Özet
Amaç: Aksiller lenf nodu metastazı olan meme kanseri hastalarında Neoadjuvan Kemoterapi (NAK) sonrası aksillanın yönetimi tartışmalıdır. NAK sonrası axillada tam yanıt olan hastalarda aksiller lenf nodu diseksiyonunun (ALND) morbiditesinden kaçınmak için mumkunse sentinel lenf nodu biyopsisi (SLNB) yapılması önerilmektedir. Bu yüzden NAK sonrası görüntüleme yöntemlerinin aksiller lenf nodu metastastazını belirlemedeki etkinliği önem kazanmaktadır. Bu çalışmanın amacı aksiller lenf nodu metastazı olup NAK alan hastalarda, NAK sonrası Ultrasonografi (USG), Magnetic Rezonans Görüntüleme (MR) ve F-18 Florodeoksiglukoz Pozitron Emisyon Tomografisi(PET BT) nin aksiller lenf nodu metastazı belirlemedeki etkinliğini araştırmaktır. Gereç ve yöntemler: Bu çalışmaya Saglik Bilimleri Universitesi, Adana Şehir Araştırma ve Eğitim Hastanesi Genel cerrahi Kliniğinde, aksiller lenf nodu metastazı nedeniyle NAK alıp, NAK sonrası USG, MR, PET BT ile aksiller değerlendirme yapılan ve ardından opere edilen tüm meme kanseri hastaları dahil edilmistir. Hastaların demografik bilgileri, klinikopatolojik parametreleri, NAK rejimi ve uygulanan operasyon çeşidi kaydedildi. NAK sonrası yapılan USG, MR ve PET BT deki kemoterapiye aksiller yanıt durumu, operasyon sonrası axiller lenf nodu histopatolojik sonucu ile karşılaştırılmıştır. Bulgular: Çalışmaya toplam 171 kadın hasta dahil edildi. Hastaların yaş ortalaması 53,28±10,62 yıl idi. NAK sonrası yapılan değerlendirmede, aksiller lenf nodu metastazını tespit etmede USG'nin sensitivitesi %59,42, spesifitesi %82,35, pozitif prediktif değeri (PPD) %82, negatif prediktif değeri (NPD) %60 idi, MR'ın aksiller lenf nodu metastazını tespit etmede sensitivitesi %36,67, spesifitesi %77,78, PPD'i %73,33, NPD'i %42,42 idi. PET BT'nin ise sensitivitesi %47,50, spesifitesi %76,67, PPD'i %73,08, NPD'i %52,27 olarak bulundu. Bu üç görüntüleme yönteminin ikili kombinasyonaları değerlendirildiğinde ise USG ve PET BT nin kombine kullaniminin spesifitesi ve PPD'i %100 olarak bulundu. Sonuç: Calismamizin sonuclarina gore NAKT sonrası axiller lenf nodu metastazını tespit etmede, USG en yüksek sensivite ve spesifiteye sahiptir. Ayrıca NAK sonrasi USG ve PET BT ile yapılan değerlendirmede her iki tetkikte aynı anda lenf nodu metastazını isaret ediyorsa bu hastalara SLNB yerine ALND, bu iki tetkikte aynı anda tam yanıt var ise SLNB tercih edilebilir. Her ne kadar calismamizin sonuclari cesaretlendirici ise de bu konuda ek klinik calismalara ihtiyac vardir.

Aim: Management of the axilla after neoadjuvant chemotherapy (NAC) in breast cancer patients with axillary lymph node metastasis is controversial. If feasible, sentinel lymph node biopsy (SLNB) is recommended for patients with a complete axillary response after NAC to avoid the morbidity of axillary lymph node dissection (ALND). Therefore, the efficacy of post-NAC imaging techniques for detecting axillary lymph node metastasis is of importance. The aim of this study was to investigate the efficacy of post-NAC ultrasound (US), magnetic resonance imaging (MRI), and F-18 fluorodeoxyglucose positron emission tomography (F-18 FDG-PET/CT) for detecting post-NAC axillary lymph node metastasis in patients who had axillary lymph node metastasis at the time of diagnosis. Materials and Methods: This study included all breast cancer patients who received NAC for axillary lymph node metastasis, underwent axillary assessment with US, MRI or F18-FDG-PET/CT, and then were operated on in the General Surgery Clinic, University of Health Sciences, Adana City Research and Training Hospital, Turkey. Patients' data was recorded including demographic data, clinicopathological parameters, NAC regimens, and operation types. The axillary response to chemotherapy on post-NAC US, MRI, and F-18 FDG-PET/CT was compared with the postoperative histopathological result of the axillary lymph node. Results: The study included a total of 171 female patients. The mean age of the patients was 53.28±10.62 years. The post-NAC assessment revealed that the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of US for detecting axillary lymph node metastasis were 59.42%, 82.35%, 82.00%, and 60.00%, respectively, while the same measures regarding MRI for detecting axillary lymph node metastasis were 36.67%, 77.78%, 73.33%, and 42.42%, respectively. The sensitivity, specificity, PPV, and NPV of F-18 FDG-PET/CT were 47.50%, 76.67%, 73.08%, and 52.27%, respectively. The evaluation of dual combinations of these three imaging techniques showed that the specificity and PPV of the combined use of US and F-18 FDG-PET/CT was 100%. Conclusion: The results showed that US has the highest sensitivity and specificity for detecting axillary lymph node metastasis after NAC. Furthermore, ALND may be preferred for these patients instead of SLNB if both examinations simultaneously indicate lymph node metastasis in the post-NAC assessment with US and F-18 FDG-PET/CT. SLNB may be preferred if these two examinations simultaneously show a complete response. Although the results are encouraging, there is a need for further clinical studies on the matter.
