama eşiklerinin daha düşük olduğu daha önce yapılmış araştırmalarda gösterilmiştir. Kadın ve erkek arasındaki temel farklılığın hormonal farklılık olduğu bilinmektedir. Kadınlardaki hormonal farklılığın en temel göstergesi ve hormon düzeylerinin değişkenliğini yansıtan en önemli süreç ise mestüral siklüstür. Biz de bu verilerden yola çıkarak çalışmamızda sağlıklı katılımcılarda farklı menstrüel fazlarda ağrı ve dokunma algılarındaki varyasyonları inceledik. Birincil amacımız farklı gonadal hormon hakimiyeti olan dönemlerde ağrıya yanıtın değişip değişmediğini değerlendirip, hormonların ağrı algılama üzerinde olabilecek etkilerini göstermektir. Bunun yanında dokunma, titreşim ve basınç duyularında menstrüel siklus fazları arasında olan değişiklikleri de incelemeyi hedefledik. Gereç ve Yöntem : Çalışmaya, hastanemiz yerel etik kurulundan onay alındıktan sonra 25-35 yaş arası ek hastalığı ve ilaç kullanım öyküsü olmayan; Celal Bayar Üniversitesi'de araştırma görevlisi, hemşire ve tekniker olarak görev yapan gönüllülerden 72 kadın dahil edildi. Gönüllülerin en az üç aydır düzenli menstrüel siklus (26-30 gün) takibi olması şart koşuldu. Ölçümler üç ay sürdü. Bu süreçte menstrüasyon düzensizliği izlenen 12 denek çalışmadan çıkarıldı. 60 sağlıklı kadın gönüllünün verileriyle çalışma tamamlandı. Gönüllülere işlem öncesi uygulanacak testlerle ilgili sözel ve görsel bilgi verildi. Gönüllülerden yazılı rıza formu alındı. Araştırmaya dahil edilen gönüllülerin bulundukları menstrüasyon dönemleri, test ölçümlerini yapan araştırmacı haricinde bir araştırmacı tarafından kaydedildi. Bu araştırmacı ölçüm yapılma dönemleri gelen gönüllüleri çağırarak teste girmelerini sağladı. Testleri uygulayan araştırmacı, gönüllünün hangi adet döneminde olduğunu bilmeden sonuçları kaydetti. Kantitatif Duysal Testler, Anesteziyoloji ve Reanimasyon Anabilim Dalı katındaki sabit ortam sıcaklığında (22-24'C) ve bu çalıma için ayırılmış sessiz bir özel odada, aynı hekim tarafından, sağ üst ekstremitede el 68 üzerinden (C7 dermatomu) ve sağ alt ekstremitede ayak üzerinden (L5 dermatomu) uygulandı. Uygulamaya önce üst ekstremiteden başlandı sonra alt ektremiteye uygulandı. Testler her gönüllüde aynı sıra ile (aşağıda testler açıklanırken kullanılan sıra), testler aralarında 5'er dakika süre bırakılarak uygulandı. Ölçümler üç menstrüel siklus boyunca; menstrüasyon sırasında (0-4. gün), preovulatuar fazda (10-14. gün) ve geç luteal fazda (20-24. gün, prementrüasyon) olmak üzere toplamda dokuz kere tekrarlandı. Uygulanan testler; dokunma algılama eşik değeri, keskin ağrı algılama eşik değeri, künt ağrı algılama eşik değeri, titreşim algılama eşik değeri, wind-up oranı ve soğuk ağrı algılama eşik değeri ölçümleridir. Bulgular : Çalışmamızda üç ay arka arkaya toplanan verilerin istatitiği sonucunda menstrüasyon fazları arasında anlamlı bir algılama eşik değer farkı izlenmedi. Birinci ay içinde yapılan karşılaştırmalar: Ayak mekanik dokunma eşik değerleri menstrüal fazda luteal faza göre yüksek (p=0.01) izlendi. El wind-up oranı değerleri luteal fazda, menstrüel ve preovulatuar faza göre anlamlı yüksek (p<0.01) izlendi. Ayak wind-up oranı değerleri luteal fazda, menstrüel faza göre anlamlı yüksek (p<0.01) izlendi. Elde künt ağrı algılama eşik değerleri geç luteal fazda, preovulatuar faza göre anlamlı yüksek (p<0.01) izlendi. İkinci ay içinde yapılan karşılaştırmalar: Elde mekanik ağrı eşiği ölçümlerinin preovulatuar fazda, luteal faza göre anlamlı yüksek (p=0.01) olduğu görüldü. Ayakta ölçülen wind-up oranlarının luteal fazda, preovulatuar faza göre anlamlı yüksek (p<0.01) olduğu görüldü. Ayakta ölçülen titreşim eşik değerlerinin preovulatuar fazda, luteal faza göre anlamlı yüksek (p=0.01) olduğu görüldü. Elde künt ağrı algılama eşik değerlerinin luteal fazda, menstrüal ve preovulatuar faza göre anlamlı düşük (p<0.01) olduğu görüldü. Elde soğuk ağrı eşik değerlerinin preovulatuar fazda, luteal faza göre anlamlı yüksek (p=0.01) olduğu izlendi. Üçüncü ay içinde yapılan karşılaştırmalar: Elde ölçülen keskin ağrı algılama eşik değerlerinin preovulatuar fazda, menstrüal ve luteal faza göre anlamlı yüksek (p<0.01) olduğu görüldü. Ayakta mekanik dokunma eşik değerlerinin menstrüal fazda luteal faza göre yüksek olduğu görüldü. Elde mekanik dokunma eşik değerlerinin ise luteal fazda, menstrüel ve preovulatuar faza göre düşük olduğu görüldü. Sonuç: Çalışmamızın sonucunda sağlıklı gönüllülerde üç ayın verilerini analiz ettiğimizde, menstrüasyon fazları arasında, ağrı ve duyusal algı eşikleri arasında anlamlı bir fark olmadığını gördük. Fakat tek siklus boyunca aldığımız ölçüm sonuçlarının analizinde fazlar arası anlamlı algı farkları olduğunu gözlemledik.

Objective: Previous studies have shown that women have lower pain perception thresholds than men. It is known that the main difference between menand women is the hormonal difference. The most basic indicator of hormonal difference in women and the most important process that reflects the variability of hormone levels is the menstrual cycle. Based on these data, we examined the variations in pain and touch perceptions in different menstrual phases in healthy participants in our study. Based on these data, we examined the variations in pain and touch perceptions in different menstrual phases in healthy participants in our study. Our primary aim is to evaluate whether the response to pain changes during periods of different gonadal hormone dominance and to show the possible effects of hormones on pain perception. In addition, we aimed to examine the changes in the senses of touch, vibration and pressure between menstrual cycle phases. Materials and Methods: After the approval of the local ethics committee of our hospital, the patients between the ages of 25-35 who had no additional disease and no history of drug use were included in the study. Among the volunteers working as research assistants, nurses and technicians at Celal Bayar University, 72 women were included. Volunteers were required to have regular menstrual cycle (26-30 days) follow-up for at least three months. The measurements took three months. During this period, 12 subjects with menstrual irregularity were excluded from the study. The study was completed with the data of 60 healthy female volunteers. The volunteers were given verbal and visual information about the tests to be applied before the procedure. Written consent form was obtained from the volunteers. The menstrual periods of the volunteers included in the study were recorded by a researcher other than the researcher who made the test measurements. This researcher called the volunteers who were in the measurement period and had themtake the test. The tester 71 recorded the results without knowing which menstrual period the volunteer was in. Quantitative Sensory Tests were performed on the right upper extremity over the hand (C7 dermatome) and on the right lower extremity over the foot (L5 dermatome) by the same physician in a special and quiet room reserved fort he study at constant ambient temperature (22-24°C) on the floor of the Department of Anesthesiology and Reanimation. The application was started from the upper extremity first and then applied to the lower extremity. The tests were administered to each volunteer in the same order (the order used when explaining the tests below), with a 5-minute interval between the tests. Measurements (during three menstrual cycles) were repeated nine times in total, during menstruation (day 0-4), preovulatory phase (10-14 days), and late luteal phase (20-24 days, premenstruation). Applied tests; touch detection threshold, sharp pain detection threshold, blunt pain detection threshold, vibration detection threshold, wind-up ratio and cold pain detection threshold measurements. Results: As a result of the statistics of the data collected in our study, a perception threshold difference between phases was not observed for threemonths. Comparisons made within the first month: Threshold values for foot mechanical touch were observed to be higher in the menstrual phase than in the luteal phase (p=0.06). Hand wind-up ratio values were significantly higher in the luteal phase than in the menstrual and preovulatory phases (p<0.01). Foot wind-up ratio values were significantly higher in the luteal phase compared to the menstrual phase (p<0.01). The blunt pain perception threshold values in the hand were significantly higher in the late luteal phase compared to the preovulatory phase (p<0.01). Comparisons made within the second month: It was observed that mechanical pain threshold measurements in the hand were significantly higher in the preovulatory phase than in the luteal phase (p=0.03). It was observed that the standing wind-up ratios were significantly higher in the luteal phase than in the preovulatory phase (p<0.01). Vibration threshold values measured in the foot were found to be significantly higher in the preovulatory phase than in the luteal phase (p=0.02). It was observed that blunt pain perception threshold values in the hand were significantly lower in the luteal phase than in the menstrual and preovulatory phases (p<0.01). It was observed that the threshold values for cold pain in the hand were significantly higher in the preovulatory phase than in the luteal phase (p=0.02). Comparisons made within the third month: It was observed that the threshold values of sharp pain perception measured in the hand were significantly higher in the preovulatory phase than in the menstrual and luteal phases (p<0.01). It was observed that the mechanical touch threshold values in the foot were higher in the menstrual phase than in the luteal phase. It was observed that the threshold values for mechanical touch in the hand were lower in the luteal phase compared to the menstrual and preovulatory phases. Conclusion: As the result of our study, when we analyzed the data of three months in healthy volunteers, we found that there was no significant difference between the menstrual phases, pain and sensory perception thresholds. However, in the analysis of the measurement results we took during a single cycle, we observed that there were significant differences in perception between the phases.
