Özet
Amaç: Covid-19'un tipik klinik özellikleri ateş, öksürük, halsizlik ve solunum yetmezliği olsa da, SARS-CoV-2 enfeksiyonu sinir sistemini etkileyerek değişken nörolojik tablolar oluşturmaktadır. Bu çalışma, COVID-19 hastalığında pozitif ve negatif dönemlerde odyovestibüler bulguların şiddetini ölçeklerle değerlendirmeyi hedeflemiştir. Gereç-Yöntem: Çalışma, Ekim-Kasım 2020 tarihleri arasında Hakkâri Devlet Hastanesi'nde, PCR test sonucu pozitif olan bireyler ile gerçekleştirilmiştir. Çalışmaya 18–50 yaş aralığında (30,50±7,63), randomize 60 (30 kadın, 30 erkek) hasta dahil edilmiştir. Bireyler“Yetişkinler için İşitme Engeli Ölçeği”,“Vertigo Semptom Skalası”,“Vertigo, Dizziness Dengesizlik Belirti Ölçeği”,“Tinnitus Engellilik Anketi”ve“Khalfa Hiperakuzi Ölçeği”ile PCR testi pozitif dönemde ve negatif çıktıktan 15 gün sonra değerlendirilmiştir. Bulgular: Hastalarda pozitif dönemde 30/60 (%50) baş dönmesi, 17/60 (%28,3) hiperakuzi, 11/60 (%18,3) tinnitus, 8/60 (%13,3) işitme kaybı görülmüştür. Vertigo Semptom Skalası, Vertigo Dizziness Dengesizlik Ölçeği ve Khalfa Hiperakuzi Ölçeği bulgularında patolojik grupta ve genel örneklemde pozitif ve negatif dönemler arasında anlamlı bir fark bulunmuştur. (p<0,05) Ancak Yetişkinler İçin İşitme Engeli Ölçeği ve Tinnitus Engellilik Anketi bulgularında genel örneklemde pozitif ve negatif dönemler arasında anlamlı bir fark bulunmazken patolojik grupta anlamlı fark bulunmuştur. (p>0,05) Sonuç: COVID-19 literatürüyle uyumlu olarak gözlemsel ve kesitsel örneklemimizde, sıklık sırasıyla baş dönmesi, hiperakuzi, tinnitus ve işitme kaybı odyovestibüler bulgular görülmüştür. Negatif dönemde de bu bulgular pozitif döneme göre azalmakla birlikte devam etmektedir. Gelecekteki çalışmalarda, daha fazla sayıda hasta, objektif tanı testleri ve daha uzun süre takip ile literatüre katkı sağlanabilir.

Aim: Although the typical clinical features of Covid-19 are fever, cough, malaise and respiratory failure, SARS-CoV-2 infection affects the nervous system and creates variable neurological pictures. In this study, it was aimed to evaluate the severity of audiovestibular findings with scales in positive and negative periods in COVID-19 disease. Materials and Methods: The study was carried out between October 2020 and November 2020 in Hakkari State Hospital with positive PCR test results. Sixty randomized patients (30 females, 30 males) between the ages of 18-50 (30.50 ± 7.63) were included in the study. In the positive period and 15 days after the test, the individuals were evaluated using the“Adult Hearing Disability Scale”,“Vertigo Symptom Scale”,“Vertigo, Dizziness Imbalance Symptom Scale”,“Tinnitus Handicap Inventory”and“Khalfa Hyperacusis Scale”. Results: In the positive period, 30/60 (50%) dizziness, 17/60 (28.3%) hyperacusis, 11/60 (18.3%) tinnitus, 8/60 (13.3%) hearing loss were observed in the patients. In the findings of the Vertigo Symptom Scale, Vertigo Dizziness Imbalance Scale and Khalfa Hyperacusis Scale, a significant difference was found between the positive and negative periods in the pathological group and the general sample. (p <0.05) However, there was no significant difference between the positive and negative periods in the general sample in the findings of the Adult Hearing Impairment Scale and Tinnitus Handicap Inventory, while a significant difference was found in the pathological group. (p> 0.05) Conclusion: In accordance with the COVID-19 literature, in our observational and cross-sectional sample, dizziness, hyperacusis, tinnitus, and hearing loss audiovestibular findings were observed, respectively. Although these findings decrease in the negative period compared to the positive period, they continue. More patients, objective diagnostic tests and longer follow-up may contribute to the literature in future studies.
