Özet
ABSTRACT THE EFFECT OF ENDOTRACHEAL SALINE INSTILLATION PRIOR TO SUCTIONING TO THE LOWER AIRWAY CONTAMINATION Hatice MERT This investigation was managed quasi-experimentally in order to determine whether the number of microorganisms passing to lower airways is more during aspiration following saline instillation, compared with no saline instillation. The sample group of the investigation was composed of 28 patients who stayed in the Medical Intensive Care and Anesthesiology Intensive Care Units of Dokuz Eylül University Research and Practice Hospital between October 1997 and May 1998, and who are over 18 years old, and applied an endotracheal tube minimum 48 hrs. » Intensive Care Unit nurses were reminded to inform the investigator about the patients extubated. The investigator had got the endotracheal tubes maximum in thirty minutes. By changing the turn of procedures, 5 ml saline and suction catheter are applied to the obtained endotracheal tubes. Samples taken were studied in a microbiology laboratory. The proliferated microorganism type was determined and the colony count was calculated. The data was evaluated by using percentage, Wilcoxon Matched Two Sample Test and Mann- Whitney U test. In the samples taken after the procedures applied to endotracheal tubes, 32.1 percent Acinetobacter and 28.6 percent Pseudomonas aeroginosa was seen, In the samples taken by means of passing suction catheter through endotracheal tubes, colony count average was 2.27 ± 1.55; while it was 3.50 ± 1.87 in the samples taken with saline. The difference between two procedures is significant statistically (p= 0.0001). The turn ofprocedures applied to the endotracheal tubes were found to be statistically insignificant (p 0.61, p = 0.83). According to the results of the investigation, some suggestions were made.
