Özet
Koronavirüs Hastalığı 2019 (COVID-19)'un klasik semptomları arasında ateş, öksürük ve nefes darlığı gibi semptomlar görülmektedir. Bununla birlikte üst solunum yolu enfeksiyonu semptomlarından; baş ağrısı, yaygın vücut ve kas ağrıları, yorgunluk, ishal, koku ve tat kaybıyla da sık karşılaşılmaktadır. Özellikle tat ve koku kaybı gibi bazı semptomlar diğer viral hastalıklara göre COVID-19 hastalığında daha ön plana çıkmaktadır. Ancak bunun hastalığa özgü patognomonik bir semptom olup olmadığı belirsizdir. Bu çalışmada, olası/kesin COVID-19 olgularında tat ve koku kaybı gibi erken sinonazal belirtilerin klinik özelliklerinin ve prevalansının belirlenmesi amaçlandı. Ayrıca, tat ve koku değişiklerinin hastalığın şiddeti ve prognoz ile ilişkisinin ortaya konması amaçlandı. Gereç ve Yöntem Ülkemizde ilk COVID-19 vakasının görüldüğü 11 Mart 2020 tarihinden itibaren 31 Mayıs 2020 tarihine kadar planlanan bu tek merkezli, retrospektif, tanımlayıcı, kesitsel, klinik gözlemsel çalışma Sağlık Bilimleri Üniversitesi İstanbul Sultangazi Haseki Eğitim ve Araştırma Hastanesi Acil Tıp Kliniği'nde gerçekleştirildi. Çalışmaya, hastanemiz Acil Tıp Kliniği'ne 11.03.2020 ile 31.05.2020 tarihleri arasında, Sağlık Bakanlığının yayınladığı tanı kriterlerine göre olası COVID-19 bulguları ile başvuran hastalar ile tanısı moleküler yöntemlerle doğrulanan ve acil servisten ayaktan taburcu edilen veya hastaneye yatırılarak takip edilen 18 yaş üstü erişkin hastalar dahil edildi. Bu vakaların önceden oluşturulan vaka veri formuna; yaş, cinsiyet gibi demografik bilgiler, başvuru semptomları, eşlik eden hastalıklar, vital bulgular, 12 klinik, radyolojik ve laboratuvar verileri kaydedildi. Verilerin toplanmasından sonra hastalar koku ve tat değişiklikleri olan (grup I) ve olmayan olası/kesin COVID-19 olguları (grup II) şeklinde iki gruba ayrıldı. Gruplar arasında demografik veriler, komorbidite, sürüntü testi sonuçları, göğüs bilgisayarlı tomografi (BT) ve laboratuvar bulguları, hastalığın şiddeti, klinik sonlanım (sağkalım, mortalite) ve ayaktan ya da hastaneye yatırılark tedavi açısından karşılaştırma yapıldı. Bulgular Grup I'deki hastaların 69'u (%46) erkek ve 81'i (%54) kadındı, 18-29 ve 30- 39 yaş aralığındaki hasta sayısı 93 (%62) ile yüksek saptandı. Grup II'deki hastaların 78'i (%52) erkek ve 72'si (%48) kadındı, 40-49 ve 50-59 yaş aralığındaki hasta sayısı 81 (%54) ile yüksek saptandı. İki grup arasında cinsiyet açısından istatistiksel olarak anlamlı fark saptanmadı. Grupların yaş aralığında ise istatistiksel olarak anlamlı fark vardı (p<0,001). Grup I'de 40 yaş altı hasta sayısı anlamlı yüksek bulundu ve ek olarak 80 yaş üzeri hasta yoktu. Grup I'deki hastaların yaş ortalaması 36,6±12,5 olup median yaş 35, Grup II'deki hastaların yaş ortalaması 48,5±13,5 olup median yaş 48 olarak saptandı. Gruplar arasındaki ayaktan ve hastaneye yatarak tedavi oranları karşılaştırıldığında Grup I'de ayaktan tedavi alan hasta sayısı 117 (%78), yatarak tedavi alan hasta sayısı 33 (%22), Grup II'de ise ayaktan tedavi alan hasta sayısı 54 (%36), yatarak tedavi alan hasta sayısı 96 (%64) olarak saptandı. Grup I'deki hastaların ayaktan tedavi alma oranında istatistiksel olarak anlamlı fark vardı (p<0,001). Grup I' de hafif hastalık kliniğinde olan hasta sayısı 130 (%86,7), şiddetli hastalık kliniğinde olan hasta sayısı 20 (%13,3) olarak saptandı ve kritik hastalık kliniğinde olan hasta yoktu. Grup II'de ise hafif hastalık kliniğindeki hasta sayısı 59 (%39,3), şiddetli hastalık kliniğindeki hasta sayısı 80 (%53,3) ve kritik hastalık kliniğinde olan hasta sayısı 11 (%7,3) olarak saptandı. Grupların hastalık şiddeti karşılaştırıldığında istatistiksel olarak anlamlı fark vardı (p<0,001). Grup I'de hafif şiddetli hasta oranı yüksekti. Gruplar arasındaki mortalite oranları incelendiğinde Grup I'de exitus ile sonuçlanan klinik sonlanım yoktu, Grup II'de 13 ise 7 (%4,7) hastanın kliniğinin exitus ile sonlandığı saptandı. Grup II'de mortalite oranı Grup I'e göre istatistiksel olarak anlamlı yüksekti (p=0,015). Çalışmaya dahil edilen vakaların acil servise ilk başvurusunda yapılan PCR test sonuçları karşılaştırıldığında, Grup I'deki hastaların %49,7'sinin (n=74), Grup II'deki hastaların %52'sinin (n=78) verdiği ilk test sonuçlarının pozitif olduğu görüldü. Gruplar arasında 1. PCR test sonuçları açısından anlamlı bir fark saptanmadı (p=0,686). Yapılan 2. PCR Test sonuçlarında ise Grup I'in %9,3'ü (n=9), Grup II'nin ise %9,5'inin (n=8) test sonucu pozitifti. Benzer şekilde gruplar arasında 2. PCR test sonuçları açısından anlamlı bir fark saptanmadı (p=0,955). Gruplar BT bulguları açısından karşılaştırıldığında Grup I'deki hastaların %62,9'unda (n=88), Grup II'deki hastaların %96,7'sinde (n=145) BT bulgusu vardı ve Grup II'nin BT tutulumu istatistiksel olarak anlamlı yüksek saptandı (p<0,001). Her iki grupta da en sık görülen BT bulgusu Çift Taraflı Buzlu Cam Görünümü'ydü. Grup II'de BT'de görülen konsolidasyon bulgusu Grup I'e göre istatistiksel olarak anlamlı yüksek saptandı. Tomografideki lezyonları yerleşim yerine bakılacak olursa en sık görülen radyolojik dağılım şekli Grup I'de %20 (n=28), Grup II'de %40 (n=60) oranla Periferik Yerleşim olarak tespit edildi. Radyolojik dağılıma bakıldığında Dağınık Yerleşim Grup I'de, Periferik Yerleşim ve Diffüz Dağılım Grup II'de istatistiksel olarak anlamlı yüksek bulundu. Grup I'deki hastalarla kıyıslandığında grup II'deki hastalarda ortalama serum Lenfosit (Leu) ve Albumin (Alb) değerleri istatistiksel olarak anlamlı düşük saptandı (her iki karşılaştırma için, p<0,001). Ayrıca, Grup I'deki hastalarla kıyıslandığında, grup II'deki hastalarda ortalama Nötrofil Lenfosit Oranı (NLO) ve C-Reaktif Protein/Albümin (CRP) değerleri istatistiksel olarak anlamlı yüksek idi (her iki karşılaştırma için, p<0,001). Sonuç Çalışmada elde edilen veriler doğrultusunda, olası/kesin COVID-19 vakalarında tat ve koku bozukluğu olan ve olmayan hasta grupları arasında cinsiyet açısından anlamlı fark bulunmazken, tat ve koku kaybının genç yaş grubunda daha 14 çok görüldüğü söylenebilir. Ayrıca tat ve koku kaybı olmayan hastalarla karşılaştırıldığında, tat ve koku kaybı olan hasta grubunda BT bulgularının sıklığı, yerleşimi ve dağılımı açısısından daha iyi seyirli olduğu gözlemlendi. Ek olarak klinik sonlanım ve hastalığın şiddetinin tat ve koku kaybı olmayan hastalarda daha ağır ve kötü olduğu sonucuna varılmıştır. The classic symptoms of the disease include fever, cough, and shortness of breath. However, from upper respiratory tract infection symptoms; Headache, generalized body and muscle aches, fatigue, diarrhea, loss of smell and taste are also frequently encountered. In particular, some symptoms such as loss of taste and smell are more prominent in COVID-19 disease than other viral diseases. However, it is unclear whether this is a disease-specific pathognomonic symptom. In this study, it was aimed to determine the clinical features and prevalence of early sinonasal symptoms such as loss of taste and smell in probable/definite COVID-19 cases. In addition, it was aimed to reveal the relationship between taste and smell changes and the severity of the disease and prognosis. Materials and Methods This single-center, retrospective, descriptive, cross-sectional, clinical observational study, planned from March 11, 2020, when the first case of COVID-19 was seen in our country, until May 31, 2020, was carried out at the Health Sciences University Istanbul Sultangazi Haseki Training and Research Hospital Emergency Medicine Clinic. The patients who applied to the Emergency Medicine Clinic of our hospital between 11.03.2020 and 31.05.2020 with possible COVID-19 findings according to the diagnostic criteria published by the Ministry of Health, and 18 years of age, whose diagnosis was confirmed by molecular methods and who were discharged from the emergency department or were followed up in the hospital. Adult patients were included. The pre-created case data form of these cases; Demographic information such as age, gender, admission symptoms, co-morbidities, vital signs, clinical, radiological and laboratory data were recorded. After data collection, patients were divided into two groups: those with smell and taste changes (group I) and probable/definite cases of COVID-19 without (group II). Comparisons were made between the groups in terms of demographic data, comorbidity, swab test results, chest computed tomography (CT) and laboratory findings, disease severity, clinical outcome (survival, mortality), and outpatient or hospitalization treatment. Results Of the patients in Group I, 69 (46%) were male and 81 (54%) were female, the number of patients aged 18-29 and 30-39 was found to be high with 93 (62%). Of the patients in Group II, 78 (52%) were male and 72 (48%) were female, the number of patients aged 40-49 and 50-59 was found to be high with 81 (54%). There was no statistically significant difference between the two groups in terms of gender. There was a statistically significant difference in the age range of the groups (p<0.001). The number of patients under the age of 40 was found to be significantly higher in Group I, and there was no additional patient over the age of 80. The mean age of the patients in Group I was 36.6±12.5, and the median age was 35, and the mean age of the patients in Group II was 48.5±13.5, with a median age of 48. When the outpatient and inpatient treatment rates between the groups were compared, the number of outpatients in Group I was 117 (78%), the number of inpatients was 33 (22%), and the number of outpatients in Group II was 54 (36%). The number of patients receiving inpatient treatment was 96 (64%). There was a statistically significant difference in the rate of receiving outpatient treatment in Group I patients (p<0.001). In Group I, the number of patients in the mild disease clinic was 130 (86.7%), the number of patients in the severe disease clinic was 20 (13.3%), and there was no patient in the critical illness clinic. In Group II, the number of patients in the mild disease clinic was 59 (39.3%), the number of patients in the severe disease clinic was 80 (53.3%), and the number of patients in the critical illness clinic was 11 (7.3%). When the disease severity of the groups was compared, there was a statistically significant difference (p<0.001). Mild severity patient rate was high in Group I. When the mortality rates between the groups were examined, it was found that there was no clinical outcome resulting in exitus in Group I, whereas 7 (4.7%) patients in Group II ended in death. The exitus rate of Group II was statistically significantly higher than Group I (p=0.015). When the PCR test results of the cases included in the study at the first admission to the emergency department were compared, the first test results of 49.7% (n=74) of the patients in Group I and 52% (n=78) of the patients in Group II were positive appeared to be. There was no significant difference between the groups in terms of the 1st PCR test results (p=0.686). In the 2nd PCR test results, 9.3% (n=9) of Group I and 9.5% (n=8) of Group II were positive. Similarly, no significant difference was found between the groups in terms of the 2nd PCR test results (p=0.955). When the groups were compared in terms of CT findings, 62.9% (n=88) of the patients in Group I and 96.7% (n=145) of the patients in Group II had CT findings and the CT involvement of Group II was statistically significant significantly higher (p<0.001). The most common CT finding in both groups was Bilateral Ground Glass View. The consolidation finding on CT in Group II was found to be statistically significantly higher than in Group I. Considering the location of the lesions on tomography, the most common radiological distribution type was Peripheral Localization with a rate of 20% (n=28) in Group I and 40% (n=60) in Group II. When the radiological distribution was examined, it was found that Scattered Localization was found to be statistically higher in Group I, and Peripheral and Diffuse Distribution in Group II. When compared with patients in group I, mean serum Lymphocyte (Leu) and Albumin (Alb) values were found to be statistically significantly lower in patients in group II (for both comparisons, p<0.001). Also, when compared with patients in Group I, mean Neutrophil Lymphocyte Ratio (NLR) and C-Reactive Protein/Albumin (CRP) values were statistically significantly higher in patients in group II (p<0.001 for both comparisons). Conclusion In line with the data obtained in the study, it can be said that while there was no significant difference in terms of gender between the patient groups with and without taste and smell disorders in probable/definite COVID-19 cases, it can be said that the loss of taste and smell is more common in the younger age group. In addition, it was observed that the patient group with taste and smell loss had a better prognosis in terms of frequency, location and distribution of CT findings when compared to patients without taste and smell loss. In addition, it was concluded that the clinical outcome and severity of the disease were more severe and worse in patients without taste and smell loss.
