Özet
AMAÇ: Böbrek nakli SDBY (son dönem böbrek yetmezliği)'nin en üstün tedavi yöntemidir. Allograft sağ kalımını etkileyen birçok faktör olup çalışmamızda alıcıların demografik verileri, KBY (kronik böbrek yetmezliği) primer etiyolojisi, nakil öncesi HD (hemodiyaliz)/PD (periton diyalizi) tedavileri veya preemptif nakil yapılması, gelişen komplikasyonlar ve immünsüpresif tedavilerin greft sağ kalımına etkisini incelemeyi amaçladık. GEREÇ VE YÖNTEM: Çalışmamız Nefroloji Kliniği Transplantasyon Polikliniği'ne 01 Eylül 2020 ve 01 Kasım 2020 tarihleri arasında takip ve tedavi devamı nedeniyle başvuran 126 böbrek nakilli hasta dahil edilerek retrospektif olarak gerçekleştirilmiştir. BULGULAR: Hastaların kronik böbrek yetmezliği primer etiyolojileri 33 (%26,2) hipertansiyon, 23 (%18,3) postrenal olaylar, 17 (%13,5) glomerülonefrit, 15 (%11,9) diabetes mellitus, 5 (%4) tekrarlayan idrar yolu enfeksiyonu, 4 (%3,2) polikistik böbrek hastalığı, 4 (%3,2) Ailevi Akdeniz Ateşi amiloidozu, 1 (%0,8) Fabry hastalığı ve 24 (%19) nedeni bilinmeyen böbrek yetmezliği şeklindedir. Hastalarımız canlıkadavra vericisi olanlar ve GFH (glomerüler filtrasyon hızı) <60 ml/dk/1.73 m2 ve GFH≥ 60 ml/dk/1.73 m2 olanlar olarak gruplara ayrılmıştır. Hastaların yaş ortalaması 47,1 ± 10,8 yıl olup canlı-kadavra vericisi olan gruplar arasında istatistiksel olarak anlamlı fark saptanmıştır (p=0,033). Hastaların ortalama HD ve PD süresi sırasıyla 41,5 ± 55,6 ve 6,9 ± 21,8 ay olup ortalama HD süresinde canlı-kadavra vericisi olan gruplar arasında istatistiksel olarak anlamı fark mevcuttur (p=0,0007), fakat ortalama PD süresi için gruplar arasında anlamlı fark saptanmamıştır (p=0,175). GFH<60 ml/dk/1.73 m2 ve GFH≥ 60 ml/dk/1.73 m2 grupları arasında HD süresi açısından istatistiksel fark saptanmıştır (p=0,024) fakat PD süresi açısından istatistiksel fark saptanmamıştır (p=0,871). Gecikmiş greft fonksiyonu 18 (%14,3) hastada mevcut olup canlı-kadavra vericisi olan gruplar arasında istatistiksel olarak anlamlı fark saptanmıştır (p=0,046) ve GFH<60 ml/dk/1.73 m2 - GFH≥ 60 ml/dk/1.73 m2 grupları arasında istastiksel fark saptanmamıştır (p=0,443). Akut humoral rejeksiyon, akut hücresel rejeksiyon, kronik rejeksiyon ve KAN (kronik allograft nefropatisi) açısından canlı-kadavra vericisi olan gruplar arasında istatistiksel olarak anlamlı fark saptanmamıştır (p=0,585, p=0,140, p=0,517, p=0,552). Canlı vericiden 90 (%71,4) ve kadavra vericiden 36 (%28,6) hastaya nakil yapılmış olup GFH<60 ml/dk/1.73 m2 ve GFH≥ 60 ml/dk/1.73 m2 grupları arasında canlı-kadavra verici açısından istatistiksel olarak anlamlı fark saptanmamıştır (p=0,321, p>0,999). Akut humoral rejeksiyon 10 (%7,9), kronik rejeksiyon 13 (%10,3) ve KAN 15 (%11,9) hastada görülmüş olup GFH<60 ml/dk/1.73 m2 ve GFH≥ 60 ml/dk/1.73 m2 grupları arasında istatistiksel olarak anlamlı fark saptanmışken (p=0,002, p=0,037, p=0,012) akut hücresel rejeksiyon 5 hastada görülmüş ve gruplar arasında istatistiksel olarak anlamlı fark saptanmamıştır (p=0,065). SONUÇ: Çalışmamızda etiyolojide HT olması, nakil öncesi HD/PD süresi, retransplantasyon öyküsü, rejeksiyon atakları, özellikle akut hücresel rejeksiyon ve kronik rejeksiyon, KAN gelişmesi, cerrahi komplikasyonlar özellikle lenfosel olması, nakil sonrası malignite saptanması, class 1 ve class 2 PRA pozitifliğinin greft survisini olumsuz etkilediği saptanmıştır. Güncel çalışmalar böbrek naklinde hasta ve greft survisini etkileyen çok sayıda risk faktörünün olduğunu göstermektedir. Eski çalışmalarla kıyaslandığında immünpatolojinin daha iyi anlaşılması ile gelişen immünsüpresif tedaviler, cerrahi teknik başarısının artması ve hasta bakımının iyileşmesi ile gün geçtikçe greft ve hasta kaybını etkileyen risk faktörlerinin değiştiği görülmektedir. Değişen risk faktörlerinin saptanması, komplikasyonların erken tanınması, non-invaziv tanı yöntemlerinin geliştirilmesi böbrek naklinde başarımızın artacağını göstermektedir.

OBJECTIVE: Kidney transplant is the best treatment method for ESRD (end stage renal disease). There are many factors affecting allograft survival, in our study we aimed to examine the demographic data of the recipients, the primary etiology of chronic renal failure, pre-transplant HD (hemodialysis) / PD (peritoneal dialysis) treatments or preemptive transplantation, complications and the effect of immunosuppressive treatments on graft survival. MATERIALS AND METHODS: We observe retrospectively 126 kidney transplant patient who had applied Nephrology Transplantation Outpatient Clinic between 01 September 2020 and 01 November 2020 for follow-up and treatment continuation. RESULTS: The primary etiology of chronic renal failure patients are; 33 (26.2%) hypertension, 23 (%18,3) postrenal events, 17 (13.5%) glomerulonephritis, 15 (%11,9) diabetes mellitus, 5 (4%), recurrent urinary tract infections, 4 (%3,2) PCKD (polycystic kidney disease), 4 (%3,2) Familial Mediterranean Fever amyloidosis, 1 (0.8%) Fabry Disease and 24 (19%) unknown causes. We divided our patients according to living/cadaver donors and GFR (glomerular filtration rate) <60 ml/min/1.73 m2 / GFR≥ 60 ml/min/1.73 m2. The average age of the patients was 47.1 ± 10.8 years and a statistically significant difference was found between the groups with live-cadaver donors (p=0.033). The mean HD and PD duration of patients is 41.5 ± 55.6 and 6.9 ± 21.8 months, respectively, and the mean HD duration is statistically significant between groups with live-cadaver donors (p=0.0007), but the mean PD duration is not significantly different between groups (p=0.175). Between the groups GFR<60 ml/min/1.73 m2 and GFR≥ 60 ml/min/1.73 m2, a statistical difference was found in terms of mean HD time (p=0.024), but no statistical difference was found in terms of mean PD time (p=0.871). Delayed graft function was present in 18 (14.3%) patients and a statistically significant difference was found between the groups with live-cadaver donors (p=0.046) and no statistical difference was found between the groups GFR<60 ml/min/1.73 m2 and GFR≥ 60 ml/min/1.73 m2 (p=0.443). Acute humoral rejection, acute cellular rejection, chronic rejection, and CAN (chronic allograft nephropathy) in terms of live-cadaver donors no statistically significant difference was detected between the groups (p=0,585, p=0,140, p=0,517, p=0,552). 90 (71.4%) live donor and 36 (28.6%) cadaver donor patients were transplanted and there was no statistically significant difference between GFR<60 ml/min/1.73 m2 and GFR≥ 60 ml/min/1.73 m2 groups in terms of live-cadaver donor (p=0.321, p>0.999). Acute humoral rejection 10 (%7,9), chronic rejection 13 (%10,3) and CAN 15 (%11,9) of patients and was seen in GFR<60 mL/min/1.73 m2 - GFR≥ 60 mL/min/1.73 m2 and statistically significant difference found between in these groups (P=0.002, p=0,037, p=0,012) but acute cellular rejection was seen in 5 patients and no statistically significant difference was detected between the groups (p=0,065). CONCLUSIONS: In our study, HT in etiology, HD/PD duration before transplantation, retransplantation history, rejection attacks, especially acute cellular rejection and chronic rejection, CAN development, surgical complications, especially lymphocele, detection of malignancy after transplantation, class 1 and class 2 PRA positivity was found to negatively affect graft survival. Recent studies show that there are many risk factors affecting patient and graft survival in kidney transplantation. Compared to previous studies, it is seen that risk factors affecting graft and patient loss are changing day by day with immunosuppressive therapies developing with a better understanding of immunopathology, increased surgical technique success and improved patient care. Detection of changing risk factors, early diagnosis of complications and development of non-invasive diagnostic methods show that our success in kidney transplantation will increase.
