Özet

GİRİŞ ve AMAÇ: Kostik (koroziv) madde, dokuya temas ettiğinde hasar oluşturma potansiyeline sahip kimyasal maddedir. Alkali (pH>11.5) ya da Asit (pH<2) maddelerin kazara içimi, yanaklar, ağız, orofarinks, özofagus, mide ve nadiren de duodenumda ciddi yanıklara sebep olabilir. Çalışmamızın amacı koroziv içimi sebebiyle başvuran pediatrik yaş grubu hastaların demografik verilerini değerlendirmek ve bu olgularda diagnostik özofagoskopi yapılmasının gerekli olup olmadığını tartışmaktır. GEREÇ VE YÖNTEM: 2010-2020 yılları arasında koroziv içimi veya içme şüphesiyle çocuk acil servise başvurup çocuk cerrahisine danışılan ve yatış verilen 0-18 yaş aralığındaki 230 hasta çalışmaya alındı. Hastaların dosyaları geriye dönük olarak incelendi. Tüm hastalar demografik özellikleri, semptomları, fizik muayene bulguları, alınan koroziv maddenin cinsi, miktarı, yalnız olup olmamaları, yanık dereceleri, ÖMD bulguları, dilatasyon ihtiyacı, süresi açısından istatiksel olarak değerlendirildi. Endoskopisinde yanık görülen ve yanık görülmeyen hastaların alınan koroziv maddenin türü, miktarı, kusma durumu, semptomları, fizik muayene bulguları, oral beslenmeyi tolere etme süresi ve yatış süresi açısından istatiksel olarak karşılaştırması yapıldı. Konservatif izlenen ve endoskopi yapılarak izlenen hastalar semptomları, fizik muayene bulguları, alınan koroziv maddenin cinsi, oral beslenmeyi tolere etme süreleri, toplam yatış süreleri ve dilatasyon ihtiyacı açısından karşılaştırıldı. BULGULAR: Çalışmaya dahil edilen 230 hastanın 102'si (%44) kız, 128'i (%56) erkek idi. Hastaların semptom ve bulguları incelendiğinde, 120 hastanın (%52,1) herhangi bir semptomu olmadığı, 94 hastanın (%40,9) fizik muayene bulgularının tamamen normal olduğu, 55 hastanın ise (%23,9) herhangi bir semptom ya da patolojik fizik muayene bulgusu olmadığı görüldü. Başvuru sırasında en sık görülen semptom kusma (%33,6), en sık görülen fizik muayene bulgusu orofarinkste hiperemi idi (%30,8). 230 hastadan 168'ine diagnostik özofagoskopi yapılırken (%73), 62 hasta oral kapalı iv. idame mayi ile takip edildi (%27). Özofagoskopi yapılan hastalar Zargar endoskopik yanık evrelemesine göre değerlendirildiğinde, 51 hastada yanık olmadığı (Grade 0) (%30,4), 23 hastada Grade 1 (%13,7), 57 hastada Grade 2A (%33,9), 33 hastada Grade 2B (%19,6), 4 hastada Grade 3A (%2,4) yanık olduğu görüldü. Çalışma grubundaki 230 hastanın 17'sinin (%7) dilatasyon ihtiyacı oldu. İçilen maddenin asit ya da alkali oluşu ile yanık oluşturma durumu arasında iii anlamlı fark bulunmadı (p:0.799). Koroziv maddeden çok miktarda içtiği görülen hastaların %92'sinde yanık tespit edilirken, koroziv maddeyi içip içmediği net olarak bilinmeyen hastaların %31'inde özofagusta yanık tespit edildi (p:0.001). Çalışmamızdaki hastaların fizik muayene bulguları ile yanık durumu arasındaki ilişki incelendi ve istatistiksel olarak anlamlı ilişki saptandı (p:0.006). Fizik muayenesi normal olan hastaların %56'sında özofagoskopide yanık saptanırken, orofarenksinde lezyon saptanan hastaların %82'sinde yanık saptandı. Orofarenks lezyonu olanlarda fizik muayenesi normal olanlara göre anlamlı şekilde daha fazla yanık saptandı (p:0,003). Hastaların kusma durumu ile özofagoskopide yanık olup olmaması arasında anlamlı ilişki saptanmadı (p:0,244). Hastaların salya durumu ile özofagoskopide yanık olup olmaması arasında anlamlı ilişki saptanmadı (p:0,180). Ancak hastalar fizik muayene bulguları (normal veya patolojik) ve semptomlarına (var veya yok) göre gruplandıktan sonra fizik muayene bulguları ve semptomlar birlikte değerlendirildiğinde, yanık durumu ile arasında anlamlı ilişki saptandı (p:0.033). Buna göre hem fizik muayenesi normal olan hem de asemptomatik olan hastaların %53'ünde özofagoskopide yanık tespit edildi. Hem fizik muayenesi patolojik olan hem de semptomu bulunan hastalarınsa %21'inde özofagoskopide yanık olmadığı görüldü. SONUÇ: Gelişmekte olan ülkelerde kostik konsantrasyonu bilinmeyen ve açık şişelerde satılan ürünler özellikle 5 yaşın altındaki çocuklar için yüksek risk oluşturmaktadır. Koroziv içimi sonrası tanısal özofagoskopi gerekliliği halen tartışma konusudur. Tanısal özofagoskopi, koroziv içimi sonrası en korkulan komplikasyonlardan olan striktür gelişimini engellememektedir. Koroziv içimi öyküsü şüpheli olan, fizik muayene bulguları normal olan, tükürüğünü yutabilen, sonradan gelişebilecek yutma güçlüğü durumunda hastaneye başvurma imkanı olan hastaların konservatif takip edilmesi hastaların büyük çoğunluğunu gereksiz endoskopiden ve anestezi riskinden koruyacaktır. Aynı şekilde içilen maddenin miktarının çok olduğu bilinen (özellikle kasıtlı içilen) durumlarda hasta semptomatikse ve fizik muayene bulguları mevcut ise tanısal endoskopi yapılması, ikinci değerlendirmeye kadarki süreçte gerekli medikal tedavi desteğinin başlanmasına olanak sağlayacaktır.

INTRODUCTION and OBJECTIVE: Caustic (corrosive) substance is a chemical substance that has the potential to cause damage when it comes into contact with tissue. Accidental ingestion of alkali (pH> 11.5) or acid (pH <2) substances can cause severe burns in the cheeks, mouth, oropharynx, esophagus, stomach and rarely in the duodenum. The aim of our study is to evaluate the demographic data of pediatric patients admitted due to corrosive ingestion and to discuss whether diagnostic esophagoscopy is required in these cases. MATERIALS AND METHODS: 230 patients between the ages of 0-18 who were admitted to the pediatric emergency department and then hospitalized in the pediatric surgery department for corrosive ingestion between 2010 and 2020 evaluated. The files of the patients were analyzed retrospectively. Patients who had no burn and patietns who had any grade of burn compered statisticaly in terms of the type and the amount of the corrosive substance taken, symptomps and physical examination findings, when the feeding started and the length of hospital stay. Patients who had undergone eosophagoscopy and the patients who were observed conservatively were compared in terms of their symptoms, physical examination findings, the type of corrosive substance taken, the time of oral feeding, the total length of stay and the need for dilatation. RESULTS: Of the 230 patients included in the study, 102 (44%) were female and 128 (56%) were male. When the symptoms and signs of the patients were examined, 120 patients (52.1%) were asymptomatic, 94 patients (40.9%) had normal physical examination findings, and 55 patients (23.9%) had no symptoms or pathological physical findings. The most common symptom at admission was vomiting (33.6%), and the most common physical examination finding was hyperemia in the oropharynx (30.8%). While diagnostic esophagoscopy was performed in 168 of 230 patients (73%), 62 patients were followed up conservatively with intravenous maintenance fluid (27%). The patients who had undergone esophagoscopy were evaluated according to Zargar's endoscopic burn grading: 51 patients had no burns (Grade 0) (30.4%), 23 patients had Grade 1 (13.7%), 57 patients had Grade 2A (33.9%), 33 patients had Grade 2B (19.6%), 4 patients had Grade 3A (2.4%) burns. Seventeen (7%) of 230 patients in the study group needed dilatation. The presence of caustic injury in v the eosophagus was not statisticaly different between acid and alkaline ingestion groups (p: 0.799). The presence of caustic injury in the eosophagus was statisticaly different between the groups of large amount of substance ingestion group and the suspicious ingestion group (p: 0.001). Of the patients who ingested a large amount of corrosive substance, 92% had corrosive burns and of the patients who had suspicious ingestion 31% had corrosive burns in the esophagus. The relationship between physical examination findings and the presence of caustic burn was investigated and a statistically significant relationship was found (p: 0.006). While 56% of the patients with normal physical examination had burns, 82% of the patients with lesions in the oropharynx were found to have caustic burns. Patients with oropharynx lesions had significantly more burn ratios than those with normal physical examination (p: 0.003). There was no significant relationship between the vomiting and the presence of caustic burns on esophagoscopy (p: 0.244). There was no significant relationship between hipersalivation and the presence of caustic burn on esophagoscopy (p: 0.180). However, when the patients were grouped according to physical examination findings (normal or pathological) and symptoms (symptomatic or asymptomatic), and when the physical examination findings and symptoms were evaluated together, a significant relationship was found between the presence of caustic burn (p: 0.033). Accordingly, in 53% of the patients with both normal physical examination and asymptomatic, caustic injury was present in esophagoscopy. It was observed that 21% of the patients whose physical examination was pathological and who were symptomatic did not have caustic burns in esophagoscopy. CONCLUSION: Products sold in open bottles with unknown caustic concentrations in developing countries pose a high risk, especially for children under 5 years of age. The need for diagnostic esophagoscopy after corrosive ingestion is still a matter of debate. Diagnostic esophagoscopy does not prevent stricture development, which is one of the most feared complications after corrosive ingestion. Conservative follow-up of patients with suspicious history of corrosive ingestion, normal physical examination findings, patients who can swallow their saliva, and patients who has the opportunity to reach to the hospital in case of swallowing difficulty, will protect the majority of patients from unnecessary endoscopy and anesthesia risk. Similarly, if the patient is symptomatic and physical examination findings are present in cases where the amount of the substance ingested is known to be a lot (especially intentional ingestion), performing diagnostic endoscopy will allow the necessary medical treatment support to be initiated in the process until the second evaluation.
